George Bernhard Shaw Far ahead of his time, June 1st, 1909, Alexander Maximov communicated in a lecture, given in the Charite in Berlin, the fundamental knowledge, that there exists a lymphoid hemopoetic stem cell. Alexander Friedenstein explained that during the following years, Maximov also showed that the idea of interaction between hemopoetic cells and their stroma to be one of the most significant experiences. Monoclonal antibodies, recombinant DNA technics and the improvement of tissue culture models are the major developments to improve our possibilities to clarify growth and differentiation functions of hemopoetic cells. During the last two decades it was shown that soluble products, released from T cells, were not only involved in inducing B cells to produce specific immunoglobulin secretion after antigen stimulation. Furthermore, lymphokines together with other cytokines regulate the growth and differentiation of hemopoetic cells. As I have learned from Dick Gershon, our knowledge of the cellular basis for immunoregulation has come a long way since 450 B.C. Thucydides comments on the possible role of immune response in controlling the Black Death. Dick Gershon speculated that no scientific interest for these interesting observations was put forth at that time. Perhaps the problems, the Athenians were having with the Spartans, converted money from basis research into the military budget.

Book excerpt: Relapse of leukemia following successful remission-induction therapy remains a major obstacle in the treatment of patients with acute leukemia. Leukemia recurs most frequently in patients with acute myeloblastic leukemia (AML) and high risk acute lymphoblastic leukemia (ALL) following chemotherapy and less often in patients with low risk ALL and particularly in patient groups> submitted to allogeneic marrow transplantation. ' It is likely that the great majority of these recurrences originate from residual leukemic cells that survive initial remission-induction chemotherapy. Today, several research groups throughout the world place emphasis on studies concerned with the detection and treatment of 'minimal residual disease' (MRD). These investigations are conducted with the common objective to tackle the remaining cells. 'Minimal Residual Disease in Acute Leukemia: 1986' summarizes the fast advancements in this area. Several disciplines are concerned with the analysis of leukemic cells. The perspectives of cytogenetic and molecular genetic approaches for applica tion in the detection of MRD are reviewed. In this respect, modern cyto genetics provide highly specific tumor markers. The resolution of cyto genetic methods can be particularly improved when combined with other techniques which select relevant subpopulations of cells. Characterization of oncogenes and gene rearrangements, including those of immunoglobulin and T-cell receptor genes, and the measurement of gene products, have been established. Techniques based on these approaches offer interesting tools for the detection of MRD. New possibilities of employing monoclonal anti bodies are also presented.

Book excerpt: Studies on the effect of recombinant human interferon (rIFN) on the growth of colonies of multi potential hematopoietic progenitors have demonstrated that IFN inhibits the growth of these cells in culture. The inhibitory effect of IFN was more pronounced in multilineage colony forming cells (CFU-GEMMT) of marrow cells of patients with chronic myelogenous leukemia (CML) compared with stern cells of normal marrow. T cell colony formation (CFU-TL) of phi_positive T cells obtained from primary multilineage hematopoietic colonies was inhibited by rIFN. Inhibition of T cell colony formation of Phi T cells was more profound by IFN compared with secondary T cells of healthy individuals. Southern blot analysis of secondary T cells from normal individuals revealed a germ line pattern when examined for T cell receptor (TcR) gene re arrangements using the cDNA of the beta chain. Subcloned Phi T cells of one patient with CML demonstrated gene rearrangements of the TcR. We also demonstrated the disappearance of phi_positive multilineage hematopoietic colonies when marrow cells were preincubated with IFN and subsequently cultured in the presence of IFN. The cytoconversion of the Phi chromosome in metaphases obtained from individually analyzed multilineage hematopoietic colonies suggests that IFN might allow the restoration of nonclonal hematopoiesis in patients with CML.
