Book excerpt: In Britain, between 1980 and 1998, the number of people classified as obese tripled to 21per cent of women and 17 per cent of men. It is estimated that this costs the economy, as a whole, �2 billion and the NHS �0.5 billion in treatment. However the response of the NHS is patchy, with no national guidelines and only 28% of health authorities taking action to address the problem. There is little activity related to the management of obesity outside of general practice but only a small proportion of GPs follow a protocol. This report recommends that there should be strategies to reduce obesity and that the Department of Health should build on the plan in the National Service Framework on coronary heart disease and work with partners and professional bodies to clarify responsibilities. It should also work with the National Institute of Clinical excellence to disseminate information. The Department of Health should also lead a cross government strategy to promote the benefits of physical activity and there should be local targets to encourage cycling and walking. It should also work with the food industry to improve the balance of diet. Much of this work needs to be aimed at school children to promote a healthy lifestyle throughout life and guidance to schools on commercial sponsorship should be strengthened so that there is no conflict with messages on healthy eating.

Book excerpt: Since the Department's 2007 Maternity Matters strategy, there has been improvement in maternity services. However, there is wide variation between trusts in performance. The Department did not fully consider the implications of delivering its ambitions and has failed to demonstrate that it satisfactorily considered the achievability and affordability of implementing the strategy. Nor has it monitored national progress against it. In 2011, one in 133 babies was stillborn or died within several days of birth. The mortality rate has fallen over time, but comparisons with the other UK nations suggest scope for further improvement. Trusts paid £482 million for maternity clinical negligence cover in 2012-13, equating to around a fifth of spending on maternity services. The level of consultant presence has substantially improved but over half of maternity units (including all of the largest units) do not meet recommended levels. The NHS is also not meeting a widely recognised benchmark of one midwife to 29.5 births. The government has commissioned more places to study midwifery, but it is unclear whether these will be enough. Meeting the benchmark would require around 2,300 additional midwives nationally. In terms of choice of place of birth, 79 per cent of women are currently within a 30-minute drive of both an obstetric and midwifery-led unit, compared with 59 per cent in 2007. However, choice is restricted where units have to close because of a lack of physical capacity or midwives. Over a quarter of units closed for half a day or more between April and September 2012

There are 205 projects in the Government Major Project Portfolio, with a combined whole-life cost of £376 billion, and annual cost of £14.6 billion. Thirty nine of those projects have a delivery confidence rating of 'red' or 'amber/red'. The report supports the changes that government has made to the central assurance system for such projects - in particular, the launch of the Major Projects Authority - and finds that they have resulted in some significant impacts. However, the system is not yet 'built to last'. Processes need to be formalised, and the Authority, HM Treasury and departments need to co-operate more if these improvements are to continue. Transparent reporting of project data would also create a more effective and enduring system. An effective central system that gives assurance over project progress is critical for ensuring successful outcomes. Capacity constraints have had an impact on the effectiveness of the assurance system. The Authority is reporting on 160 more projects as part of the portfolio and carrying out more in-depth assurance work, but has 40 per cent fewer staff than the body it replaced. As a result, processes for sharing project lessons and for continuously improving the system remain informal, and there is overdependence on key individuals in the system.

The analysis of departments' accounts supports the Efficiency and Reform Group's estimate that Government spending moratoria and efficiency initiatives, including cuts to back-office and avoidable costs, contributed around half of the figure, some £3.75 billion. However, the report warns that departments are less well-placed to make the long-term changes needed to achieve the further 19 per cent over the four years to 2014-15, as required by the spending review. This is partly because of gaps in their understanding of costs and risks, making it more difficult to identify how to deliver activities and services at a permanently lower cost. Fundamental changes will be needed to achieve sustainable reductions on the scale required. It is unclear how far spending reductions represent year-on-year changes in efficiency, or whether front-line services are affected; and the departments' forward plans examined by the NAO are not based on a strategic view. Departments' financial data on basic spending patterns is sufficient to manage budgets in-year, but information about the consequences of changes in spending is less good. Longer term reform is a Cabinet Office priority and departments will need to look beyond short-term cost cutting measures and make major operational change. Cost reduction plans also need to build in contingency measures to cover unexpected risks.

