Book excerpt: In the United States, the National Biodefense Strategy is a biosecurity strategy that the federal government was directed to adopt by the National Defense Authorization Act for Fiscal Year 2017. Following the congressional directive, in September 2018, President Trump announced and issued the strategy. The strategy placed the U.S. Department of Health and Human Services in charge of leading the federal government's biodefense efforts, and designated HHS to lead a committee of high-level officials from the Defense Department, Agriculture Department, and Homeland Security Departments, to review the biodefense capabilities of the United States Intelligence Community and 15 other executive branch agencies.

Book excerpt: The National Academies of Sciences, Engineering, and Medicine was asked to articulate a 5-year strategic vision for international health security programs and provide findings and recommendations on how to optimize the impact of the Department of Defense (DOD) Biological Threat Reduction Program (BTRP) in fulfilling its biosafety and biosecurity mission. Because BTRP is just one of several U.S. government programs conducting international health security engagement, both the strategic vision and the success of the program rely on coordinating actions with the U.S. government as a whole and with its international partners. This report provides several recommendations for optimizing BTRP success in its current mission and the wider-looking strategic vision it proposes.

Book excerpt: COVID-19 has demonstrated the enormous dangers posed by biological threats, impacting virtually every community in the world. Mitigating these risks remains an urgent domestic and global imperative. Decisive action is required to build on the investments made for, and the lessons learned from, the COVID-19 response to protect the Nation and our partners from the full range of biological threats to humans, plants, animals, and the environment. Through this Strategy, the U.S. Government will optimize its own efforts and harness the work of essential partners—inside and outside government, domestically and internationally – to assess, prevent, prepare for, respond to, and recover from biological events, whether naturally occurring, accidental, or deliberate, that can harm the American people and the global community.

Book excerpt: Globalization of the food supply has created conditions favorable for the emergence, reemergence, and spread of food-borne pathogens-compounding the challenge of anticipating, detecting, and effectively responding to food-borne threats to health. In the United States, food-borne agents affect 1 out of 6 individuals and cause approximately 48 million illnesses, 128,000 hospitalizations, and 3,000 deaths each year. This figure likely represents just the tip of the iceberg, because it fails to account for the broad array of food-borne illnesses or for their wide-ranging repercussions for consumers, government, and the food industry-both domestically and internationally. A One Health approach to food safety may hold the promise of harnessing and integrating the expertise and resources from across the spectrum of multiple health domains including the human and veterinary medical and plant pathology communities with those of the wildlife and aquatic health and ecology communities. The IOM's Forum on Microbial Threats hosted a public workshop on December 13 and 14, 2011 that examined issues critical to the protection of the nation's food supply. The workshop explored existing knowledge and unanswered questions on the nature and extent of food-borne threats to health. Participants discussed the globalization of the U.S. food supply and the burden of illness associated with foodborne threats to health; considered the spectrum of food-borne threats as well as illustrative case studies; reviewed existing research, policies, and practices to prevent and mitigate foodborne threats; and, identified opportunities to reduce future threats to the nation's food supply through the use of a "One Health" approach to food safety. Improving Food Safety Through a One Health Approach: Workshop Summary covers the events of the workshop and explains the recommendations for future related workshops.

Book excerpt: GAO has reported on the inherent fragmented nature of the federal and nonfederal resources needed to protect the nation from potentially catastrophic biological threats. GAO called for a strategic approach to help the federal government better leverage resources and manage risk The White House issued the National Biodefense Strategy and the Presidential Memorandum on the Support for National Biodefense to promote a more efficient and coordinated biodefense enterprise. This report addresses the extent to which the Strategy and implementation efforts are designed to enhance national biodefense capabilities and any implementation challenges that exist. GAO is making four recommendations to the Secretary of HHS, including working with other agencies to document methods for analysis and the processes, roles, and responsibilities for enterprise-wide decision making.

