This secure channel enables eHealth national services to connect and exchange patients' health data rapidly, safely and efficiently. Your personal data will always be treated and protected in accordance with existing EU legislation and with current practices in both your country of residence and your country of travel. In addition, before a health professional accesses any of your data, he or she should inform you about your rights. The new electronic cross-border health services are being established progressively in 25 EU countries until the end of 2025. While the goal to improve average levels of population health is important, there has been an increasing focus on disparities at national and European levels. Improvements have been seen over the past few decades in both health status and living and working conditions has widened tremendously in the European Union (EU) and will continue to do so as it goes through the enlargement process. The diversity in living conditions has translated into diversity in patterns of health across the region. Inequalities in income, education, housing and employment affect population health, both directly (for example, good housing reduces risks associated with poor health) and indirectly through psychosocial factors (such as stress). From the life course perspective, individuals are affected by different sets of risks related to disease and illness; certain diseases and causes of health are more likely to affect young people, whereas the majority are associated with older ages. Investigating differences in health status within and between European countries provides the focus of this report. The relationship between living conditions, socioeconomic factors and health is discussed and analysed with the objective of stimulating a debate and policy action for creating a healthier and more equitable society. We aim to present an overview of key issues and not comprehensive literature review or exhaustive analysis of the topics involved.

At the same time they could act as focal points for medical training and research information dissemination and evaluation especially for rare diseases. The idea of pooling resources in order to better address medical conditions that are rare or require very specialized expertise or equipment corresponds with moves towards concentration of specialized health care services often motivated by common health systems challenges like tightening financial constraints workforce shortages and growing attention for quality and safety. This book examines the different ways in which the concept of reference networks has been implemented in European countries and what kind of medical conditions or interventions it covers in various countries. It also looks at the motivations behind the establishment of such networks the regulatory and administrative processes for identifying and designating them as well as the financial arrangements needed for their proper functioning. This study outlines the key policy implications and challenges for developing the concept of reference networks at national and European levels.
