Available in PDF, EPUB and Kindle. Book excerpt: This is the first textbook devoted entirely to understanding and treating necrotizing enterocolitis (NEC), one of the leading causes of death and disability in premature infants. NEC continues to occur in neonatal units across the world, and the overall mortality has remained stubbornly high since its first description decades ago. Despite significant research into NEC, and a greater understanding of its underlying causes, there is no single source of information to which the care team can turn for guidance. This book fills that important gap in clinical care. In selecting the topics for this inaugural textbook, three guiding principles have been followed. First, to include chapters that provide detailed information for the medical team – the doctors and nurses, the therapists and pharmacists, the chaplains and the child life workers – so that each team member can optimally recognize, prevent and treat patients with this disease. Second, to ensure that chapters cover the depth and breadth of the latest clinical and scientific research into NEC, each selected to identify specific preventative strategies or therapies for this disease. Third, and perhaps most importantly, to focus not only on the child with NEC, but also on the child’s family, in order to provide comprehensive information about a disease that families have barely heard about, until the jarring day when their precious infant is affected by it. This book therefore: serves as a "how-to guide" for the care of the infant with NEC summarises critical new research and offers guidelines for future key research areas addresses the complex and difficult issues surrounding care of the critically ill infant suffering from NEC Written for the entire health care team including paediatric surgeons, neonatologists, developmental paediatricians, epidemiologists, ethicists, child life professionals, therapists and specialist nurses, each team member will find this book of value. This book was written to demystify this cruel disease, and to unlock its closely held secrets of pathogenesis, diagnosis, treatment and prevention.

NEC was viewed as a fait accompli; a sometimes-dire disease that was to be confronted only after it reared its perfidious head. This monograph asks the reader to rethink their

Book excerpt: Improved conditions of care for premature infants have led to markedly increased survival rates over the last few decades, particularly in very low and extremely low birth weight infants. Nutritional measures play a central role in the long-term outcome, health and quality of life of these premature infants. In this publication, leading experts from all 5 continents present the most recent evidence and critical analyses of nutrient requirements and the practice of nutritional care (with the focus on very low birth weight infants) to provide guidance for clinical application. After the introductory chapters, covering nutritional needs and research evidence in a more general manner, topics such as amino acids and proteins, lipids, microminerals and vitamins, parenteral and enteral nutrition as well as approaches to various disease conditions are addressed. Due to its focus on critical appraisals and recommendations, this book is of interest not only for the researcher who wants to keep up to date, but also for the clinician faced with premature infants in his practice.

The clinician needs to suspect at least one disease process as a starting point, because the book is ordered alphabetically according to diagnoses. From this point there are both surgical and imaging differential diagnoses listed. These can also be looked at within the book. For the clinician there is a dedicated page to assist with clinical symptoms and signs, alternative diagnoses and urgency of the radiological investigation, based on important information that is needed from imaging. With regard to imaging, there is a list of primary, follow-on and alternative investi- tions appropriate for the suspected diagnosis. There are lists of imaging features with s- porting images, tips and radiological differential diagnoses. The alphabetic organization makes for a jump to the next suspected diagnosis with ease to ? nd something more suitable for the current patient’s needs.

Available in PDF, EPUB and Kindle. Book excerpt: Necrotizing enterocolitis is an acute inflammatory necrosis of bowel that primarily afflicts premature infants in the neonatal intensive care unit setting. Although patients who develop this disease have high morbidity and mortality rates, the pathogenesis is poorly understood, and therefore there are no specific preventive or treatment strategies that have been clearly effective. Recent studies have suggested that the pathophysiology of necrotizing enterocolitis includes alterations in the inflammatory response leading to dysregulated pro-inflammatory signaling in premature infants, as well as abnormal intestinal bacterial colonization patterns that can activate these inflammatory pathways, and these factors are discussed in depth in the following chapters. While human milk feedings are currently the standard of care for the prevention of this challenging condition, new approaches will be described based on sound evidence that might have a significant impact for premature infants throughout the world.

Available in PDF, EPUB and Kindle. Book excerpt: Although pediatric surgery is a distinct and evolving specialty, it still remains an integral part of most general surgical and paediatric medical practice. Nevertheless, surgery in children does differ from adult practice in various fundamental ways, and there are key physiological and anatomical differences that constantly need underlining. Progress and improvement in outcome has also been rapid but it is sometimes difficult for practitioners to keep themselves up-to-date with the usual surgical or paediatric text books. This book will give a concise overview of all important topics and is designed to provide information in order to recognise the common surgical conditions; namely typical symptoms and signs, investigation and then treatment management. It will also provide an anatomical and physiological background to aid understanding, in addition to emphasising logical, and where possible, evidence-based practice by the use of flow charts, tables and algorithms. Authored by an international range of leading contributors, this is the first book of its kind to offer comprehensive coverage to this topic in a quick reference, pocket-book format.

Book excerpt: This book provides up-to-date, comprehensive, and accurate information on the diagnostic imaging of nontraumatic abdominal emergencies in pediatric patients. All of the most common neonatal and pediatric emergencies are covered, with separate discussion of diseases that occur more commonly in newborns and those typically encountered later in childhood. For each condition, the main signs observed using the various imaging techniques – X-ray, Ultrasonography, Computed Tomography, and Magnetic Resonance – are described and illustrated with the aid of a wealth of images. Attention is drawn to those features of particular relevance to differential diagnosis, and the prognostic value of diagnostic imaging is also explained. The final section addresses topics of special interest, including the acute onset of abdominal neoplasms, the problems associated with radiation protection in the emergency setting, and medicolegal issues and informed content. The book will be of value for all radiologists working in emergency settings in which pediatric patients (newborn and children accessing the emergency department) are regularly examined.