Book excerpt: This book examines the recent intersection of national security and public health regarding biological threats to the U.S. populace and proposes improvements to the executive and legislative development of U.S. policy addressing biological threat mitigation. Over the last 20 years, the national security community has engaged with disease-related issues that have traditionally been the scope of public health agencies. The federal government's response has been to create a single national biodefense strategy, which has been largely ineffective in improving conditions due to poor terminology, a lack of leadership, and a failure to assess government programs. Applying a public policy framework, Albert J. Mauroni examines how the government addresses biological threats--including disease prevention, bioterrorism response, military biodefense, biosurety, and agricultural biosecurity and food safety. He proposes a new approach to countering biological threats, arguing that lead agencies should focus on implementing discrete portfolios with annual assessments against clear and achievable objectives.

Book excerpt: This report documents the accomplishments of the FAO Project TCP/MIC/3603/C2 – “National Aquatic Animal Health and Biosecurity Strategy” that was implemented in 2019 for the Federated States of Micronesia (FSM). These include the following: (i) Round-table discussions on aquaculture development, biosecurity legislation, aquatic animal health and aquaculture biosecurity (21–22 May 2019); (ii) Technical Seminar on Basic Aquatic Animal Health and Aquaculture Biosecurity (23 May 2019); (iii) National Consultation on Aquaculture Development, Biosecurity Legislation, Aquatic Animal Health (24 May 2019); and (iv) Introductory training course on risk analysis within the Progressive Management Pathway for Improving Aquaculture Biosecurity (PMP/AB) (27–28 May 2019). The various activities undertaken during the field mission provided the basis for drafting the National Strategy on Aquatic Animal Health (NSAAH) and the National Aquatic Pathogen List (NAPL) for FSM. There is a need to conduct another round of national consultations in order to generate feedback prior to finalizing the documents and approval. The next step will be to incorporate them into the government’s policy documents and work with partners for joint resource mobilization to support implementation. The report also contains a list of recommendations that the Government of FSM should consider to improve capacities in aquatic animal health and aquaculture biosecurity.

Book excerpt: SARS CoV-2 virus has spared none and respected no boundaries between nations, rich and poor, people of color and race - it has left the world "gasping for breath". The picture bears an uncanny resemblance to what the images would be like during a biological war. This unmissable connect has been the force behind writing this book. The ongoing revolution in biotechnology has initiated transformational changes in the biological warfare. It has created the means to modify the genome of a virus through genetic engineering, thus enabling development of highly lethal biological weapons. The book contains some basic information concerning genetic engineering, so that the military readership can better appreciate the employment of future biological weapons like binary and stealth weapons etc. Equally interesting is the discussion of novel strategic concepts like: small weapon big bang, retaliatory assured destruction, fog of ambiguities and uncertainties, bio umbrella and multi space battles. It is followed by the study of Bioterrorism and asymmetric conflict, the space in which the biological warfare is most likely to manifest. The book dwells heavily on the need for jointmanship between the defence forces and the civilian agencies to fight the menace of any future pandemic or an act of bioterrorism. Many constructive propositions have been put forth towards developing national biosecurity strategy, state level coordination and optimal utilization of resources.

Book excerpt: In response to a request from the U.S. Congress, this book examines how the unique experience and extensive capabilities of the Department of Defense (DOD) can be extended to reduce the threat of bioterrorism within developing countries outside the former Soviet Union (FSU). During the past 12 years, DOD has invested $800 million in reducing the risk from bioterrorism with roots in the states of the FSU. The program's accomplishments are many fold. The risk of bioterrorism in other countries is too great for DOD not to be among the leaders in addressing threats beyond the FSU. Taking into account possible sensitivities about a U.S. military presence, DOD should engage interested governments in about ten developing countries outside the FSU in biological threat reduction programs during the next five years. Whenever possible, DOD should partner with other organizations that have well established humanitarian reputations in the countries of interest. For example, the U.S. Agency for International Development, the Centers for Disease Control and Prevention, and the World Health Organization should be considered as potential partners.

Book excerpt: Based on a series of regional meetings on university campuses with officials from the national security community and academic research institutions, this report identifies specific actions that should be taken to maintain a thriving scientific research environment in an era of heightened security concerns. Actions include maintaining the open exchange of scientific information, fostering a productive environment for international scholars in the U.S., reexamining federal definitions of sensitive but unclassified research, and reviewing policies on deemed export controls. The federal government should establish a standing entity, preferably a Science and Security Commission, that would review policies regarding the exchange of information and the participation of foreign-born scientists and students in research.
