Book excerpt: Negative pressure wound therapy (NPWT) involves applying a controlled sub-atmospheric pressure environment across the surface of a wound in an airtight dressing. A pump is used to maintain negative pressure, usually between 75 and 125 mmHg, in a consistent or intermittent manner. The mechanism by which NPWT is thought to promote wound healing is through increasing local perfusion, eliminating tissue edema, drawing wound edges together, removing exudates and proinflammatory cytokines, inhibiting bacterial growth, and promoting cell hyperplasia. NPWT systems have been widely adopted for a broad range of wound indications including diabetic foot ulcers (DFUs). In 2011, the FDA published a warning regarding contraindications and risk factors to consider before NPWT use. Application of NPWT on exposed organs, exposed vasculature, necrotic tissue with eschar present, untreated osteomylelitis, malignancy in the wound, or anastomotic sites is contraindicated. Risk factors for NPWT use include patients at high risk for bleeding and hemorrhage, patients with infected wounds, sharp edges in the wound, patient size and weight, and circumferential dressing application. This FDA update on serious complications was prompted by reports on 12 deaths and 174 injuries associated with NPWT use since 2007. NPWT is in widespread use, however, it is expensive and presents possible serious adverse events. The purpose of this report is to retrieve and review existing evidence of clinical effectiveness and cost-effectiveness, and to retrieve and review the existing guidelines for NPWT treatment of DFUs.

Therapy, KCI, San Antonio, Tex) were contrasted and standard dressings in 45 patients with diabetic foot ulcers who were admitted to the Department of Plastic and Reconstructive Surgery, Medical Park Hospital, Bursa, Turkey. Twenty-four patients were arbitrarily divided into 2 gatherings NPWT gathering and control group. At first, the mean surface range of wounds in the NPWT gathering was 109 cm2, the control aggregate 94.8 cm2. The mean length of time of open wound forethought was 11.25 days in the NPWT collection and 15.75 days in the control cluster (P =0.05). Not only is NPWT more cost effective, but it produces better patient outcomes for those with difficult to heal wounds. Diabetics are a vulnerable population that requires the best care possible and NPWT seems to be the obvious choice. It is time that more medical professionals consider NPWT as the chosen standard of care for diabetic ulcers.

Book excerpt: OBJECTIVES: To systematically review the efficacy and safety of negative pressure wound therapy (NPWT) for treatment of chronic wounds in the home setting. DATA SOURCES: On June 2014, we searched MEDLINE(r), Embase(r), the Cochrane Central Register of Controlled Trials, and the Cumulative Index to Nursing and Allied Health Literature (CINAHL(r)). REVIEW METHODS: Two independent reviewers screened search results. We included studies examining the use of NPWT in patients with chronic wounds, including venous leg ulcers, arterial leg ulcers, diabetic foot ulcers, pressure ulcers, and mixed etiology chronic wounds. We searched for comparative trials that followed subjects in the home setting. We extracted data into standardized forms and summarized results qualitatively. RESULTS: We retrieved 5,912 citations, and found seven studies which met our criteria for inclusion. Six of the studies compared NPWT devices to other wound care methods. One study compared two different NPWT devices. Data were limited by variability in the types of comparator groups, variable quality in study design, and limited reporting of outcomes. CONCLUSIONS: We were unable to draw conclusions about the efficacy or safety of NPWT for the treatment of chronic wounds in the home setting due to insufficient evidence. Though NPWT has been used across the wound care spectrum, significant research gaps remain. Standardization of wound care research protocols, such as providing consistency in comparator groups, robust randomized study designs, larger trials, and common definitions of outcomes, would be helpful in providing evidence to inform decisions about the use of NPWT.

Additionally, the cyclical application of sub-atmospheric pressure alters the cytoskeleton of the cells in the wound bed, triggering a cascade of intra-cellular signals that increases the rate of cell division and subsequent formation of granulation tissue. A combination of these mechanisms makes the NPWD therapy a versatile tool in the armamentarium of wound healing. NPWD is generally well tolerated and, with few contraindications or complications, is fast becoming a mainstay of current wound care.
