With the aging of the American population, concern arises regarding potential increases in rates of crash involvement and injury. Age-related factors may impair driving ability, such as age-related decrements in cognitive and physical functioning, increased prevalence of medical conditions or age-related medical conditions, and increased use of multiple medications. Using population databases, this study analyzed the association of the impairing effects of multiple medication use, drug interactions, and drug disease interactions on motor vehicle crashes (MVC) in individuals age 50 years and greater. Methodology: The main objectives of this study were to determine the relative frequency of various combinations of medications used by those who have experienced a MVC and those who have not by analyzing proprietary and non-proprietary databases; and to conduct a case-control study of possible associations between the use of medications (and combinations thereof) and MVCs amongst older drivers.

Book excerpt: OBJECTIVE: Illegal drug use is a persistent problem, prescription drug abuse is on the rise, and there is clinical evidence that drug use reduces driving performance. This study describes trends in characteristics of drivers involved in fatal motor vehicle crashes who test positive for drugs. METHODS: We used the Fatality Analysis Reporting System-a census of motor vehicle crashes resulting in at least one fatality on U.S. public roads-to investigate suspected drug use for the period 1993-2010. RESULTS: Drugged drivers who were tested for drug use accounted for 11.4% of all drivers involved in fatal motor vehicle crashes in 2010. Drugged drivers are increasingly likely to be older drivers, and the percentage using multiple drugs increased from 32.6% in 1993 to 45.8% in 2010. About half (52.4%) of all drugged drivers used alcohol, but nearly three-quarters of drivers testing positive for cocaine also used alcohol. Prescription drugs accounted for the highest fraction of drugs used by drugged drivers in fatal crashes in 2010 (46.5%), with much of the increase in prevalence occurring since the mid-2000s. CONCLUSIONS: The profile of a drugged driver has changed substantially over time. An increasing share of these drivers is now testing positive for prescription drugs, cannabis, and multiple drugs. These findings have implications for developing interventions to address the changing nature of drug use among drivers in the U.S.

During the past 20 years, 25 states and the District of Columbia have legalized cannabis and/or cannabidiol (a component of cannabis) for medical conditions or retail sales at the state level and 4 states have legalized both the medical and recreational use of cannabis. These landmark changes in policy have impacted cannabis use patterns and perceived levels of risk. However, despite this changing landscape, evidence regarding the short- and long-term health effects of cannabis use remains elusive. While a myriad of studies have examined cannabis use in all its various forms, often these research conclusions are not appropriately synthesized, translated for, or communicated to policy makers, health care providers, state health officials, or other stakeholders who have been charged with influencing and enacting policies, procedures, and laws related to cannabis use. Unlike other controlled substances such as alcohol or tobacco, no accepted standards for safe use or appropriate dose are available to help guide individuals as they make choices regarding the issues of if, when, where, and how to use cannabis safely and, in regard to therapeutic uses, effectively. Shifting public sentiment, conflicting and impeded scientific research, and legislative battles have fueled the debate about what, if any, harms or benefits can be attributed to the use of cannabis or its derivatives, and this lack of aggregated knowledge has broad public health implications. Book excerpt: Background Motor vehicle crashes occur daily and may be fatal for the driver, passengers or bystanders. Crashes have been shown to be associated with certain medical conditions and exposure to prescription medications that impair the ability to drive. We sought to create a linked, population-based longitudinal crash and medical care use dataset and to evaluate the relationships between prescription opioid and sedative hypnotic medication exposures and automobile crashes in Washington State. Methods Crash data from the Washington State Department of Transportation from 2002 to 2009 were linked to medical, pharmacy and administrative data from the Group Health Cooperative (GHC) for all eligible members. Continuous prescription exposures were defined based on fill records for opioids and sedatives. We estimated multivariate extended Cox regressions to investigate the association between police-reported crashes and prescription fills for opioids and sedative hypnotics. We also stratified our analyses by the length of continuous prescription fills. Results A total of 676,694 subjects were included in the study. These individuals experienced 2.41 motor vehicle crashes per 100 person-years. Opioid prescription fills were associated with a hazard ratio of 1.37 (95% CI: 1.33 to 1.41). Stratifying opioid exposure by the length of continuous prescriptions resulted in a range of hazard ratios from 1.76 at 30 days (95% CI: 1.59 to 1.96) up to 11.33 at 120 days (95% CI: 9.18 to 13.98). Overall incident sedative prescriptions were associated with a hazard ratio of 2.23 (95% CI: 1.92 to 2.59). Stratifying continuous prescriptions by length revealed the highest risk group to be 121-240 days for all sedatives, trazodone and temazepam (overall HR: 6.84, 95% CI: 4.15 to 11.2). Zolpidem had no risk of crash in the first 30 days, yet risk climbed to a peak risk between 241-360 days (HR: 10.7, 95% CI: 3.9 to 29). Discussion The risk of motor vehicle crash is increased in people who fill prescriptions for opioids and sedative hypnotics. The hazard ratios estimated are equivalent to blood alcohol concentrations in excess of double the legal limit to drive. Public health may be improved by reducing driving time when using these medications.

Book excerpt: This study is designed to evaluate the correlation between fatal vehicle crashes (FVC) and consumption of alcohol and/or drugs among drivers. Between 1996 and 2000 in Hong Kong, a total of 197 FVC cases of deceased drivers were investigated. The blood and/or urine samples of the victims were examined for the presence of alcohol and drugs. The 197 cases were then classified into two groups: single-vehicle crashes (SVC) and multiple-vehicle crashes (MVC). Out of the 106 cases for the latter group, alcohol and/or drugs were detected in 22 cases (21%) while the remaining 84 cases (79%) were regarded as no significant finding. As for the 91 cases in SVC group, 51 cases (56%) were positive for alcohol and/or drugs. The findings indicate that a driver consuming alcohol and/or drugs has a higher risk of being involved in a FVC. The most frequently detected drugs for SVC group (11 cases) were: 46% central nervous system (CNS) stimulants (including designer drugs like MDMA); 36% cannabis; 18% benzodiazepines and 9% ketamine. The detected drug for the only case in the MVC group was a CNS stimulant. The number of cases with ketamine, methamphetamine and MDMA detected has increased in recent years as these party drugs have gained popularity in Hong Kong.
