Book excerpt: Genistein (GS) is a naturally occurring isoflavone that interacts with estrogen receptors and other molecular targets. Human exposure to GS is through consumption of soy products, incl. soy-based infant formula and dietary supp. Consumption of soy and GS has been assoc. with a variety of beneficial effects in animals and humans, but concerns have also been raised concerning potential adverse effects of GS, particularly with regard to reproductive toxicity and the induction or potentiation of carcinogenesis, due primarily to its weak estrogenic activity. Because of these concerns, GS was selected as one of the compounds to be examined in a protocol utilizing Sprague-Dawley rats to evaluate the effects of multigenerational and long-term exposures to doses of estrogenic agents that produce subtle reproductive tract lesions in developmentally exposed Sprague-Dawley rat pups. Available in PDF, EPUB and Kindle. Book excerpt: For many years, Springer has been publishing an impressive series of textbooks of pharmacology which have set standards in medical science. Surprisingly, an extensive overview of the current state of the art in research on estrogens and anti estrogens was still lacking. The present two volumes on estrogens and anti estrogens provide a comprehensive review of a field of research in which remarkable progress has been made over the past few years. New insights into the mechanisms of steroid hormone action resulted in a tremendous number of pUblications from which new principles of preventive and therapeutic appli cations of estrogens and anti estrogens emerged. Although various electronic data bases provide easy access to this copious information, there was a clear necessity for a monograph-style textbook which assesses and summarizes current knowledge in this rapidly expanding field of research. It should be noted, however, that, due to this dynamic development, it is barely possible to comprehensively update every aspect of basic and clinical knowledge on estro gens and antiestrogens. Thus, the intention of the editors was to provide the reader with an overview of the "classic" and most recently explored areas of research and stimulate future interests in basic and applied endocrinology. Estrogens were among the first steroid hormones described in the scientific literature. Since they were first isolated, since the chemical, synthesic and pharmacological characterization of naturally occurring estrogens and, later on, of orally active derivatives, estrogen research has produced continu ously hallmark results in reproductive endocrinology worldwide.

Available in PDF, EPUB and Kindle. Book excerpt: I opened my series editor manuscript of The Handbook of Contraception: A Guide for Practical Management, edited by Drs. Donna Shoupe and Siri Kjos, on a tiny plane on the way to giving a lecture in Albany, NY. I expected to peruse the ma- script, and found that I could not put it down. The Handbook of Contraception: A Guide for Practical Management is an incredibly informative and enjoyable read. In keeping with the objective of this series for primary care clinicians, there is a quality in this title that is uncommon among medical textbooks. The chapters of this book are written with extraordinary intelligence and und- standing, and with attention to practical considerations in the selection and mana- ment of contraceptive options. The authors have reviewed the science behind contraception, including the chemical structure and effects of hormonal contraception, physiology of contraception, efficacy rates, and side effects, as well as the practical considerations that are relevant in helping patients choose between different cont- ceptive options. They do this with a clarity of language and intent that lets the book cover with sufficient detail the full range of questions that any primary care clinician will have regarding any of the traditional or new contraceptive options. Also included in each chapter is a section on “counseling tips,” which explicitly answers many of the questions that clinicians and their patients often have when discussing contraceptive options. For a book so useful and well done, the editors and authors deserve our thanks.

Book excerpt: For anyone who treats postmenopausal women, this latest edition of Rogerio Lobo's classic work combines the best from two well-known references: Menopause, and the second edition of Treatment of the Postmenopausal Woman. By adding significant discussions of the basic science behind menopause, it is possible to objectively assess the clinical value and limitations of current approaches to treatment and provide a basis and rationale for strategies that will result in better individualized and specialized care. Not only does the third edition discuss diagnosis and treatment of menopause but it covers biological, anatomical, physiological, pathobiological, and pharmacological aspects as well bringing together, in one source, all of the information needed to understand and treat postmenopausal conditions. Over 50% new material representing the vast amount of information available since the Women's Health Initiative (WHI) clinical trials were completed making this the most up-to-date reference on postmenopausal women Includes several new sections on comparisons between clinical trials and observational data, urology, and pelvic support Each section is preceded by a preface to put the area into context with many chapters having suggested treatment regimens

They review both basic and clinical knowledge in fourteen chapters. The book begins with the biochemistry of testosterone, its biosynthesis, metabolism and mechanisms of action in target organs. Three chapters deal with specific aspects of testosterone action, namely its role in spermatogenesis, its psychotropic effects and its effects on bones. Syndromes caused by androgen resistance are described in order to highlight the importance of properly functioning enzymes and receptors in the target organs. Causes and symptoms of male hypogonadism, the major indication for testosterone treatment, are described. Five chapters are devoted to the pharmacology, pharmacokinetics and clinical uses and abuses of testosterone preparations. The new transdermal testosterone application is described in detail. Side effects of testosterone treatment are reviewed. The possible role of androgens in the development of prostatic hypertrophy and carcinoma is discussed extensively since this question is of major concern to the clinician.

Available in PDF, EPUB and Kindle. Book excerpt: The response to the First Edition of this text confirmed our belief that there was a need for a book of this kind. The multi-authored approach has been retained, ensuring that authoritative, current information is incorporated into each chapter and that references are up-to-date. The section on diseases of the vagina has been enhanced by a greater emphasis on the clinical aspects. The chapter on DES induced lesions has been updated with the data of the DeSAD study and the section on adenocar cinoma of the cervix has been enlarged by the description of the undifferenti ated lesions including "glassy cell and signet cell" carcinoma. A departure from the traditional chapter approach has been made in the discussion of endometrial hyperplasia and carcinoma in order to present a conceptual view of these diseases. A similar presentation of diseases of the myometrium has been made. The subjects of lymphomas and mesenchymal tumors of the ovary have been enlarged upon and presented in a separate chapter. Fine needle aspiration in the diagnosis of ovarian tumors and of non malignant disorders of the ovary has become an increasingly useful technique. Two new chapters have been added to cover this subject. Many of the changes made in the second edition were in response to reviewers of the First Edition. It is hoped that their constructive suggestions have been addressed. Ancel Blaustein, M.D.

Presented as an accessible pocket-sized handbook, the chapters are organized in an outline format, offering only the most essential information on the etiology, staging (including TNM staging) and treatment for each cancer type. Individual chapters are devoted to the molecular biology of cancer, cancer prevention, cancer screening, the mechanisms of chemotherapy, and diagnostic imaging in cancer. Additionally, each chapter lists all the major phase III clinical trials, and therefore, serves as an excellent reference of the major randomized controlled trials for each cancer reported to date. Specific chapters are also dedicated to the discussion of oncologic emergencies, pain and palliation, and prescription complications. At the conclusion of the book, a glossary of oncologic terms and chemotherapeutic drug programs, a table of common cancer incidences, and an overview of the mechanisms, common uses, and related toxicities of various anti-cancer agents are featured. In addition, performance status tables, mathematical formulas and a listing of common biomedical / cancer web sites are highlighted.
