Book excerpt: (Uncorrected OCR) Abstract Incidence of colorectal cancer (CRC) is rising at an alarming pace in Hong Kong during the past decades. Review of documented cases showed that, though this increase was contributed mostly by patients above 50 years of age, the overall incidence in the young population was exceptionally high when compared to most other countries. Detailed genetic studies were thus initiated. A survey of p53 mutations showed that the proportion of insertion/deletion mutations was significantly higher than that of the world database. Incidence of high-level micro satellite instability (MSI-H) showed an inverse correlation to age of cancer diagnosis. Many of these young patients had germline mutation in one of the mismatch repair (MMR) genes, thus were patients of hereditary non-polyposis colorectal cancer (HNPCC) syndrome. Multivariate analysis defined that young age at diagnosis, proximal tumour location, strong family history of CRC, and personal history of metachronous cancer were independent predictors of MSI-H phenotype. A large series of MSI-H tumours were further studied to dissect the mechanisms of MMR genes inactivation. Results demonstrated that patients with family/personal history of cancer usually had germline mutation in either hMSH2 or hMLHl, while sporadic late-onset cases showed biallelic hMLHl promoter methylation. The earlyonset, apparently sporadic cases, showed a complex mixture of genetic alterations including germline and somatic mutations, promoter methylation, loss of heterozygosity and suppression of wild type RNA by as yet undiscovered mechanisms; although at times no detectable abnormality could be found. In addition, methylation in the proximal region of the hMLHl promoter more reliably correlated with transcriptional silencing. During the course of these analyses, a novel yet inconspicuous 1.8kb germline deletion within the hMLHl gene was 11 found, which was later identified by haplotype analysis as the first founder mutation in.

However, it has been only the last decade where enormous strides have been made toward understanding neoplastic development. Explosive growth in the discipline of cancer genetics is so rapid that any attempt to review this subject becomes rapidly outdated and continuous revisions are warranted. Conclusive evidence has been reached associating specific chromosomal abnormalities to various cancers. We have just begun to characterise the genes, which are involved in these consistent chromosomal rearrangements resulting in the elucidation of the mechanisms of neoplastic transformation at a molecular level. The identification of over 50 oncogenes has led to a better understanding of the physiological process. Tumor suppresser genes, which were discovered through inheritance mechanisms, have further shed some light towards understanding the loss of heterozygosity during carcinogenesis. The message emerging with increasing clarity concerning specific pathways which regulate the fundamental process of cell division and uncontrolled growth. The advances in molecular biology have led to a major insight in establishing precise diagnosis and treatment of many cancers resulting in prevention of death. The field is expanding so rapidly that a complete account of all aspects of genetics of cancer could not be accommodated within the scope of a single volume format. Nevertheless, a few very specific topics have been chosen, which readers may find of great interest in hopes that their interest may be rejuvenated concerning the bewildering nature of this deadly disease. The contributors to Volume 3 have provided up-to-date accounts of their fields of expertise. Although the contributors have kept their chapters brief, they include an extensive bibliography for those who wish to understand a particular topic in depth.
