Based upon visual inspection of the dam and review of the drawings provided by the Pennsylvania Department of Environmental Resources (DER), North Arrowhead Lake Dam is considered to be in poor condition.

The dam was built in 1914 over a preexisting dam, Lower Dorney Dam, and was reconstructed in 1961 by the Pennsylvania Fish Commission. The facility is considered to be in fair condition. The dam is classified as a 'High' hazard structure consistent with its potential in the event of failure to cause extensive property damage and possible loss of life to residents in the Arrowhead Lake development. The dam is classified as an 'Intermediate' size dam by virtue of its 1,213 acre-foot normal storage capacity. Design documentation was extremely limited. There were no as-built drawings and records indicate that, by today's standards, compaction of the embankment would have been considered, at best, marginal. However, the embankment slopes appear to be reasonable; there were no observed signs of slope instability and the embankment has performed satisfactorily for 65 years. Therefore, the embankment is at least marginally stable.

Book excerpt: North Lake Dam is located on a branch of Hornbecks Creek in Delaware Township, Pike County, Pennsylvania. The facility is located approximately 1,500 feet upstream and west of wild Acres Lake, and about five miles from U.S. Route 209 which parallels the Delaware River in this area. Observations made during the visual inspection indicate the embankment is in fair condition, but in need of general maintenance. No evidence of seepage through the downstream embankment face, sloughing, erosion or animal burrows was observed. The embankment crest is noticeably low (in excess of 1-foot) near the spillway relative to the remainder of the crest. About two-thirds of the embankment is overgrown along an area extending approximately 400 feet to the right of the spillway. The vegetation is characterized as primarily brush and high weeds with clumps of small trees strewn throughout. In contrast, much of the embankment crest and upstream face are bare and unprotected with only light vegetation visible. Several long tire ruts scar the crest near the spillway. A small area characterized by swamplike vegetation is located about 350 to 375 feet from the right abutment along the downstream embankment toe. Seepage was observed at the downstream embankment toe near the outlet conduit flowing at a rate of about two to three gpm. The flow is suspected of being either seepage through the embankment foundation, or perhaps drainage from a concealed toe drain. A precise assessment of the condition is difficult without adequate design data are drawings.

Based on visual observations and review of the information obtained from the Pennsylvania Department of Environmental Resources, Dam Safety Section, Trout Lake Dam is considered to be in poor condition.

Deficiencies noted during the inspection included the lack of functional drawdown facilities and minor seepage through the masonry near the right abutment. Based on the size and hazard classification of the dam, the recommended Spillway Design Flood (SDF) varies between 1/2 the Probable Maximum Flood (PMF) and the PMF. The 1/2 PMF has been selected as the SDF due to the size of the dam and reservoir, and the downstream conditions. The hydrologic and hydraulic computations indicate that the combination of reservoir storage and spillway discharge capacity will pass only 3 percent of the PMF without overtopping the dam. Overtopping the dam could cause failure, which would lead to a significant increase in downstream loss of life and property damage. Therefore, the spillway for Lake Quinn Dam is considered to be seriously inadequate, and the dam is judged to be unsafe, non-emergency.

A paved roadway has been constructed on the dam crest and a bridge has been constructed over the spillway. The dam was constructed in the early 1950's to provide a lake for recreational purposes. Hemlock Lake Dam has a maximum storage capacity of 117 acre-feet and a maximum height of 27 feet. The dam is classified as 'Small' size. Due to the potential for excessive property damage and the loss of more than a few lives in the damage center which consists of 5 houses with door sills as low as 5 feet above the stream bed located within 1,500 feet downstream of the dam, the structure classified as a 'High' hazard. Examination of the results of the hydrologic and hydraulic analyses indicates that the spillway is capable of passing approximately 26% of the Probable Maximum Flood (PMF) without the dam being overtopped. The selected Spillway Design Flood (SDF) for this 'Small' size, 'High' hazard potential structure is the PMF. The water surface elevation at the damage center is 2.4 feet higher for the breach condition than for the non-breach condition. The spillway is classified as 'Seriously Inadequate, Unsafe, Non-Emergency' since breaching of the dam significantly increases the downstream hazard potential. Based on visual observations and a review of the information obtained from the Pennsylvania Department of Environmental Resources (DER), Hemlock Lake Dam appears to be in poor condition.

Book excerpt: Brier Crest Woods Dam is a homogeneous earthfill embankment with a toe drain. The dam, including the spillway, is 790 feet long and is 16 feet high at maximum section. The spillway is located near the middle of the dam. It is a concrete gravity weir with a concrete exit channel apron. The spillway crest is 25 feet long and is 5.0 feet below the design top elevation of the dam. The outlet works consists of a 12-inch diameter corrugated metal pipe in the spillway weir with a sluice gate at the upstream end. Based on visual inspection, available records, calculations, and past operational performance, Brier Crest Woods Dam is judged to be in good condition. The existing spillway will pass the Probable Maximum Flood (PMF), which is twice the Spillway Design Flood (SDF), with 0.2 foot of freeboard. The spillway capacity is rated as adequate. No stability problems were evident for the embankment. The spillway weir meets recommended guidelines for stability.

The facility is an integral part of the surrounding development known as Wild Acres. The community is nestled in the mountains of Pike County less than five miles west of U.S. Route 209 which parallels the Delaware River in this area. Observations made during the visual inspection indicate the embankment and adjacent dike are in good condition. Heavy brush growth and small trees were observed along the embankment crest and upstream face. No evidence of seepage through the downstream embankment face, excess settlement, sloughing, erosion or animal burrows was encountered by the inspection team. Some rock slope protection has been displaced immediately adjacent the spillway right wingwall, leaving small bare areas on both the upstream and downstream embankment faces. The top of the concrete corewall exhibits excessive cracking and spalling along its entire length except for that portion associated with the dike along the right abutment which is in relatively good condition.

The embankment has a top width of 42 feet and downstream and upstream slopes of 2H:1V an 1H:1V, respectively. The dam was constructed in 1928-29 to provide a lake for recreational purposes. Based on the visual observations, discussions with the Owner's representative, and information obtained from the Pennsylvania Department of Environmental Resources, Division of Dam Safety, Hunter Lake Dam is considered to be in fair condition.

Book excerpt: According to criteria established for these studies, Lake Henry Dam is classified as unsafe, nonemergency, because of the seriously inadequate spillway capacity, the significant seepage, and the whirlpools that have reportedly developed previously in the lake. The recommended Spillway design Flood (SDF) for the size and hazard category of the dam is the Probable Maximum Flood (PMF). The existing spillways can pass about 19 percent of the PMF before overtopping of the dam occurs. It is judged that the dam would fail during the 1.2 PMF. Failure of the dam would increase the hazard to loss of life downstream. As a whole, the dam is judged to be in poor condition.

Location of observed deficiencies are shown on the General Plan presented. The profile and typical sections of the dam are presented and are based on field survey made on the day of inspection. The survey datum for this inspection is elevation 1495 feet above mean sea level for the normal water surface of the lake. On the inspection date (12/02/80), the lake level was approximately at elevation 1495, which is 1.5 feet below the low point on the dam crest. Deficiencies observed during the field inspection are described below, and are further illustrated. Visible features are depicted in photographs presented.
