Book excerpt: Examination of documents and visual inspection of Inghams Dam and appurtenant structures did not reveal conditions which would constitute a hazard to life or property. The dam has a number of problem areas which require further investigation and remedial action. These areas include: (1) Additional hydrologic investigations are required to more accurately determine the site specific characteristics of the watershed. It has been determined that the spillway embankment would be overtopped for all storms exceeding approximately 24% of the Probable Maximum Flood (PMF) spillway capacity = 21,000 cfs. The spillway is adjudged as 'seriously inadequate' and the dam is assessed as unsafe, non-emergency; (2) The stability analysis conducted in 1970 did not include the condition of ice loading pressures on the dam. Additional investigation is required to determine the type and extent of remedial action required; (3) The reservoir drain has not been operated since completion of the dam. The drain should be investigated and returned to operating condition; and (4) An inspection of the spillway should be conducted during no flow conditions to assess the integrity of the spillway.

Book excerpt: The Ephratah Dam is 760 feet long, buttressed concrete and cyclopean masonry dam containing an ogee spillway, 3 arched sections, a concrete gravity section and an earth embankment section. The maximum height of the dam is 65 feet. The ogee spillway is 251 feet long and has a crest elevation of 974.15. A 3 feet diameter reservoir drain is located at the base of the middle arched section. The flow of the drain is controlled by a manually operated butterfly valve located in the building at the toe of the dam. The power generation intake system is located on the upstream face of the south abutment. A 6.5 feet diameter wood stave pipe carries water from the intake 2 miles to the power plant. All regulating outlets are operational. The concrete portions of the dam exhibit signs of major deterioration as indicated by the extensive seepage which appears on the downstream face. Gunite and epoxy injection treatments have failed to control the flow. The discharge capacity of the spillway is inadequate for all flow in excess of 17% of the Probable Maximum Flood (PMF). The spillway is not considered seriously inadequate, based on the Corps of Engineer's Screening Criteria, since the dam is a gravity structure and the stability analyses indicate that the dam is not unstable during a PMF event.

Maximum height of the embankment above the old stream bed which is located below the spillway is 23 feet. The crest of the embankment is 24 feet wide, upstream slope is 1 vertical on 2 horizontal and the downstream slope is 1 vertical on 2.25 horizontal. The elevation of the embankment is 1710.0. The top of the south embankment section is totally exposed whereas the north section is heavily riprapped. Slopes are protected by riprap. Examination of available documents and visual inspections of the dam did not reveal conditions which constitute an immediate hazard to human life or property. However, additional studies should be undertaken to further evaluate condition affecting the dam.

However, the dam has some deficiencies which require remedial work. The hydrologic/hydraulic analysis indicates that the dam will be overtopped by flows in excess of 48 percent of the Probable Maximum Flood (PMF). The dam will be overtopped by 4.9 feet and 0.09 feet by the PMF and 1/2 PMF respectively. Failure of the dam during the 1/2 PMF event would significantly increase the downstream hazard from that which would exist just prior to failure of the dam. The spillway capacity, therefore, is assessed as seriously inadequate and the dam is assessed as unsafe, non-emergency. The structural analysis for the concrete spillway section indicates satisfactory stability for all loading conditions investigated.

It is, therefore, recommended that within 6 months of notification to the Owner, a detailed hydrologic/hydraulic investigation of the structure should be undertaken to more accurately determine the site specific characteristics of the watershed and their effect upon the overtopping potential of the dam. The results of these investigations will determine the appropriate remedial measures which will be required to achieve a spillway capacity adequate to discharge the outflow from at least the 1/2 PMF. In the interim, a detailed emergency action plan must be developed and implemented during periods of unusually heavy precipitation. Also, around-the-clock surveillance of the structure must be provided during these periods.

The Phase I inspection of the Marcy Reservoir Dam No. 11 did not indicate conditions which would constitute an immediate hazard to human life or property. However, the dam has some deficiencies which require further investigations and remedial work. A large area of seepage exists on the southwest slope of the embankment. This seepage extends approximately 250 feet along the embankment, and reaches are elevations of 17 feet below the crest of the dam. A detailed investigation should be commenced within three months to determine the source and quantity of flow in the area of seepage. The hydrologic/hydraulic analysis indicates that the spillway system is capable of passing the 1/2 PMF with 1.25 feet of freeboard. But, the PMF will overtop the concrete box at the downstream end of the spillway and run down along the embankment toe. Therefore, the spillway is assessed as inadequate according to the Corps of Engineers' screening criteria.

The classification of 'unsafe' applied to a dam because of a 'seriously inadequate' spillway means that there appears to be a serious deficiency in spillway capacity and if a severe storm were to occur, over-topping and failure of the dam could take place, significantly increasing the hazard to loss of life downstream of the dam. It is recommended that within 3 months of notification to the owner, detailed hydrological/hydraulic investigations of the structure should be undertaken to more accurately determine the site specific characteristics of the watershed and their effect upon the overtopping potential of the dam. Seepage found in and around the spillway during the inspection was of major concern.

This capacity is 28% of the Probable Maximum Flood and 56% of the 1/2 Probable Maximum Flood. The spillway is inadequate to pass the 1/2 PMF without overtopping the dam. The westerly section, Dam B, is unstable under all of the loading conditions prescribed by the Corps of Engineers' screening criteria. Therefore, the spillway structure is determined to be seriously inadequate according to the Corps of Engineers' screening criteria. The dam break flood wave is adjudged to have a significant impact on the residential structures 1/2 mile downstream. The spillway is, therefore, seriously inadequate and the dam is assessed as unsafe, non-emergency. Additional structural investigations involving borings should be undertaken to determine the affect of uplift pressure on the base of the dam and provide further analysis of the structural stability of the dam.

The area immediately upstream from the spillway is filled in with sediment and the remains of a previously existing timber crib dam. The right spillway abutment is adjacent to an intake and gate/powerhouse which has been abandoned. The examination of documents and the visual inspection of Harrower Pond Dam revealed conditions which constitute an immediate hazard to human life and property. The condition of the dam is considered to be 'unsafe, emergency', for several reasons. As a result of the 'unsafe, emergency' conditions of the dam, a 'Summary Abatement Order' was issued by the Commissioner of the New York State Department of Environmental Conservation on May 23, 1980.
