It is recommended that the owner: (a) Develop a formal emergency warning system to notify downstream residents should hazardous conditions develop. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy precipitation. (b) Observe the cracking in the spillway overflow wall and outlet conduit headwall in future inspections and take remedial measures if necessary. (c) Develop formal manuals of operation and maintenance to ensure the continued proper care of the facility.

The dam consists of an earth embankment approximately 460 feet long with a maximum height of 38 feet from the downstream toe and a crest width of 14 feet. The flood discharge facilities for the dam consist of a drop inlet primary spillway located at the center of the embankment and an emergency spillway on the left abutment (looking downstream). The primary spillway structures consist of a two-stage reinforced concrete riser, a 24-inch-diameter reinforced concrete conduit, and a reinforced concrete impact basin at the downstream end of the outlet pipe. The emergency spillway is a 200-foot-wide trapezoidal earth channel excavated into the left abutment. The reservoir outlet for the dam consists of a 15-inch steel pipe extending from the upstream toe of the dam to the drop inlet structure. Flow through the reservoir outlet is controlled by a manually operated sluice gate lcoated in the drop inlet structure.

The dam is constructed with a masonry spillway located about 35 feet left of the right abutment. The crest of the spillway is 100 feet long and about 11 feet below the top of its wingwalls. Based on a visual inspection, operational history, and available engineering data, the dam is consisered to be in poor condition. The defunct facility has been essentially abandoned since 1952 and its reservoir is sediment filled. Results of the hydrologic and hydraulic analysis indicate that the facility is capable of passing and/or storing only 36 percent of the PMF without embankment overtopping. Structural deficiencies noted by the inspection team included substantial embankment overgrowth, an access road cut through the embankment near the left abutment, cracking of the masonry spillway wingwalls, and a sediment filled reservoir. Due to the seriously inadequate spillway and overall poor condition, the facility is considered unsafe but non-emergency as failure is not considered imminent.

The overflow section is approximately 15 feet wide and 3 feet deep and discharges onto an approximately 30-foot-long concrete chute. According to the previous inspection reports, the outlet facilities consist of two cast-iron pipes 4 and 6 inches in diameter. the location of the pipes and the manner in which flow through these pipes is controlled is unknown. In the design drawings, flow through these pipes is shown to be controlled by valves located in a valve chamber on the downstream side of the dam. However, this valve chamber could not be located at the site during this inspection. As it presently exists, the dam has no functional emergency drawdown facilities. (Author).

Book excerpt: Based on the evaluation of the conditions of Musser Forests Dam, the dam is considered to be unsafe due to severe erosion in the primary spillway and emergency discharge channel along the toe of the dam near the left abutment and due to the lack of an operable outlet works for the reservoir. However, it is not considered to be an emergency condition. Field observations indicate that the crest of the dam is irregular with elevations ranging from 1.7 to 5.9 feet above the emergency spillway crest level. The irregularities do not appear to be due to recent movements. The upstream slope of the dam has no erosion protection and has been partially eroded by wave action. The emergency spillway approach channel is overgrown with tall grass and cattails which may constitute a partial obstruction to flow through the spillway. The flood discharge capacity of Musser Forests Dam was evaluated and found to pass 30% of the probable maximum flood (PMF) without overtopping the low spot on the crest of the dam. Therefore, according to the recommended criteria, the flood discharge capacity of Musser Forests Dam is classified to be inadequate.

The crest width is approximately 10 feet. As designed, the downstream face of the dam was to be protected by grass, and a vertical concrete wall on the upstream side was to provide shoreline protection. Presently, the downstream face of the dam is covered with brush and trees up to 10 feet high and the concrete wall on the upstream side has partially collapsed. The spillway structures are comprised of an 18-foot-wide concrete ogee overflow section which discharges into an 18-foot-long concrete channel, incorporating an energy dissipator sill. Based on the evaluation of the existing conditions, the condition of Sample Run Dam is considered to be poor. The dam is abandoned and is in complete disrepair. A valve house located at the downstream toe of the dam has collapsed, covering most of the operating equipment with rubble. The flood discharge capacity of the dam was evaluated according to the recommended procedure and was found to pass 20 percent of the probable maximum flood (PMF).

The emergency spillway is capable of discharging the peak inflow resulting from a storm of PMF intensity and is, therefore, considered adequate. It is recommended that the owner: (a) Repair the damaged pond drain gate control and replace the missing manhole cover atop the service spillway riser. Consideration should be given to redesigning the gate control mechanism to protect it from future vandalism. (b) Consult with the U.S.D.A., Soil Conservation Service, to assess the erosion on the upstream slope in the area of the service spillway riser and implement any remedial measures deemed necessary. (c) Curtail the unauthorized vehicular travel across the embankment and repair and reseed any damaged (bare) areas. If control of unauthorized travel is not practical, some provision should be made to protect the embankment from rutting and erosion by placing a layer of gravel or stone across the crest. (d) Develop a formal operation and maintenance manual to ensure the continued proper care of the facility. In addition, a formal warning system should be implemented which provides detailed procedures to protect the lives and property of downstream residents. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy percipitation.

Book excerpt: Pond No. 4 Dam is classified as a 'small' size, 'significant' hazard dam, with a recommended 1/2 PMF (Probable Maximum Flood) spillway design flood. Based on the review and evaluation of available design information and visual observations of conditions as they existed on the dates of the field reconnaissances, the general condition of Pond No. 4 Dam is assessed poor. This evaluation is specifically based on a steep upstream embankment slope, sparse vegetation cover on the crest and upstream slope, and minor slope distress and tree and woody shrub growth on the downstream embankment. Seeps located along the downstream embankment toe also represent a potential hazard and warrant periodic observation by the dam owner. In addition, the unprotected spillway channel bottom and sideslopes may also present hazard to the dam during large spillway discharges. Analysis using the HEC-1 Dam Safety version computer program indicated the spillway channel section can pass the recommended 1/2 PMF spillway design flood without overtopping the embankment crest. Spillway discharge capacity is therefore considered adequate. (Author).

Book excerpt: Based on the evaluation of existing conditions, the condition of Rossiter Dam is considered to be unsafe/nonemergency due to a seriously inadequate spillway capacity. The overall condition of the dam is considered to be poor. The spillway structures have partially collapsed and the dam is overgrown with large trees and thick brush, precluding a complete inspection. The spillway capacity was evaluated according to the recommended criteria and was found to be seriously inadequate. According to the recommended criteria, small dams in the high hazard category are required to pass from one-half to full Probable Maximum Flood (PMF). In view of the small size of the dam, one- half of the PMF was selected as the spillway design flood. Flood discharge capacity was evaluated according to the recommended procedure and was found to pass 20 percent of the PMF without overtopping the embankment. Since the spillway capacity is less than 50 percent of the PMF and results of a breach analysis indicate that the potential loss of life and downstream damage would be significantly increased due to a dam failure, the spillway is classified to be seriously inadequate.
