The overall evaluation of the facilities indicates that Lake Kimberly Dam is in poor condition. The growth of trees, brush and weeds should be removed, and the upstream slope should be protected from wave action. The crest profile is irregular and should be brought to a uniform elevation higher than the emergency spillway elevation in order to prevent overtopping. The downstream slope should be repaired where the slough has occurred and where the outlet pipe has eroded the embankment. The outlet pipe should be extended beyond the downstream toe of the embankment, and a rock protected plunge pool should be constructed at the end of the outlet pipe.

In accordance with the Corps of Engineers' evaluation guidelines, the size classification of this dam is intermediate and the hazard classification is high. These classifications indicate that the Spillway Design Flood (SDF) should be the Probable Maximum Flood (PMF). The spillway capacity is adequate for passing 43 percent of the PMF peak inflow without overtopping the dam. The spillway, therefore, is considered to be inadequate, but not seriously inadequate. Some recommendations presented for immediate action by the owner are: (1) that measures shall be taken to provide an adequate spillway capacity, which shall include the raising of the embankment profile uniformly to at least its design crest elevation; (2) that the downstream toe and an area 20 feet beyond the toe be cleared on a regular basis of all brush, weeds and trees, permitting close observation of the seepage; and (3) that the slab joints in the spillway channel be filled with a joint material.

The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store about 77 percent of the PMF prior to embankment overtopping. Consequently the spillway is assessed as being inadequate, but not seriously inadequate. Deficiencies noted by the inspection team included the following: (1) large trees rooted within the downstream embankment face; (2) a damaged and inadequate spillway discharge channel; and (3) a bent gate stem and partially obstructed outlet associated with the blowoff conduit.

Book excerpt: Warner Dam is owned by William S. Newlin, Jr., and is classified as a 'Low' hazard - 'Small' size dam. The dam was found to be in fair overall condition at the time of inspection. Hydraulic/hydrologic evaluations revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 50- year flood to the 100-year flood is required for Warner Dam. The 100-year flood was chosen as the SDF. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately develop remedial measures to provide adequate spillway capacity. The inspection revealed certain items of remedial work which should be performed by the owner without delay. These include: (1) Provide adequate spillway capacity; (2) Repair the spalling on the downstream face of the concrete corewall; (3) Ensure that the outlet works are operable, or develop an emergency drawdown plan; and (4) Cut all trees on the embankment at ground level. All trees with a trunk diameter greater than 3 inches should have their root systems removed. All resultant areas of erosion and cavities should be filled, graded, compacted and seeded. In addition, the following operational measures are recommended to be undertaken by the owner: (1) Develop a detailed emergency operation and warning system; (2) During periods of unusually heavy rainfall, provide around-the-clock surveillance of the dam; and (3) When warning of a storm of major proportions is given by the National Weather Service, activate the emergency operation and warning system.

The original purpose of the impounded water was for ice harvesting and recreation. Additionally, water releases from the impoundment activated a saw mill. Presently, the lake is being used for recreation. The overall appearance of the dam is poor. Location of observed deficiencies are shown on the General Plan. The profile and typical sections of the dam are presented and are based on field survey made on the day of inspection. The survey datum for this inspection is elevation 1527 feet above mean sea level which is stamped on the USGS Bench Mark (No. TT15K, 1931) on the large sandstone slab, upstream of the service spillway. On the inspection date (12/10/1980), the lake was being drawn down for the winter. The lake level was approximately at elevation 1523.6, or 2.8 feet below its normal level. Deficiencies observed during the field inspection are described. Visible features are depicted in photographs.

Book excerpt: The dam is located approximately 2 miles northwest of the Village of Colley, Wilmot Township, Bradford County, Pennsylvania. The inspection consisted of: Visual inspection of the retaining structure, abutments and toe; Examination of the spillway facilities, exposed portion of any outlet works and other appurtenant works; Observations affecting the runoff potential of the drainage basin; and Evaluation of the downstream area hazard potential. The dam appears to be in good condition and adequately maintained. From a brief survey conducted during the inspection, it was noted that the low spot on the crest of the dam exits at the top left edge of the spillway crest. It was observed during the inspection that a new concrete cap had been placed on the structure. The concrete appeared to be in good condition. The dam was constructed with dry rubble masonry with an upstream earthfill. No concrete exists on the downstream face of the structure or on the bench located along the downstream face of the structure. The rubble masonry appeared to be in good condition and well maintained.

The overall visual evaluation of the facilities indicates that the Wolff Farm Dam is in good condition. However, it is strongly recommended that the downstream slope be provided with a more stable cover. The discharge through the emergency spillway should be directed away from the embankment and the source of the settlement over the 12-inch outlet pipe should be identified and the condition corrected. The rock protection at the outlet pipe should be rearranged so as not to block the open end of the pipe.

Book excerpt: Based on a visual inspection, operational history, and available engineering data, the facility is considered to be in good condition. The size classification of the facility is intermediate and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the PMF (Probable Maximum Flood). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 45 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency. Calculations also indicate that if the spillway were constructed in accordance with available desing drawings, the facility could pass and/or store approximately 63 percent of the PMF.
