Although at this time no signs of significant distress were observed, it appears that the center of the dam has significantly settled. The design drawings show the design freeboard to be five feet, but the available freeboard is only approximately two feet. It also appears that fill has been placed on the downstream slope, increasing the crest width from the design value of 10 feet to about 30 feet near the center line of the dam and greater than 50 feet near the left abutment. The dam has no functional low level outlet facility.

The upstream side of the dam is protected by a small amount of riprap and the downstream face is covered with grass. The flood discharge facilities for the dam consist of a concrete open-channel spillway located near the left abutment. The outlet facilities consist of a 12-inch-diamenter corrugated metal pipe extending through the embankment along the original streambed. Based on the evaluation of existing conditions, the condition of Lake Sophia Dam is considered to be unsafe/nonemergency due to seriously inadequate spillway capacity. The flood discharge capacity was evaluated according to to the recommended procedure and was found to pass approximately 15 percent of the PMF without overtopping the dam. Results of the breach analysis indicate that downstream damage would be significantly increased due to a dam failure.

The profile and typical sections of the dam are presented and are based on field survey made on the day of inspection. The survey datum for this inspection is elevation 1527 feet above mean sea level which is stamped on the USGS Bench Mark (No. TT15K, 1931) on the large sandstone slab, upstream of the service spillway. On the inspection date (12/10/1980), the lake was being drawn down for the winter. The lake level was approximately at elevation 1523.6, or 2.8 feet below its normal level. Deficiencies observed during the field inspection are described. Visible features are depicted in photographs.

The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

The assessment of the general condition of the dam is based upon visual observations and review of available data. Detailed investigations and analyses involving topographic mapping, subsurface investigations, material testing, and detailed computational evaluations are beyond the scope of a Phase I investigation; however, the inspection is intended to identify any need for such studies which should be performed by the owner. In reviewing this report, it should be realized that the reported condition of the dam is based on observations of field conditions at the time of inspection along with data available to the inspection team. In cases where the reservoir was lowered or drained prior to inspection, such action, while improving the stability of the dam, removes the normal load on the structure and may obscure certain conditions which might otherwise be detectable if inspected under the normal operating environment of the structure.

The spillway has undergone a gradual structural failure that has resulted in a reduction of spillway capacity. Continuation of the gradual failure, or perhaps a sudden failure, appears to be possible. There are no functional outlet works for the dam. Maintenance of the project is inadequate. There is no program for regular inspection of the dam.

Book excerpt: Based on the evaluation of the existing conditions, the condition of Big Elk Lake Dam is considered to be good. The flood discharge capacity of the dam was evaluated according to the recommended criteria and it was found that the dam cannot pass the recommended spillway design flood (100-year flood) without overtopping the dam. THerefore, flood discharge capacity of the dam is classified to be inadequate. The following recommendations should be implemented as soon as possible or on a continuing basis. (1) The owner should determine the nature and extent of improvements required to provide adequate spillway capacity. (2) The owner should investigate the need to install low level outlet facilities or prepare plans to lower the lake in the event of an emergency. (3) Around-the-clock surveillance should be provided during unusually heavy runoff and a formal warning system developed to alert the downstream residents in the event of emergencies. (4) The owner should develop a formal operating and maintenance plan and inspect the dam regularly and perform necessary maintenance. (Author).

Book excerpt: Page's Lake Dam is owned and operated by the Lakeside Outing Club and is classified as a 'Significant' hazard - 'Intermediate' size dam. The dam was found to be in good overall condition on 27 October 1980. Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway will not pass the 1/2 Probable Maximum Flood (1/2 PMF) without overtopping the dam. A spillway design flood (SDF) in the range of the 1/2 PMF to the Probable Maximum Flood (PMF) is required for Page's Lake Dam. The 1/2 PMF was chosen as the SDF because the dam is on the low end of the 'Intermediate' size category in terms of storage. During the 1/2 PMF, the dam is overtopped by a maximum depth of 3.34 feet for a total duration of 11.83 hours. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately initiate an engineering study to further evaluate the spillway capacity and develop recommendations for remedial measures to reduce the overtopping potential of the dam.

Book excerpt: The dam consists of a zoned, earthfill embankment that is 590 feet long and 52 feet high at its maximum section. The main spillway is a drop spillway located near the right abutment of the dam. It consists of a concrete riser connected to a 30-inch diameter reinforced concrete pipe under the embankment. The auxiliary spillway is an excavated channel along the left abutment of the dam. It is a grass-linked excavation in earth. Based on visual inspection, available records, calculations and past operational performance, PA-467 Dam is judged to be in good condition. The existing spillway can pass the Probable Maximum Flood(PMF) without overtopping of the dam. The spillway capacity is rated as adequate. There is a slope stability analysis for the embankment, and it indicates that the embankment has adequate factors of safety. There is no evidence of significant problems threatening the embankment.
