Book excerpt: Methici 11 in-resistant Staphylococcus aureus is now recognised as an important community and hospital acquired pathogen with serious morbidity, mortality, and health care cost in tertiary care centers and nursing homes (Araj etal., 1996; submitted). In this study, 32 MRSA isolates were typed according to their antibiotic susceptibility patterns, whole-cell protein profiles, as determined by SDS-PAGE, and their plasmid profiles. The relatedness of the isolates was assessed, and the validity of each typing method evaluated. Antimicrobial susceptibility testing of the 32 isolates against 16 antimicrobial agents was done using the disk diffusion method. Sixteen different susceptibility patterns were identified. All isolates (100%) were resistant to ampicillin, penicillin, and oxacillin. Sixteen isolates (50%) were resistant to tetracycline; 12 (37.5%) to clindamicin, erythromycin, and gentamicin; 10 (31.25%) to sulfamethoxazole - trimethoprim; 9 (28.1%) to augmentin, cephalothin, and imipenem; 6 (18.75%) to norfloxacin; 5 (15.6%) to pefloxacin; 3 (9.4%) to rifampicin; and 2 (6.25%) to nitrofurantoin. However, all the isolate were susceptible to vancomycin and fusidic acid. Protein analysis by SDS-PAGE of whole-cell protein showed a total of 50-69 protein bands which were dominated by 5 major band-clusters similar in all the isolates. Variations in minor bands were evident but the crowded protein profiles rendered qualitative analysis of the isolates difficult. Instead, numerical analysis of the protein patterns was used. Correlation studies revealed a degree of similarity of the isolates ranging between 74.1 and 100%. Overall, SDS-PAGE analysis of MRSA proved to be cumbersome and not highly informative as far as subtyping MRSA isolates. PPA revealed 17 different groups among the 32 MRSA isolates. The number of plasmids varied between i and 9. A 12 Kb and a 23 KB plasmids vi were most common among the isolates with 81.25% frequency for the former and a 31.25% frequency for the latter. In addition to that, 12 isolates which had similar antibiotic susceptibility patterns were further subdivided into 7 groups by PPA. This technique proved to be a valuable typing tool for MRSA, the results were reproducible, and the banding patterns easy to interpret. REAP was attempted on 4 isolates belonging to 2 different groups according to PPA. This resulted in further differentiating 1 of the 2 groups into 2 subgroups. Taken together, the data shows that the MRSA isolates in Lebanon are highly heterogeneous. Continued monitoring and typing, using both conventional methods and PPA, of new MRSA isolates is recommended.

Book excerpt: Methicillin-resistant Staphylococcus aureus (MRSA) have been a major cause of healthcare-associated (HA) infection globally for several decades. During this time many distinct clones have emerged independently around the world, some of which have achieved pandemic status. More recently, community-associated (CA) and livestock-associated MRSA clones have also emerged, some of which have become established in hospitals and other healthcare facilities, and sometimes have displaced previously predominant HA clones. Importantly, MRSA can frequently exhibit resistance to a wide range of clinically relevant antibiotics, which limits treatment options and complicates patient management and outcomes. Investigating routes of transmission and spread of MRSA in healthcare facilities have conventionally been undertaken by combining available epidemiological information with data from DNA-based typing systems such as pulse-field gel electrophoresis typing, spa typing, multilocus sequence typing, and more recently, DNA microarray profiling. However, these approaches can frequently lack the discriminatory ability to differentiate between MRSA isolates in healthcare environments where a relatively small number of clones may predominate. The advent of high-throughput whole genome sequencing (WGS) over the last decade with the development of affordable, easy-to-use benchtop DNA sequencing platforms, associated sequencing chemistry and bioinformatics tools, has revolutionized studies of MRSA epidemiology and evolution. The significantly enhanced discriminatory power and resolution afforded by WGS has also provided hitherto unimaginable insights into the origins, emergence and factors that drive the evolution of specific MRSA clones. Furthermore, WGS has highlighted the very significant contributions of mobile genetic elements (MGEs) encoding virulence factors and resistance genes from coagulase-negative staphylococcal (CoNS) species to the emergence and evolution of MRSA. This Research Topic brings together a collection of original research articles and up-to-date reviews that highlight the significant impact WGS is having on our understanding of the epidemiology and routes of transmission of HA- and CA-MRSA in humans and the phylogenetics and evolution of specific MRSA clones. The Research Topic also highlights the impact that WGS is having on our understanding of antimicrobial resistance in MRSA by acquisition of MGEs and the role of specific CoNS species in the origins and evolution of particular MGEs that can promote the survival of MRSA following acquisition. Finally, the Research Topic highlights the immense potential impact of WGS technology in surveillance, rapid pathogen detection, identification of virulence factor profiles and antibiotic resistance genotypes, possibly from clinical samples directly.

