The topics discussed included those dealing with the physi ology and pathophysiology of proteases and their inhibitors, the interactions of proteases and hormones, the kallikrein-kinin system, complement and the coagulation system, the function of proteases in the kidney and the intestinal tract as well as the role of proteases in lung diseases, pancreatitis, arthritis and hypercatabolic states (multiple trauma, septicemia, acute renal failure). The papers presented answered many questions, but raised many more concerning the significance of proteases and their inhibitors in clinical medicine. It was unfortunately impossible in this volume to in clude the extended, lively and extremely stimulating discussions which were enjoyed by the participants during the conference. The meeting has provided a unique framework for close inter action between scientists from various disciplines, including bio chemistry, physiology, surgery, anaesthesiology, endocrinology, hematology, pulmonology and nephrology. We would like to express our thanks and appreciation for all those who have stimulated, encouraged and supported us to hold this symposium in WUrzburg. When I was a medical student, lectures in cytology were a special, segregated part of the curriculum in the histology course, given along with general anatomy, and they were, as I recall, the surest of cures for insomnia. I still possess Cowdry's three-volume set entitled Special Cytology, published in 1934, and leafing through these books today is rather like examining a medieval manuscript. You could never have guessed what was going to happen to the field. At that time it was all structure, and all guesswork about the structure. When cells were packed together in various tissues, how did the geometry of packing work? How many sides did a liver cell have, in real life? What on earth were all those granules in side, and what were the best stains for looking at them? One thing about those granules, they never moved. Indeed, nothing moved. Cytology turned into cell biology much later on, and suddenly came alive. As has been the case in so many diSciplines in biology, it was brought to life by techniques. New instruments and cytochemical methods were devised for look ing at cells, manipulating cells, more or less in vivo.

Oxygen uptake increases markedly, frequently more than 50-fold; the phagocytes begin to produce large quantities of superoxide and hydrogen peroxide; and they immediately begin to metabolize large amounts of glucose by way of the hexose monophosphate shunt. This series of changes has become known as the respiratory burst. It was first believed that the major function of this respiratory burst was to generate powerful antibacterial agents by the partial reduction of oxygen. It is becoming apparent that the respiratory burst has much wider application, and its physiological function in many different biolog ical areas is clear. Here we describe the causative agents (shock) and the consequences (sepsis, organ failure) in two main sections, divided on the basis of their pathophysiology.
