Book excerpt: This supplement carries final figures on the reported incidence of notifiable diseases in 1973 based on summaries submitted to the Center for Disease Control by the individual States through the National Morbidity Reporting System and on data collected by the Tuberculosis Control Division and the Venereal Disease Control Division, Bureau of State Services, and various surveillance activities of the Bureau of Epidemiology. Data for charts and graphs are from the National Morbidity Reporting System, Bureau of Epidemiology national surveillance programs, Bureau of State Services, and the National Center for Health Statistics. The population estimates listed on page 2 and page 7 and used in computing rates are from the Bureau of the Census, Current Population Reports, Series P-25. The morbidity data in this report represent cases reported to the State health departments and subsequently to the Center for Disease Control. The accuracy of reporting, that is, the number of cases that are reported to the Center as compared with the actual number that occur, varies with disease.

Groups often neglected in analyses of national data, such as the elderly Hispanic and Asian Americans of different origin and immigrant generations, are compared. This volume documents the differentials and considers possible explanations. Selection processes play a role: selective migration, for instance, or selective survival to advanced ages. Health differentials originate early in life, possibly even before birth, and are affected by events and experiences throughout the life course. Differences in socioeconomic status, risk behavior, social relations, and health care all play a role. Separate chapters consider the contribution of such factors and the biopsychosocial mechanisms that link them to health. But no central surveillance system provides the information needed to analyze how chronic disease impacts the U.S. population, to identify public health priorities, or to track the progress of preventive efforts. A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases outlines a conceptual framework for building a national chronic disease surveillance system focused primarily on cardiovascular and chronic lung diseases. This system should be capable of providing data on disparities in incidence and prevalence of the diseases by race, ethnicity, socioeconomic status, and geographic region, along with data on disease risk factors, clinical care delivery, and functional health outcomes. This coordinated surveillance system is needed to integrate and expand existing information across the multiple levels of decision making in order to generate actionable, timely knowledge for a range of stakeholders at the local, state or regional, and national levels. The recommendations presented in A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases focus on data collection, resource allocation, monitoring activities, and implementation. The report also recommends that systems evolve along with new knowledge about emerging risk factors, advancing technologies, and new understanding of the basis for disease. This report will inform decision-making among federal health agencies, especially the Department of Health and Human Services; public health and clinical practitioners; non-governmental organizations; and policy makers, among others.
