However, the dam has some deficiencies which require further investigation and remedial action. Using the Corps of Engineers screening criteria for review of spillway adequacy, it has been determined that the dam would be overtopped for all storms exceeding approximately 75 percent of the PMF. The spillway is, therefore, considered to be 'inadequate'. A review of the structural stability analysis performed by the designer indicated calculated safety factors against sliding to be less than recommended minimum value. A more detailed analysis which was performed as a part of this Phase I investigation produces a similar conclusion. Therefore, it is recommended that within 6 months of notification to the owner a detailed field investigation and monitoring program be undertaken to determine the actual hydrostatic uplift pressures along the base of the dam and that further detailed structural stability analyses be performed using this information. Debris should be removed from the upstream face of the dam as part of the routine maintenance program. Finally, leakage along the south side of the south tainter gate should be monitored on a daily basis with these observations documented in the dam operator's log book.

Book excerpt: Visual inspection of this dam and engineering analyses which have been performed revealed that several deficiencies exist on this structure. The worst of these deficiencies was the localized spalling and erosion of the gunite covering the dam. This was observed in three areas on the downstream face and toe. A more thorough examination of the gunite when there is no discharge over the spillway is needed to evaluate the extent and cause of this deterioration. If the gunite is not repaired, the interior concrete of the dam will become exposed to erosion. Using the Corps of Engineer's Guidelines for the initial review of spillway adequacy, it has been determined that the structure would be overtopped by all storms exceeding 31 percent of the Probable Maximum Flood (PMF). During a one-half PMF storm, the abutments of the dam would be overtopped by 3.4 feet. The dam is a concrete arch-gravity structure with bedrock at both abutments. While overtopping is not likely to induce failure from erosion, a stability analysis indicates that the factor of safety is below the recommended level during the one-half PMF storm. If the dam were to fail, it would increase the degree of flooding in downstream residential areas. Therefore, the spillway is assessed as seriously inadequate.

The structural stability analyses indicate that the factors of safety against both overturning and sliding are less than desireable. When the dam is subjected to severe loading conditions (ice load, flood flows), the safety factors fall to critical levels. Further investigation of the stability is needed including subsurface investigations and concrete coring. This information should then be incorporated into a detailed stability evaluation. Appropriate modifications to the dam should then be made. It is recommended that within 6 months of the date of notification of the owner these investigations should be commenced, within 18 months, necessary modifications to improve the stability of the structure should be completed. The hydrologic/hydraulic analysis performed indicates that the spillway does not have sufficient capacity to discharge the peak outflow from one-half the Probable Maximum Flood (PMF). However, a high tailwater condition could be expected for this storm event and a dam failure would not significantly increase the hazard to loss of life from that which would exist just before an overtopping induced failure. Therefore, the spillway capacity of this structure has been rated as inadequate.

The structural analysis indicates unsatisfactory stability against overturning according to the Guidelines criteria for the cases of the dam subject to forces possible during normal operation (including ice loading) the 1/2 PMF, and the PMF conditions. Under each of these conditions, the resultant of forces acting on the dam is located outside the middle third of the base indicating that tensile stresses would develop in the dam section. Computations prepared according to the Corps of Engineers' Recommended Guidelines for Safety Inspection of Dams establish the spillway capacity as 65% of the Probable Maximum Flood and 130% of the 1/2 Probable Maximum Flood. The hydrologic/hydraulic analysis indicates the spillway would pass the 1/2 Probable Maximum Flood but would be overtopped by one foot during a Probable Maximum Flood event. According to the Guidelines, the spillway is inadequate.
