Book excerpt: Visual inspection of this dam and engineering analyses revealed that the poor condition of the dam creates a hazard to human life and property. As a result of these determinations, the dam has been assessed as unsafe, non-emergency. One of the spillway sections has failed completely, creating a large void in the dam. The other spillway channel is in poor condition, having cracks in the concrete and several small voids. The earth fill upstream of the dam completely blocks the failed spillway section and partially blocks the other section. Due to the serious nature of the deficiencies, it is recommended that within 30 days of the notification to the owner that the dam has been assessed as unsafe, repair work on the structural deficiencies should be commenced. These repairs should be completed within 3 months. Within 3 months of the notification of the owner, a detailed hydrologic/hydraulic investigation should be commenced. This investigation should accurately define the site specific characteristics of the watershed and determine appropriate mitigating measures to be taken in response to the seriously inadequate spillway capacity. These measures should be completed within 1 year of the date of notification.

Book excerpt: On the basis of the visual examination, there were no signs of impending structural failure or other conditions which would warrant urgent remedial action; however, a number of serious deficiencies were noted. Since there were no drawings available, the evaluation of this dam is based primarily on visual examination, limiting measurements at the site, approximate hydraulic and hydrologic computations, and application of engineering judgement. It is recommended that the following additional investigations be performed by a registered professional engineer engaged by the owner: (1) The present embankment sections adjacent to the spillway represent a potential hazard to the dam with respect to possible erosion, piping and/or failure of the dam during high reservoir levels. Therefore, regrade this to make the embankment section conform to the embankment cross sections elsewhere. In addition, adequately protect these sections from spillway outflows; and (2) Investigate the seepage that was observed near the downstream toe of slope, including observation during high and low reservoir levels, evaluate the cause and recommend appropriate remedial measures.

Due to the appreciable size of Eaton Reservoir, the breaching of that dam would undoubtedly have a serious effect on Clayton's Dam. Similarly, the failure of either of these dams would have a serious effect on two small dams also on Eaton Brook, located 0.5 miles and 1.7 miles downstream of Clayton's Dam.

Book excerpt: Visual inspection of this dam and engineering analyses which have been performed revealed that several serious deficiencies exist on this structure. The worst of these deficiencies was the substantial amount of leakage through the dam. There were several areas of concentrated leakage along the interface between the dam and its foundation. Concentrated leakage such as this could adversely affect the stability of the structure. Immediately upon receipt of this notification, a system for providing around-the-clock surveillance of the dam during periods of unusually heavy precipitation should be developed and implemented. Structural stability analyses, based on available information, indicate that factors of safety against sliding are less than desireable for all conditions analyzed. When the dam is subjected to severe loading conditions such as an ice load or a water level at the top of the dam, the factors of safety fall to critical levels. It is recommended that within 3 months of the date of notification of the owner, investigations into the deficiencies on this structure should be commenced. An investigation into the leakage problem is required to determine a means of eliminating or controlling the leakage. Further studies of the structural stability of this structure are also required. Additional hydrologic and hydraulic investigations are required due to the seriously inadequate spillway. Remedial measures deemed necessary as a result of these studies should be completed within 18 months.

Based on the evaluation of the existing conditions, the condition of Alexander Lake Dam is considered to be fair. A minor seepage area and some wet areas are located along the downstream toe. The vertical and horizontal alignment of the dam crest is irregular. However, the observed conditions are not consistent to significantly affect the overall performance of the dam at this time. The owner's representative reported that the low level outlet pipe for the embankment was plugged by concrete prior to the filling of the dam, as required by the design drawings. Therefore, the dam has no functional low level outlet facilities that could draw down the reservoir in the event of an emergency. The spillway capacity was evaluated according to the recommended procedure and was found to pass 80 percent of the Probable Maximum Flood (PMF) without overtopping the dam and full PMF with a minor overtopping of the embankment. The spillway capacity of the dam is rated to be inadequate because the spillway cannot pass the recommended spillway design flood of full PMF without overtopping the dam. (Author).

Dam overtopping, the resulting erosion of the embankment and hence, dam breaching would cause water surface levels downstream to reach depths which would pose significant danger to residents. Therefore, the spillway is adjudged to be seriously inadequate and the dam is assessed as unsafe, nonemergency.
