Book excerpt: The aortic stenosis is one of the most frequent valvular heart diseases, which is mostly diagnosed in older patients. With the symptoms onset, the lifespan of such patients dramatically decrease unless an aortic valve replacement is performed. However, a considerable proportion of such patients do not undergo cardiac surgery owing to a perceived high risk, being the advanced age one of main reasons to deny surgical aortic valve replacement. Such deficiency in the care of these patients has favoured the development of transcatheter aortic valve replacement or implantation (TAVR or TAVI), which has revolutionized the treatment of aortic stenosis. This treatment is now a routine therapy in high-risk patients with aortic stenosis, even if cardiac surgery is not contraindicated. Indeed, the last years have witnessed a shift in the use of this technology to lower risk populations. TAVR technology has experienced a dramatic evolution to integrate enhanced iterations which have allowed, along with the improvement of the technology, a reduction in the risk of complications associated with this therapy. However, several concerns remain. One of them is the occurrence of arrhythmias and conductions disturbances, in particular left bundle branch block and atrioventricular block, which are frequent complications of TAVR. Despite the evolution of the technology and development of new devices leading to a reduction in the overall rate of complications, no new iterations have been integrated to prevent the occurrence of such complications. Moreover, the use of new-generation devices seems to be associated with an increased risk of such complications, and therefore, its incidence is expected to increase in the next future. Nonetheless, little is known about the impact and evolution of these disorders. This thesis evaluates the incidence and evolution of new-onset arrhythmias and conduction abnormalities after TAVR and the impact of new-onset left bundle branch block and permanent pacemaker implantation on late clinical and echocardiographic outcomes.

Book excerpt: Aortic Valve Transcatheter Intervention Calcific aortic stenosis (AS) is the most common heart valve anomaly, with a largely age-dependent prevalence, a calculated annual incidence rate in the range of 4-5% in general populations and up to 6% in patients aged 75 years and over. Surgical aortic valve replacement (SAVR) was previously the only option available to patients with symptomatic, severe aortic stenosis. After the first-in-human transcatheter aortic valve implantation (TAVI) was performed by Alain Cribier in 2002, the treatment strategy for patients with symptomatic AS has been revolutionized. Since then, TAVI has grown exponentially, as a result of accruing evidence demonstrating safety and efficacy, and reduced invasiveness compared with SAVR. TAVI devices are continuously expanding to include several valve design options. As this strategy is continuously evolving to treat younger patients and lower-risk populations, aside from the long-term durability of the valve systems, procedural safety will become the focus of newer-generation devices. This book is a practical handbook devoted to the optimization of TAVI procedures, through a focused containment of complications. Through an integrated evaluation of the clinical status, imaging techniques and laboratory findings, the authors provide readers with clear messages on preventive and therapeutic recommendations.

Book excerpt: The advent of transcatheter aortic valve implantation (TAVI) has meant a paradigm shift in the treatment of aortic stenosis. TAVI has become the preferred treatment for patients at intermediate to high surgical risk and its expansion towards the treatment of low-risk patients is under study. During the last years, the successive improvements in transcatheter heart valve systems have led to a progressive reduction of periprocedural complications and death. However, some issues remain to be resolved. Conduction disturbances (CDs) such as new-onset left bundle branch block (LBBB) or high-degree atrioventricular block requiring permanent pacemaker (PPM) implantation have not decreased over time and remains the most frequent complication of the procedure. Whereas previous studies focused on the occurrence of CDs in the periprocedural period, scarce data exist on late (after discharge) episodes. On the other hand, the prevalence and clinical impact of new-onset tachyarrhythmias in patients following TAVI have been less studied, especially after the hospital discharge. Different drawbacks remain unsolved in this context (e.g. long-term ECG evolution in patients without ECG-CDs, management of new-onset LBBB, predictors of ECG regression in new-onset LBBB patients). Furthermore, continuous electrocardiographic monitoring has emerged as a useful tool to unravel arrhythmic disorders (either brady and tachyarrhythmias) in the early phase post-discharge, but data in the TAVI setting is scarce. Thus, their safety and clinical usefulness remain to be elucidated. The main objectives of this PhD research project are: (i) to assess the incidence and clinical impact of late arrhythmic disorders (post discharge) in TAVI recipients (ii) to demonstrate the safety and usefulness of non-invasive continuous ECG monitoring post-TAVI.
