Les méthodes de prélèvement actuelles sont peu satisfaisantes. L'objectif de notre étude était de démontrer qu'il est possible d'obtenir des urines en moins de 3 minutes, de manière non invasive, par stimulation vésicale (SV) chez le nourrisson n'ayant pas acquis la marche pour lequel un prélèvement urinaire est nécessaire. Matériel et méthodes : étude transversale, monocentrique, entre Septembre et Novembre 2014 aux urgences pédiatriques de Nice CHU-Lenval. Etait inclus : tout enfant de moins de 2 ans, n'ayant pas encore acquis la marche, nécessitant un prélèvement urinaire, en présence d'un des 4 investigateurs formé. La technique de SV consistait à alterner sur 3mn, tapotements sus pubien et pressions circulaires lombaires, répétée à 30mn si échec. La douleur était évaluée avec l'EVENDOL avant(T0), pendant(T1) et après la SV à 1(T2) et 5mn(T3). Résultats: Sur 142 NRS inclus de 4,7 mois (±4.0) d'âge moyen, le taux de succès d'obtention d'urine était de 55,6% à la 1ière tentative et de 23.2% à la 2nde dans un délai moyen de 63.6s (±55.1). Les scores médians EVENDOL à T0, T1, T2 et T3 étaient respectivement de 0, 6, 0 et 0 pour la1ère SV, et respectivement de 0, 7, 0 et 0 pour la seconde SV. Les taux de succès décroissent avec l'âge et le poids avec respectivement des taux variant de 86,9% chez les nouveaux nés, à 28,6% pour les + d'un an, et variant de 76,4% chez les NRS pesant moins de 4,5kg à 34,6% pour les NRS de + de 8,5kg. Conclusion: La SV semble être une alternative efficace comme moyen non invasif et indolore de prélever les urines de qualité (2è jet) avec un taux de succès satisfaisant et pourrait remplacer les méthodes actuellement employées.
