Et si il est également un marqueur de moins bon pronostic fonctionnel avec un déficit résiduel plus fréquent et plus lourd, il constitue surtout un facteur de moins bon pronostic vital avec un risque de décès à 6 mois, 15 fois plus élevé à 80 ans qu'à 65 ans ou moins. L'objectif initial de cette étude était de décrire et analyser les taux de mortalité en post AVC à 7 jours, 1 mois, 3 mois et 6 mois de 2 cohortes de 36 patients chacune, âgé de 80 ans et plus, victimes d'un premier AVC constitué, et hospitalisés dans le département de Gériatrie du CHU de Bordeaux de manière consécutive. La première cohorte de 36 patients a été repertoriée, de mai 2001à mai 2002, pour une pré-étude servant à définir les bases et les objectifs d'un PHRC sur le "devenir fonctionnel des sujets âgés (>=80 ans) après un premier accident vasculaire cérébral" dont le promoteur est le CHU de Bordeaux. La deuxième cohorte est constituée des 36 premiers patients recrutés pour ce PHRC, sur le centre investigateur de Bordeaux, pour la période allant de août 2003à août 2004. Chaque population est constituée de 27 femmes et de 9 hommes, avec un handicap préexistant comparable, mais d'âge moyen significativement différent avec dans le premier cas 89,7 ans (+- 5.1) contre 84,4 ans (+-11.7) dans le deuxième. Les patients présentent également un état de vigilance plus grave avec respectivement un score de Glasgow à 11.42 (+-3.2) versus 12.83 (+-3.5). De plus le taux de mortalité à 6 mois est de 61% dans le premier échantillon contre 36% pour le deuxième. Nous nous sommes alors attachés à comparer ces deux cohortes, et nous avons remarqué que si les deux populations de départ sont significativement différentes, ce sont en revanche les mêmes patients qui décèdent et au même âge (l'âge moyen de décès n'est pas significativement différent avec dans le premier cas 90,4 ans +- 4.9 versus 87.6 ans +-4.5). Plus de la moitié des décès surviennent durant le premier mois (64% des décès pour le premier groupe contre 53% pour le deuxième) et quasiment 50% du total des décès sont dues à des complications infectieuses (46,1 % versus 53,3%). 75 % des décès des patients victimes d'un AVC ischémique est le fait des TACI dans la première cohorte contre 55% dans la deuxième. 70% des décédés ont un déficit moteur jugé majeur à la phase aiguë et la totalité ont des troubles vesico-spinctériens et ce quel que soit le groupe. Méthodes : étude prospective sur 67 patients hospitalisés au centre hospitalier de Valenciennes. Résultats : L’âge moyen était de 85.1 ans avec une nette prépondérance féminine. Le facteur de risque le plus souvent retrouvé était l’HTA (75 %). Plus les patients étaient âgés, plus l’état clinique initial était grave et les complications fréquentes. Les patients les plus âgés bénéficiaient moins des examens complémentaires et des progrès thérapeutiques, n’avaient pas accès à la rééducation. L’étude retrouvait 16 % de décès à la phase aiguë et 33 % de décès à 6 mois. Les facteurs de risque significatifs de décès à la phase aiguë étaient la gravité clinique initiale (NIH>10) et l’existence de complications. A 6 mois, l’âge supérieur à 90 ans devenait facteur de risque de décès. 45 % des patients vivant initialement au domicile y vivaient toujours 6 mois après l’AVC. Concernant la dépendance fonctionnelle à 6 mois, le seul facteur de risque significatif était l’existence d’une démence préalable. Conclusion : Le pronostic vital et fonctionnel de ces patients n’était pas aussi défavorable que ce qui est communément admis. L’âge ne doit pas un critère limitant la prise en charge. Des outils d’aide à la décision pourraient guider cette prise en charge.

These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines.