To date the performance of some of the major projects at Sellafield has been poor. The Authority inherited a legacy of poor planning and neglect over several decades when it took ownership of Sellafield in April 2005. The Authority achieved an important milestone in May 2011 when it approved a more robust lifetime plan for the clean-up of Sellafield site by 2120. The improved lifetime plan contributed to an increase in the Authority's provision for decommissioning the site to £67 billion (undiscounted) as at March 2012, up from £47 billion as at March 2009. Significant uncertainties and scheduling risks remain, which the authority is working to understand and address. For example, there is considerable uncertainty over the time required and cost of completing facilities to treat and store highly radioactive material held in deteriorating legacy ponds and silos. Today's report concluded that it is too early to judge whether the appointment of Nuclear Management Partners Limited as the parent body of Sellafield is delivering value for money.

Book excerpt: This report concludes that the Department for Work and Pensions has not achieved value for money in its early implementation of Universal Credit. The Department was overly ambitious in both the timetable and scope of the programme, took risks to try to meet the short timescale and used a new project management approach which it had never before used on a programme of this size and complexity. It was unable to explain how it originally decided on its ambitious plans or evaluated their feasibility. Nor did it have any adequate measures of progress. Over 70 per cent of the £425 million spent to date has been on IT systems, and £34 million of its new IT systems has been written off. The existing systems offer limited functionality - the current IT system lacks a component to identify potentially fraudulent claims so that the Department has to rely on multiple manual checks on claims and payments. Problems with the IT system have delayed national roll-out of the programme, which will reduce the expected benefits of reform and - if the 2017 completion date remains - increase risks by requiring the rapid migration of a large volume of claimants. The source of many problems has been the absence of a detailed view of how Universal Credit is meant to work. In addition, poor control and decision-making undermined confidence in the programme and contributed to a lack of progress. The Department has particularly lacked IT expertise and experienced frequent changes in senior management.

Developing further the theme of adsensory technologies of the sign, in conjunction with Daniel Bell’s theory of the codification of knowledge as an axial feature of the structuring of post-industrial society, this book explores gentrification in heterotopic post-industrial urban spaces. It brings together case studies from London’s Grenfell Tower, exploring perilous façadism refurbishment and London’s Garden Bridge project and speculative capital regeneration. These studies illustrate, empirically, the extent to which advertising adsensory technologies have become integral to the gentrification of post-industrial urban spaces. Several of the case studies engage critically with the empirical observation that, in the post-industrial urban ecology of inner-city regeneration, adsensory technologies extend avariciously into the infrastructure of neoliberal, managerialist gentrification. In addition, the book explores the forms of capital accumulation which are emerging from the integration of adsensory technology into the gentrification of post-industrial urban spaces, and examines a new form of capital accumulation in inner-city gentrification, predicated on the (de)generative integrity of adsensory financialisation.

Book excerpt: In respect of its largest defence projects there are early signs that the Ministry of Defence has begun to make realistic trade-offs between cost, time, technical requirements and the amount of equipment to be purchased. Nevertheless, the continuing variances to cost and time show the MOD needs to do consistently better. This report, which gives a progress review of the 16 largest defence projects, shows that in the last year there has been a total forecast slippage of 139 months and increase in costs of £468 million. This means that, since the projects were approved, costs have increased by £6.6 billion (around 12 per cent more than the planned cost) and the projects have been delayed by 468 months, taking almost a third longer than originally expected. It would be unrealistic to expect MOD and industry to identify every risk at the start of technically challenging projects. However, the continuing problems indicate that MOD has more to learn from historic. The MOD is accepting the capability risk and some wider costs resulting from these project delays and is having to make difficult decisions about long-term capabilities. The MOD has made a significant investment in new and upgraded helicopters to address the shortfall identified in the NAO's 2004 report. The MOD has also spent £787 million on air transport and air-to-air refuelling aircraft to support current operations and address capability gaps, such as those caused by the previously reported delays to the A400M transport aircraft. The Group has been responsible for a number of new initiatives, including the moratorium on central government's signing new ICT contracts worth more than £1 million without agreement from the Minister for the Cabinet Office; a mandatory review of all ICT projects currently underway to identify waste; and a drive to renegotiate contracts with suppliers. Most of the Group's initiatives are at a very early stage and it is too early for the National Audit Office to evaluate their effect. This review will, however, set the ground for future evaluations of these and other initiatives designed to achieve value for money from ICT spending.
