Book excerpt: Despite the advances in conventional, novel agent and high dose chemotherapy multiple myeloma (MM) remains incurable. In order to overcome resistance to current therapies and improve patient outcome, novel biologically-based treatment approaches are being developed. Current translational research in MM focusing on the development of molecularly-based combination therapies has great promise to achieve high frequency and durable responses in the majority of patients. Two major advances are making this goal possible. First, recent advances in genomics and proteomics in MM have allowed for increased understanding of disease pathogenesis, identified novel therapeutic targets, allowed for molecular classification, and provided the scientific rationale for combining targeted therapies to increase tumor cell cytotoxicity and abrogate drug resistance. Second, there is now an increased understanding of how adhesion of MM cells in bone marrow (BM) further impacts gene expression in MM cells, as well as in BM stromal cells (BMSCs). As a result of these advances in oncogenomics on the one hand and increased understanding of the role of the BM in the pathogenesis of MM on the other, a new treatment paradigm targeting the tumor cell and its BM microenvironment to overcome drug resistance and improve patient outcome has now been developed. Thalidomide, lenalidomide, and Bortezomib are three agents which target the tumor cell in its microenvironment in both laboratory and animal models and which have rapidly translated from the bench to the bedside. Ongoing efforts are using oncogenomics and cell signaling studies to identify next generation of therapies in MM on the one hand, and to inform the design of combination trials on the other. This new paradigm for overcoming drug resistance and improving patient outcome in MM has great promise not only to change the natural history of MM, but also to serve as a model for targeted therapeutics directed to improve outcome of patients with MM.

Book excerpt: Expert physicians and clinical researchers summarize and explain all the recent advances in the biology and treatment of bone marrow-based malignancy. On the biological side, the authors show the characteristics of the malignant cell and describe the significant roles played by oncogenic changes, chromosomal anomalies, Kaposi's sarcoma herpes virus, and cytokines. New epidemiological findings and prognostic factors are also analyzed. On the clinical side, the authors provide a comprehensive review of conventional treatment regimens, as well as a discussion of newer experimental approaches involving immunologic targeting, inhibitors of drug resistance, and antitumor agents.

Available in PDF, EPUB and Kindle. Book excerpt: Multiple Myeloma: The second issue of Emerging Cancer Therapeutics focuses on multiple myelomas also known as plasma cell myeloma, KahlerÌs Disease and myelomatosis. It is estimated that over 20,000 new cases are diagnosed each year with over 10,000 people dying from these malignancies. Multiple myelomas are highly treatable but rarely curable. Thus it is crucial for the practitioner to be up-to-date on the latest insights regarding their management. Management options and outcomes for individuals with multiple myelomas improved dramatically with the introduction of chemotherapy. Even further improvements in prognosis have occurred because of the introduction of newer therapies such as pulse corticosteroids, thalidomide, bortezomib, and autologous and allogeneic stem cell transplantation. With the increased number of therapeutic options to choose from, the clinician is better placed to offer effective therapy but at the same time is challenged to keep abreast of the rapidly changing treatment landscape and the newly emerging data that is shaping the treatment options today and in the future. Multiple Myeloma provides a comprehensive and in-depth review of this group of malignancies. This volume reviews the current literature, provides critical evaluations of the data and offers evidence-based recommendations. About the Series: Emerging Cancer Therapeutics is an invited review publication providing a through analysis of key clinical research related to cancer therapeutics, including a discussion and assessment of current evidence, current clinical best practice, and likely near future developments. There is an emphasis throughout on multidisciplinary approaches to the specialty, as well as on quality and outcomes analysis. Book excerpt: Multiple myeloma is a plasma cell malignancy characterized by complex heterogenous cytogenetic abnormalities that accounts for 1.4% of all cancers, and approximately 10% of hematologic malignancies. The clinical manifestations of multiple myeloma include lytic bone lesions, cytopenia, hypercalcemia, renal dysfunction, hyperviscosity of the blood, immunodeficiency, and peripheral neuropathy. Based on the clinical and genetic data, probably all cases of multiple myeloma arise from an asymptomatic monoclonal gammopathy of unknown significance. The exact mechanism of the transition from MGUS to overt multiple myeloma is still not well understood. Recent oncogenomic studies have further advanced our understanding of the molecular pathogenesis of multiple myeloma. This book will give a comprehensive overview of the genetic and molecular epidemiology of multiple myeloma in order to get a more refined and conclusive understanding of this disease.

Chapters are written by international authorities and contain color photos, diagrams, and algorithms outlining preferred treatment strategies. Relevant scientific information is integrated throughout, but the focus here is on providing practical therapeutic guidance for oncologists and hematologists caring for myeloma patients. The book covers all myeloma subtypes and related disorders, including amyloidosis, Waldenstrom macroglobulinemia, plasmacytoma, MGUS, and POEMS syndrome.

Available in PDF, EPUB and Kindle. Book excerpt: Multiple myeloma is currently still an incurable disease, but during the past decade knowledge of its molecular pathogenesis has increased rapidly. This has led to remarkable progress in both diagnosis and therapy, including in particular the approval of novel and first-in-class drugs such as thalidomide, bortezomib, and lenalidomide. This book, written by internationally acknowledged experts, covers a wide range of topics relating to multiple myeloma, including history, epidemiology, pathophysiology, clinical features, staging, and prognostic systems. The principal focus, however, is on therapy, with detailed information on the various promising treatment options which give hope that this cancer will be transformed into a chronic disease or even become curable. Individualized therapy and the variety of supportive treatment options, as described in this volume, will help in achieving this goal, as well as in reducing adverse events and improving quality of life.
