Available in PDF, EPUB and Kindle. Book excerpt: Based on his experiences of helping to fight cuts and closures in Suffolk, Michael Mandelstam delivers a damning verdict on the mismanagement of the NHS at national, regional and local level. He charts the widespread cutbacks and closures, both rural and urban, to clinics, A&E services, beds, wards and scores of community hospitals. He outlines how humane care, particularly for older people, is compromised by the ruthless determination of NHS management to increase patient throughput and hit government-set targets. The author highlights how the chaotic change to the NHS is being driven by concealed agendas - including privatisation of the NHS, obsessive interference from central government as well as selective use, if not abandonment, of evidence-based practice. Seriously flawed and damaging decisions are the result, affecting the population at large as well as those most vulnerable - older people with chronic and complex needs, people with physical or learning disabilities and people with mental health problems. Above all, he exposes the scandalous lack of transparency and accountability behind changes that threaten to destroy the NHS.

Available in PDF, EPUB and Kindle. Book excerpt: This authoritative guide to the law of continuing healthcare provides clarity on a contentious issue for those in long-term care: which adults are eligible for full NHS funding, as opposed to self-funded social care. Written by seasoned legal expert Michael Mandelstam, it provides practitioners with clear information on both the letter and spirit of the law, written in an accessible style suitable for a wide range of health and social care practitioners. The book gives all the need-to-knows in a handy A-Z format for quick reference, including key legal rules, guidance and case law. It contains also an extended analysis, with detailed evidence, of NHS continuing healthcare over the last 30 years up to the present. This is critical in order to understand why the rules are so complex, confusing and sometimes disregarded, and why decisions can seem counter-intuitive, unfair and difficult to challenge. The book is essential reading to assist the making of decisions that are fair, lawful and transparent.

Available in PDF, EPUB and Kindle. Book excerpt: With health and social care increasingly asked to work in partnership, many existing textbooks and educational opportunities are too 'uni-professional' to be able to respond fully to the joined-up services agenda. Against this background, this book provides a comprehensive and up-to-date analysis of both health and social care. With chapters on the origins of community health and social care; current services; partnership working; direct payments; independent living; anti-discriminatory practice; user involvement; and support for carers, the book is also packed with student-friendly features. These include a focus on both health and social care in a time of increasing inter-agency working; a combined summary of current policy and practice dilemmas with useful theoretical frameworks; service user-focused case studies and reflective exercises to aid further study and analysis; comparative material throughout exploring relevant issues, their impact and lessons learned and compliance with the Social Policy Quality Assurance Agency (QAA) for Higher Education benchmark statement. With partnership working now part of core business rather than an optional extra, this book is essential reading for anyone studying or working in health and social care. It provides practical material to populate the theoretical and conceptual knowledge of social policy students, and conceptual material to help make sense of the practical experience of professional students on training courses. Health and social care practitioners will find the book of value, informing their continuing professional development.

Available in PDF, EPUB and Kindle. Book excerpt: This review incorporates the views and visions of 2,000 clinicians and other health and social care professionals from every NHS region in England, and has been developed in discussion with patients, carers and the general public. The changes proposed are locally-led, patient-centred and clinically driven. Chapter 2 identifies the challenges facing the NHS in the 21st century: ever higher expectations; demand driven by demographics as people live longer; health in an age of information and connectivity; the changing nature of disease; advances in treatment; a changing health workplace. Chapter 3 outlines the proposals to deliver high quality care for patients and the public, with an emphasis on helping people to stay healthy, empowering patients, providing the most effective treatments, and keeping patients as safe as possible in healthcare environments. The importance of quality in all aspects of the NHS is reinforced in chapter 4, and must be understood from the perspective of the patient's safety, experience in care received and the effectiveness of that care. Best practice will be widely promoted, with a central role for the National Institute for Health and Clinical Excellence (NICE) in expanding national standards. This will bring clarity to the high standards expected and quality performance will be measured and published. The review outlines the need to put frontline staff in control of this drive for quality (chapter 5), with greater freedom to use their expertise and skill and decision-making to find innovative ways to improve care for patients. Clinical and managerial leadership skills at the local level need further development, and all levels of staff will receive support through education and training (chapter 6). The review recommends the introduction of an NHS Constitution (chapter 7). The final chapter sets out the means of implementation.

Available in PDF, EPUB and Kindle. Book excerpt: The Future of Nursing explores how nurses' roles, responsibilities, and education should change significantly to meet the increased demand for care that will be created by health care reform and to advance improvements in America's increasingly complex health system. At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: People have always travelled within Europe for work and leisure, although never before with the current intensity. Now, however, they are travelling for many other reasons, including the quest for key services such as health care. Whatever the reason for travelling, one question they ask is "If I fall ill, will the health care I receive be of a high standard?" This book examines, for the first time, the systems that have been put in place in all of the European Union's 27 Member States. The picture it paints is mixed. Some have well developed systems, setting standards based on the best available evidence, monitoring the care provided, and taking action where it falls short. Others need to overcome significant obstacles.