Molecular typing of SA has been extensively used for epidemiological purposes, but often correlation between clinical findings and genotype is missing. Aim of this study is to evaluate the genetic background of SA causing community-acquired severe infections in a tertiary-care pediatric hospital in Rome, Italy, in the period January 2015 - August 2017.Case Presentation SummarySA isolated from clinical samples were retrospectively selected from community-acquired severe infections. Molecular analyses were performed for detection of genes encoding for mecA and Panton-Valentine leukocidin (PVL). MRSA strains were further investigated through the analysis of the SCCmec cassette and spa-typing. 24 SA strains were selected (source: blood 20; wound 3; nasopharyngeal aspirate 1). 14 strains were MSSA/PVL-; 5 MRSA/PVL+; 3 MRSA/PVL- and 2 MSSA/PVL+. Through spa-typing, the 16 MSSA were clustered in 11 groups. SSCmec IV-a was detected in 7 MRSA isolates. Interestingly, the 5 MRSA/PVL+ strains shared the same SCCmec (IV-a) and spa-type (t024). From a clinical perspective, among these five particular strains, 2 were responsible for severe wound infections, one for necrotizing pneumonia, one for liver abscesses in a neonate, one for hip osteomyelitis complicated by septic shock, multiple intramuscular abscess and necrotizing pneumonia.Discussion In our population, t024 was the most prevalent spa-type and was associated with MRSA/PVL+ strains, which caused severe diseases. It was not detected in MSSA strains. Molecular typing allows the monitoring of SA clusters and outbreaks, and in our experience it was also useful as a marker of severe disease. The circulation of this particular spa-type in patients not linked epidemiologically deserves further evaluation.

In this study we examined three S. aureus typing techniques; pulsed-field gel electrophoresis (PFGE), multilocus sequence typing (MLST), and Staphylococcal protein A (spa) sequencing for subspeciation of community-acquired methicillin-resistant S. aureus (CA-MRSA). It is hypothesized that PFGE will result in a higher level of discrimination among the strains, while MLST and spa typing will result in highly portable data that lacks the discriminatory power of PFGE. Thirty CA-MRSA isolates that were obtained from Florida and Washington State were characterized by molecular typing methods. Whole genome restriction analysis was performed by PFGE using the SmaI enzyme. Sequence-based typing analyses, MLST and spa typing, were performed by polymerase chain reaction (PCR) followed by sequencing. PFGE data was analyzed using the BioNumerics® software package and sequence-based data was analyzed using DNAstar®. MLST Alleles were assigned using the online MLST database (www.mlst.net) and spa types were assigned using the Ridom SpaServer (www.ridom.de/spaserver). Molecular characterization of the 30 isolates resulted in 21 pulsotypes, four MLST sequence types (STs), and six spa types. Combining data from both MLST and spa typing resulted in only seven strain categories, many of which grouped isolates that are not epidemiologically linked. These data demonstrate that techniques such as MLST and spa typing are not well suited for tracking isolates with limited evolutionary diversity such as the CA-MRSA epidemic clones.

Book excerpt: Methicillin-resistant Staphylococcus aureus (MRSA) is a pandemic human pathogen accounting for most of health-care associated infections throughout the world. However, in recent years, a more virulent strain of MRSA has emerged in the community defined as community-associated MRSA (CA-MRSA). These emerging strains of CA-MRSA are described to have different antibiotic susceptibility profiles, possess the SCCmec type IV element and usually produce the Panton-Valentine leukocidin (PVL) toxin. The majority of these CA-MRSA strains are associated with skin and soft tissue infections and necrotising pneumonia, with a 34% mortality rate. Identification and characterisation of MRSA isolates is mainly performed using phenotypic methods, which are time consuming. Little information exists on the prevalence and characteristics of MRSA isolates including antibiotic susceptibility patterns, PVL-producing CAMRSA strains, the SCCmec types and genotypes that might be circulating in the Steve Biko Academic Hospital. Identification and characterisation of MRSA isolates based on these criteria are important in controlling possible outbreaks in the clinical setting. In this study, 97 clinical MRSA isolates from the Steve Biko Academic Hospital, South Africa were collected between April 2006 to February 2007. These isolates were analysed and characterised using multiplex PCR (M-PCR), real-time PCR as well as staphylococcal protein A (spa) and hyper-variable region (HVR) typing. The aim of this study was to determine the antibiotic profiles, prevalence of MRSA isolates, the SCCmec types and the genotypes. Antibiotic susceptibility determination was performed using the disk diffusion susceptibility method as guidelined by the CLSI. Six distinct antibiotypes were identified with a total of 73%, 71%, 70% and 7% of MRSA isolates resistant to clindamycin, erythromycin, gentamicin and fusidic acid, respectively. The presence of Staphylococcus aureus specific 16S rRNA, the mecA and PVL genes was determined using a modified M-PCR assay. A total of 4% of the MRSA isolates possessed the PVL gene. Real-time PCR analysis also showed a 100% prevalence of the PVL gene in the same 4% MRSA isolates confirming the results of the first M-PCR assay. The second M-PCR was used to determine the SCCmec type prevalence and to distinguish between health-care associated MRSA (HA-MRSA) and CA-MRSA. SCCmec typing showed 67% of the isolates belonged to SCCmec type II and 14.4% SCCmec type III, both types belonging to HA-MRSA. A total of 4% of the MRSA isolates were CA-MRSA belonging to SCCmec type IVd. Genotyping results showed three distinct spa clusters whilst HVR showed six distinct clusters. Molecular-based assays proved to be useful tools to determine the prevalence and monitoring of MRSA outbreaks as well as to identify the SCCmec types, subtypes and genotypes of MRSA strains that might be circulating in the hospital. The determination of the different antibiotypes of MRSA can assist in the monitoring of the antibiotic resistant profile trends in the Steve Biko Academic Hospital, thus assisting with the correct implementation of antibiotic regimens for suspected MRSA infections. In an endeavour to assess the dissemination of MRSA strains especially PVL expressing CA-MRSA strains, it is of paramount importance to continuously monitor the emergence of these strains in clinical settings.
