There is no overarching programme or single accountability for encouraging people to save for retirement. The Treasury leads on overall savings strategy, and DWP on workplace saving but, without a whole system view there is a risk that individual, but co-dependent interventions may not be effective in increasing saving for retirement. Spending on the state pension and pensioner benefits increased from 5.5 per cent of GDP in 1990 to 6.9 per cent in 2011-12, in part because of the growing pensioner population, but also because of increased spending per capita on pensioner benefits. The Government expects to reduce the potential long-term spending liability by increasing the future state pension age, introducing automatic enrolment into workplace pensions and changing the state pension. However the existing initiatives to manage this problem face challenges. Government does not have a formal published strategy to influence employers. The success of encouraging saving for retirement through automatic enrolment into a pension also depends on the responses of individuals, pension providers and employers - in particular on the proportion of employees who remain with the schemes. The value of the UK state pension, as a proportion of pre-retirement earnings, has historically been low compared to other developed countries, and projections for future spending on state pensions and pensioner benefits may prove too optimistic

However, the complexity of the deal combined with poor quality of data, initial planning, and infrastructure meant that it took the Cabinet Office longer than intended to finalise the transaction. Furthermore, the Department and MyCSP still face many large challenges in transforming the service. The biggest challenge is improving data to support the implementation of the new 2015 pension scheme. The deal is projected by the Department to reduce the cost of administration by 25 per cent, to £13 per member per year by 2019. Scheme members should receive a better quality of service as a result of significant investment in the business by the private sector partner, Equiniti Paymaster, and a payment mechanism that penalizes MyCSP if it misses the service standard levels in the contract. The deal also means that MyCSP now has a credible plan to meet the challenges of the 2015 civil service pension reforms, including data improvement. The Department did not initially make the most of the opportunities to learn from this transaction as a pathfinder, but has now reviewed the lessons learned from executing the transaction and has put in place an evaluation strategy. The Cabinet Office must now ensure it evaluates the longer-term comparative performance of MyCSP and captures and disseminates the lessons learnt from the deal.

It is estimated that this costs the economy, as a whole, �2 billion and the NHS �0.5 billion in treatment. However the response of the NHS is patchy, with no national guidelines and only 28% of health authorities taking action to address the problem. There is little activity related to the management of obesity outside of general practice but only a small proportion of GPs follow a protocol. This report recommends that there should be strategies to reduce obesity and that the Department of Health should build on the plan in the National Service Framework on coronary heart disease and work with partners and professional bodies to clarify responsibilities. It should also work with the National Institute of Clinical excellence to disseminate information. The Department of Health should also lead a cross government strategy to promote the benefits of physical activity and there should be local targets to encourage cycling and walking. It should also work with the food industry to improve the balance of diet. Much of this work needs to be aimed at school children to promote a healthy lifestyle throughout life and guidance to schools on commercial sponsorship should be strengthened so that there is no conflict with messages on healthy eating.

Book excerpt: The Charity Commission is not regulating charities effectively and there is a gap between what the public expects of the Commission and what it actually does. The NAO has concluded that the Commission does not do enough to identify and tackle abuse of charitable status. Between 2007-08 and 2013-14, the Commission's annual budget fell 40 per cent in real terms to £22.7 million but the number of main registered charities has remained fairly constant at around 160,000. In response to budget cuts, the Commission has reviewed how it works and successfully reduced demand for its services, but it has not identified what budget it would need to regulate effectively. The Commission makes little use of its enforcement powers, for example suspending only two trustees and removing none in 2012-13. And it can be slow to act when investigating regulatory concerns. The NAO found cases where periods of several months passed during which the Commission took no action. Furthermore, the Charity Commission does not take tough enough action in some of the most serious regulatory cases. It is also reactive rather than proactive, making insufficient use of the information it holds to identify risk. The Charity Commission needs to think radically about alternative ways of meeting its objectives with constrained resources. It also needs to make greater use of its statutory powers in line with its objective of maintaining confidence in the sector; and develop an approach to identify and deal with those few trustees who deliberately abuse charitable status. This report publishes alongside another NAO report, the Cup Trust.

Book excerpt: In 2009-10 the Department for Work and Pensions overpaid its customers by an estimated £1.1 billion and made underpayments of £500 million. However, the scale of the challenge faced by the Department should not be underestimated. The benefits system is large, encompassing over 27 different benefits and a total caseload of around 20 million people. In addition, the Department has had to respond to the recent recession in which Jobseekers Allowance caseload almost doubled between 2008 and 2009. The recent announcement of the introduction of Universal Credit is an opportunity to simplify many of the regulations, but such changes will take a long time to implement. In the meantime, the onus remains on the Department to keep the costs of mistakes to a minimum. The Department has demonstrated a clear commitment to reducing administrative error, but there is scope for improvement in the quality of information used to assess where the Department should focus its efforts. Although DWP has initiated an exercise to understand fully the causes of error, this will not be complete until the spring of 2011. There is also scope for further work in collecting and analysing the full costs and benefits of the Department's interventions in order to assess cost effectiveness. The Government announced a new strategy in October 2010 with a greater emphasis on preventing errors from arising and this is now an opportunity for lessons to be learned.

As of July 2006, the trained strength of the armed forces stood at around 180,690, with an estimated shortfall of 5,170 against the Departments requirement. Overall the armed forces are not in manning balance, with the figures masking a wider shortage of trained personnel within a range of specific trade groups across all three services. All three services expect to be within manning balance by April 2008, though historically the services have consistently run below the full manning requirement. The NAO has identified 88 operational pinch point trades where there is insufficient trained strength to perform operational tasks. The report sets out a number of conclusions and recommendations, including: that the Department should review overall manning requirements within individual operational pinch points and also develop guidelines on the expected levels of voluntary outflow for individual operational pinch points; regular surveys of personnel should be carried out, focusing on factors that reduce retention; the Department should also assess the impact of the work/life balance and the extent to which breaches of individual harmony may be understated; further, the Department should look to investigate measures to provide greater stability and certainty of work patterns for personnel between operational deployments; also that a cost effective analysis should be conducted on the payment of financial retention incentives and the impact on decisions to continue serving in the armed forces; the Department should also review the scope of schemes which provide opportunities to offer competitive salaries, and consider the recruitment to a wider range of trades than is currently the practice; the Department should also develop a clear order of priority for the wide range of long-term projects it has planned and commit to firm timescales and funding programmes for the most important. Book excerpt: Since the Department's 2007 Maternity Matters strategy, there has been improvement in maternity services. However, there is wide variation between trusts in performance. The Department did not fully consider the implications of delivering its ambitions and has failed to demonstrate that it satisfactorily considered the achievability and affordability of implementing the strategy. Nor has it monitored national progress against it. In 2011, one in 133 babies was stillborn or died within several days of birth. The mortality rate has fallen over time, but comparisons with the other UK nations suggest scope for further improvement. Trusts paid £482 million for maternity clinical negligence cover in 2012-13, equating to around a fifth of spending on maternity services. The level of consultant presence has substantially improved but over half of maternity units (including all of the largest units) do not meet recommended levels. The NHS is also not meeting a widely recognised benchmark of one midwife to 29.5 births. The government has commissioned more places to study midwifery, but it is unclear whether these will be enough. Meeting the benchmark would require around 2,300 additional midwives nationally. In terms of choice of place of birth, 79 per cent of women are currently within a 30-minute drive of both an obstetric and midwifery-led unit, compared with 59 per cent in 2007. However, choice is restricted where units have to close because of a lack of physical capacity or midwives. Over a quarter of units closed for half a day or more between April and September 2012
