Book excerpt: Estimates of the health care expenses of the U.S. civilian noninstitutionalized (community) population are critical to policymakers and others concerned with access to medical care and the cost and sources of payment for that care. In 2007, health care expenses among the U.S. community population totaled $1.13 trillion. Medical care expenses, however, are highly concentrated among a relatively small proportion of individuals in the community population. As previously reported in 1996, the top 1 percent of the U.S. population accounted for 28 percent of the total health care expenditures and the top 5 percent for more than half. More recent data have revealed that over time there has been some decrease in the extent of this concentration at the upper tail of the expenditure distribution. Using information from the Household Component of the Medical Expenditure Panel Survey (MEPS-HC) for 2006 and 2007, this report provides detailed estimates of the persistence in the level of health care expenditures over time. Studies that examine the persistence of high levels of expenditures over time are essential to help discern the factors most likely to drive health care spending and the characteristics of the individuals who incur them. The MEPS-HC data are particularly well suited for measuring trends in concentration and persistence. All differences between estimates discussed in the text are statistically significant at the 0.05 level unless otherwise noted.

Using information from the Household Component of the Medical Expenditure Panel Survey (MEPS-HC) for 2008 and 2009, this Statistical Brief provides detailed estimates for the U.S. civilian noninstitutionalized non-elderly (under age 65) population that was uninsured for the entire 2006-2009 period and identifies groups most at risk of lacking any coverage during that four-year period. The inclusion of questions in MEPS to determine health insurance coverage profiles for the period covering 2006 and 2007, in concert with information on health insurance profiles from the National Health Interview Survey for 2007, facilitated these analyses of extended longitudinal profiles. Book excerpt: The Household Component of the Medical Expenditure Panel Survey (MEPS-HC) is designed to produce estimates of the health care use, expenditures, sources of payment, and insurance coverage among the U.S. civilian noninstitutionalized population. While the MEPS-HC was designed primarily to ensure reliable estimates at the national and regional level for a large variety of population subgroups, the survey design permits estimation with reasonable precision for certain measures in many states. Many factors can influence health care expenses in a particular state, including the demographic, socioeconomic, and health status characteristics of the population. Further, the prevalence and types of health insurance coverage in a state can impact access to care, the level of expenditures, and the extent to which different sources finance health care services. This Statistical Brief presents variations from the national average in health care expenses for the 10 most populous states in 2006. The Brief examines selected measures for the U.S. population, including 1) the proportion of the population with selected types of expenses, 2) the average amount of expenses, and 3) the distribution of payments for health care expenses across various sources. The 10 states presented in this Brief together comprised just over half of the U.S. population in 2006 and accounted for a similar share of the nation's health care expenses. The states are displayed in the order of population size. Only those estimates with statistically significant differences from the national average using a multiple comparison procedure at the .05 significance level are noted in the text.

Book excerpt: Highlights: Overall, the mean and median expenses of persons with any expenses in 2007 (84.9 percent of the population) were $4,404 and $1,206, respectively. -- In 2007, hospital inpatient expenses and ambulatory care, including office-based visits and hospital outpatient care, each accounted for nearly one third of total expenses for the U.S. civilian noninstitutionalized population, while prescribed medicines accounted for approximately one-fifth of total expenses. -- Hospital inpatient expenses comprised the largest single share of expenses for persons age 65 and over, while expenses for ambulatory care in office and hospital outpatient settings comprised the largest share for persons under age 65. -- In 2007, private insurance covered 42.4 percent of total expenses; individuals and family members paid 16.2 percent out of pocket, Medicare paid 24.8 percent, and Medicaid/CHIP paid 8.7 percent. While the proportions paid out of pocket did not differ markedly across age 1 groups, shares paid by public and private insurance sources varied considerably by age. -- Mean expenses for persons age 65 and over with an expense ($9,696) were more than double the amount for persons under 65 ($3,499). Uninsured persons under 65 had the lowest mean expenses while persons 65 and over with Medicare and other supplemental public insurance had the highest mean expenses.
