Book excerpt: Diversos estudios realizados en pacientes médicos y quirúrgicos demuestran que la hospitalización en si misma es un factor relacionado con el deterioro funcional de los pacientes ancianos. La Hipótesis general de esta memoria es que si un paciente de estas características ingresa en una Unidad de Cuidados Intensivos (UCI) este hecho comporta un deterioro funcional más acusado que el de un ingreso convencional especialmente en los más mayores y la mortalidad tanto intra como extrahospitalaria debe ser elevada. Es esperable que la recuperación funcional post alta de la UCI sea lenta e incompleta especialmente en los más ancianos. Deben existir factores predictivos clínicos y/o biológicos que permitan predecir el grado de recuperación funcional de estos pacientes y es esperable un incremento en la incidencia de síndromes geriátricos. El objetivo general de este trabajo es conocer el impacto que causa la hospitalización en una UCI sobre la autonomía funcional y calidad de vida de pacientes mayores a lo largo de los 12 meses siguientes al alta hospitalaria. Como objetivos concretos : ·Analizar la mortalidad hospitalaria y acumulada a los 3, 6 y 12 meses posterior al alta de la UCI de pacientes mayores de 65 años y valorar las diferencias con el grupo de pacientes mayores de 75 años. ·Identificar factores predictivos de mortalidad en pacientes mayores de 65 años después de un ingreso por patología médica en la UCI. ·Evaluar el estado funcional y la calidad de vida a los 3, 6 y 12 meses después del alta de una UCI médica. ·Identificar los factores predictivos de buena o mala evolución funcional de estos pacientes tras el ingreso en una UCI médica. ·Evaluar la prevalencia e incidencia de síndromes geriátricos en estos pacientes Se evaluaron de forma prospectiva los pacientes mayores de 65 años ingresados en el Área de Vigilancia Intensiva (AVI) del Hospital Clínico de Barcelona por una patología médica. El criterio de inclusión hace referencia a todos aquellos pacientes mayores de 65 años que ingresan en la AVI por patología médica con una situación funcional basal preservada (Índice de Barthel ≥ 60). Se excluyó a los pacientes que ingresaron de forma electiva (postoperatorio electivo), después de la recuperación de un paro cardiorespiratorio o aquellos con una enfermedad de base con un pronóstico de vida inferior a un año. A todos los pacientes se les realizó un estudio clínico, valoración geriátrica integral, valoración de la intensidad terapéutica y valoración de la calidad de vida. Nuestros resultados demuestran que la tasa de supervivencia tras un ingreso en la UCI a los 12 meses en ancianos sanos es baja. Sin embargo observamos que entre aquellos que sobreviven al menos un año, más de dos terceras partes preservan un nivel de autonomía funcional y calidad de vida similar en comparación a su situación de base. A su vez el nivel funcional (Índice de Barthel) y la calidad de vida (EQ- 5D) en el momento del alta hospitalaria son los mejores factores predictivos para una recuperación funcional plena a largo plazo. Por otra parte sugieren firmemente que la fragilidad preclínica antes del ingreso en UCI reflejada por una puntuación más baja en el Índice de Lawton puede ser un buen marcador para identificar una población con un alto riesgo de experimentar una mala evolución tras el alta. Los pacientes con una supervivencia inferior al año eran más mayores, habían requerido con mayor frecuencia ventilación mecánica y tenían una capacidad funcional basal peor (Índice de Lawton y EQ-5D).
