Book excerpt: Warner Dam is owned by William S. Newlin, Jr., and is classified as a 'Low' hazard - 'Small' size dam. The dam was found to be in fair overall condition at the time of inspection. Hydraulic/hydrologic evaluations revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 50- year flood to the 100-year flood is required for Warner Dam. The 100-year flood was chosen as the SDF. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately develop remedial measures to provide adequate spillway capacity. The inspection revealed certain items of remedial work which should be performed by the owner without delay. These include: (1) Provide adequate spillway capacity; (2) Repair the spalling on the downstream face of the concrete corewall; (3) Ensure that the outlet works are operable, or develop an emergency drawdown plan; and (4) Cut all trees on the embankment at ground level. All trees with a trunk diameter greater than 3 inches should have their root systems removed. All resultant areas of erosion and cavities should be filled, graded, compacted and seeded. In addition, the following operational measures are recommended to be undertaken by the owner: (1) Develop a detailed emergency operation and warning system; (2) During periods of unusually heavy rainfall, provide around-the-clock surveillance of the dam; and (3) When warning of a storm of major proportions is given by the National Weather Service, activate the emergency operation and warning system.

The auxiliary spillway is an excavated channel along the left abutment of the dam. It is a grass-linked excavation in earth. Based on visual inspection, available records, calculations and past operational performance, PA-467 Dam is judged to be in good condition. The existing spillway can pass the Probable Maximum Flood(PMF) without overtopping of the dam. The spillway capacity is rated as adequate. There is a slope stability analysis for the embankment, and it indicates that the embankment has adequate factors of safety. There is no evidence of significant problems threatening the embankment.

A light rain was falling at the time of inspection. The dam and appurtenant structures were found to be in poor overall condition at the time of inspection. Seepage was observed at three locations passing through deteriorated portions of the concrete core wall. These seepage areas are the result of progressive deterioration of the exposed areas of the concrete core wall. The seepage has caused erosion gullies and slight irregularities on the downstream face of the dam.

Book excerpt: Based on visual inspection (18 July 1979), available records, calculations, past operational performance, and according to criteria established for these studies, Lake Montrose Dam is judged to be unsafe, nonemergency, because the spillway capacity is rated as seriously inadequate. The existing spillway can pass only 17% of the Probable Maximum Flood (PMF) without overtopping of the dam. Based on the type of construction and the condition of the dam, it is judged that the dam could not withstand the depth and duration of overtopping that would occur for the PMF or the 1/2 PMF. Failure of the dam would cause an increased hazard to loss of life downstream. As a whole, the dam is judged to be in poor condition. The spillway has undergone a gradual structural failure that has resulted in a reduction of spillway capacity. Continuation of the gradual failure, or perhaps a sudden failure, appears to be possible. There are no functional outlet works for the dam. Maintenance of the project is inadequate. There is no program for regular inspection of the dam.

The design drawings show the design freeboard to be five feet, but the available freeboard is only approximately two feet. It also appears that fill has been placed on the downstream slope, increasing the crest width from the design value of 10 feet to about 30 feet near the center line of the dam and greater than 50 feet near the left abutment. The dam has no functional low level outlet facility.

The western stream contains Romobe Lake (PennDER I.D. No. 58-10) which is 1750 feet upstream from Hathaway Pond Dam. The eastern tributary contains Ball Lake and an unnamed smaller pond in series. Ball Lake is 2450 feet upstream and will retain considerable storage while the smaller unnamed pond was considered to have no effect on Hathaway Pond Dam. Two homes, a trailer, and township road are located 1800 feet downstream from Hathaway Pond Dam. These could suffer economic damage in the event of a dam failure.

The size classification of the facility is intermediate and its hazard classification is considered to be high. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 45 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency. Calculations also indicate that if the spillway were constructed in accordance with available desing drawings, the facility could pass and/or store approximately 63 percent of the PMF.

A spillway design flood (SDF) in the range of the 1/2 PMF to the Probable Maximum Flood (PMF) is required for Page's Lake Dam. The 1/2 PMF was chosen as the SDF because the dam is on the low end of the 'Intermediate' size category in terms of storage. During the 1/2 PMF, the dam is overtopped by a maximum depth of 3.34 feet for a total duration of 11.83 hours. The spillway is therefore considered 'Inadequate.' In accordance with the guidance provided, the spillway design flood (SDF) ranges between 1/2 the PMF and the full PMF. Based on the size and extent of downstream hazard for the dam, the SDF selected for this facility was the PMF. The hydrologic and hydraulic computations indicate that the combination of reservoir storage and spillway discharge capacity will pass 88% of the PMF prior to overtopping the embankment. Therefore, in accordance with the recommended criteria for Phase I inspections, the spillway for Longford Lake Dam is considered to be inadequate.

Consequently the spillway is assessed as being inadequate, but not seriously inadequate. Deficiencies noted by the inspection team included the following: (1) large trees rooted within the downstream embankment face; (2) a damaged and inadequate spillway discharge channel; and (3) a bent gate stem and partially obstructed outlet associated with the blowoff conduit.
