Do you need a multimedia tool to guide you through all aspects of clinical management? Just as laparoscopic surgery revolutionized surgical practice in the 1980’s and 90’s, offering genuine competition to traditional open surgery, Natural Orifice Translumenal Endoscopic Surgery (NOTES) presents a genuinely different alternative for surgeons and patients alike as we move forward in the 21st century. Advantages over laparoscopic surgery include lower anesthesia requirements, faster recovery and shorter hospital stays, avoidance of transabdominal wound infections, less immunosuppression, better postoperative pulmonary and diaphragmatic function and of course, the potential for "scarless" abdominal surgery. Combining high-level text with a superb companion website, Natural Orifice Translumenal Endoscopic Surgery (NOTES): Textbook and Video-Atlas provides you with a hands-on, in-depth and practical multimedia tool on this exciting breakthrough, to consult either on the ward or in surgery. Highlights include: Full coverage of the fundamentals of NOTES, its current clinical applications and techniques, and future perspectives Over 50 outstanding videos illustrating NOTES being performed by the experts Full-color throughout and superbly illustrated with over 250 figures Edited by global leaders and pioneers in the field, who have recruited a world-class contributor team As well being a valuable text for gastroenterologists, GI surgeons and general surgeons, Natural Orifice Translumenal Endoscopic Surgery (NOTES): Textbook and Video-Atlas will also appeal to urologists, urologic surgeons, gynecologists, gynecologic surgeons, thoracic and ENT surgeons.

This book was released on 2009 with total page 156 pages. Available in PDF, EPUB and Kindle. Book excerpt: Natural Orifice Translumenal Endoscopic Surgery (NOTES) avoids skin incisions by accessing the abdominal cavity through natural orifices. Benefits include less pain, fewer complications and no scars. The aims of this study were to evaluate safety and efficacy of access techniques for natural orifice surgery, to investigate safety and efficacy of closure methodologies and to compare the inflammatory response induced by NOTES with that following conventional surgery. Access techniques were evaluated for safety and efficacy by measuring resultant injury and time required to access the peritoneal cavity in an acute porcine model. Four different gastrotomy closure modalities were evaluated for safety and efficacy by measuring clinical data, evidence of infection and closure integrity in a chronic porcine study. Markers of inflammation were measured in a chronic study comparing NOTES to conventional surgery. 70 anterior transgastric access procedures were performed without any serious injury to adjacent organs. NOTES access required significantly longer than laparoscopic access. Of the seven methods evaluated none was significantly superior to others. In distinguishing between safe and unsafe alternate access sites, endoscopic ultrasound (EUS) provided a statistically significant difference for antral and posterior stomach access points but not for rectal access. Gastrotomy closure time varied widely for the four techniques evaluated. No leak was detected with any method and the strength of closure for each technique was equivalent to control. Three of the four methods resulted in injury and evidence of infection was found in all groups. Postoperative elevation of inflammatory markers was not significantly different between NOTES and laparoscopy except in the case of cortisol, which was greater in the laparoscopic group. This study supports the safety of anterior transgastric access while demonstrating the potential use of EUS in minimizing risks of injury at alternate sites. Likewise, evidence for the reliability of numerous closure techniques was demonstrated with concerns for the safety. Additionally, inefficient access and closure data exposed the technical challenges facing NOTES practitioners. Most importantly, NOTES produced an inflammatory response which was not significantly greater than that produced by conventional laparoscopic surgery, validating its potential and emphasizing the need to overcome the aforementioned technical challenges.

Book excerpt: Biomedical Engineering in Gastrointestinal Surgery is a combination of engineering and surgical experience on the role of engineering in gastrointestinal surgery. There is currently no other book that combines engineering and clinical issues in this field, while engineering is becoming more and more important in surgery. This book is written to a high technical level, but also contains clear explanations of clinical conditions and clinical needs for engineers and students. Chapters covering anatomy and physiology are comprehensive and easy to understand for non-surgeons, while technologies are put into the context of surgical disease and anatomy for engineers. The authors are the two most senior members of the Institute for Minimally Invasive Interdisciplinary Therapeutic Interventions (MITI), which is pioneering this kind of collaboration between engineers and clinicians in minimally invasive surgery. MITI is an interdisciplinary platform for collaborative work of surgeons, gastroenterologists, biomedical engineers and industrial companies with mechanical and electronic workshops, dry laboratories and comprehensive facilities for animal studies as well as a fully integrated clinical "OR of the future". Written by the head of the Institute of Minimally Invasive Interdisciplinary Therapeutic Intervention (TUM MITI) which focusses on interdisciplinary cooperation in visceral medicine Provides medical and anatomical knowledge for engineers and puts technology in the context of surgical disease and anatomy Helps clinicians understand the technology, and use it safely and efficiently
