Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Book excerpt: The assessment of Pioneer Lake Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The dam and appurtenant structures appear to be in fair condition. No major erosion was observed on the embankment crest. An erosion area was observed at the outlet for the 12 in. diameter cast iron pipe. Erosion at the outlet of the 12 in. diameter pipe exists on the downstream slope of the dam near the toe. Seepage 100 ft. right of the spillway channel was measured to be approximately 50 gallons per minute. A 30 in. corrugated metal pipe exists beneath the roadway near the outlet for the emergency spillway. It was observed during the inspection that the pipe is collapsing near the center of the pipe. Trees were observed growing along the upstream edge of the crest, and dense vegetation was observed along the entire length of the downstream slope. The Pioneer Lake Dam is a high hazard-small size dam. The recommended spillway design flood (SDF) for a dam of this size and classification is in the range of the 1/2 PMF to PMF. Due to the downstream potential for loss of life and property damage, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling approximately 11% of the PMF without overtopping the embankment. Based on criteria established by the Corps of Engineers, the spillway is termed seriously inadequate. Pioneer Lake Dam is classified as unsafe, non-emergency.

It consists of a reinforced concrete riser structure near the upstream toe of the dam, a 24-inch diameter conduit, and a concrete outlet structure at the downstream toe of the dam. The auxiliary spillway is an excavated, trapezoidal channel at the left abutment of the dam. Based on visual inspection, available records, calculations, past operational performance, and according to criteria established for these studies, Maple Lake Dam is judged to be in good condition. Based on existing conditions, the main and auxiliary spillways will pass an approximate minimum of 48 percent of the Probable Maximum Flood (PMF before overtopping of the dam occurs. No stability problems were evident for the embankment at the time of the visual inspection, but a potential hazard to stability exists due to erosion that might occur when there is flow in the auxiliary spillway.

The size classification of the facility is intermediate and its hazard classification is considered to be high. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 45 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency. Calculations also indicate that if the spillway were constructed in accordance with available desing drawings, the facility could pass and/or store approximately 63 percent of the PMF.

The auxiliary spillway is an excavated channel at the right abutment of the dam. It is grass-lined except for portions that were excavated into rock. The hazard classification is high hazard. Downstream conditions indicate that a high hazard classification is warranted for PA-468 Dam. It is owned by the Susquehanna County Commissioners. Its purpose is for flood control.

The original purpose of the impounded water was for ice harvesting and recreation. Additionally, water releases from the impoundment activated a saw mill. Presently, the lake is being used for recreation. The overall appearance of the dam is poor. Location of observed deficiencies are shown on the General Plan. The profile and typical sections of the dam are presented and are based on field survey made on the day of inspection. The survey datum for this inspection is elevation 1527 feet above mean sea level which is stamped on the USGS Bench Mark (No. TT15K, 1931) on the large sandstone slab, upstream of the service spillway. On the inspection date (12/10/1980), the lake was being drawn down for the winter. The lake level was approximately at elevation 1523.6, or 2.8 feet below its normal level. Deficiencies observed during the field inspection are described. Visible features are depicted in photographs.

The upstream slope in the lower portion is 3.5H:1V. The downstream slope is 2.5H:1V and grassed. The embankment is homogeneous with a rock toe. Upstream of the dam is a concrete control tower which contains the principal spillway. Flow enters the concrete spillway tower and drops down the tower and into a 48 inch reinforced concrete pipe to an impact basin at the toe of the dam. In addition, the concrete spillway tower has a 4 foot by 5 foot sluice gate to act as a reservoir drain. The emergency spillway is located in a natural saddle at the opposite end of the reservoir. The emergency spillway has a 300 foot long concrete slab. (Author).

Book excerpt: Humboldt Dam is a homogeneous earthfill embankment with a masonry core-wall. The embankment is 974 feet long and 41 feet high at maximum section. A masonry gravity retaining wall, which acts as the spillway training wall, is at the right end of the embankment. The outlet works, which is near the middle of the embankment, consists of a masonry and corrugated metal intake structure with sluice gates, a 12-inch diameter cast-iron pipe(CIP), and a valve house at the toe of the dam. The intake structure is located on the upstream slope of the embankment; a bridge extends to it. Water supply pipes also extend from the intake structure. The masonry gravity spillway is at the right abutment of the dam. Its crest is 3.0 feet below the design elevation of the top of the dam and is 33.0 feet long. The spillway channel has a variable bottom width and a masonry gravity training wall on the left. The spillway channel extends along the right side of the valley.

Book excerpt: Based on visual inspection, available records, calculations and past operational performance, Lake Henry Dam is judged to be in good condition. The existing spillway can pass 60 percent of the Probable Maximum Flood (PMF) without overtopping of the dam. The spillway capacity is rated as inadequate. If the embankments were raised 0.7 foot to their design elevation, the dam could pass the PMF with 0.05 foot of freeboard. The spillway capacity would then be rated as adequate. A low area between the two embankments acts as an auxiliary spillway. There is no stability analysis for the embankments. There is no evidence of significant problems threatening the embankments. The spillway weir is judged to be stable.

Book excerpt: The condition of Bel-Air Lake Dam is considered to be unsafe/ nonemergency due to a seriously inadequate spillway capacity. The overall condition of the dam is considered to be fair. Although at this time no signs of significant distress were observed, it appears that the center of the dam has significantly settled. The design drawings show the design freeboard to be five feet, but the available freeboard is only approximately two feet. It also appears that fill has been placed on the downstream slope, increasing the crest width from the design value of 10 feet to about 30 feet near the center line of the dam and greater than 50 feet near the left abutment. The dam has no functional low level outlet facility.
