Book excerpt: Ce travail décrit la conception et la mise en place d’un systèle de gestion de dossier médical au sein d’un service de réanimation et soins intensifs chirurgicaux. Dans une première partie, on justifie la nécessité du recours à l’outil informatique en énumérant d’une part : les besoins spécifiques du service : accessibilité du dossier clinique et paraclinique, enregistrement des données issues du monitorage, allègement de certaines routines médico-administratives afin d’améliorer la disponibilité du personnel soignant et donc la qualité de la surveillane et des contacts humains avec les patients. D’autre part, il rappelle les besoins non spécifiques du service tels que gestion du dossier médical et la communication avec les autres services. Suit alors les solutions techniques et logicielles existantes, en discutant leurs modalités d’application à ce service. La seconde partie est consacrée à l’exposé des choix adoptés qui furent micro-informatiques compte tenu de l’expérience préalable du service et des possibilités étendues que cette technologie offrait, pour coût relativement modeste. Par ailleurs, la possibilité d’interface de la base de données médicales avec le réseau de monitorage (système SYCOMED en voie d’implantation) permettra par la suite d’inclure de façon automatique les données de surveillance dans le dossier médical informatisé. La troisième partie décrit la mise en place du systèlen tant logicielle que matérielle, fait le bilan des objectifs déjà atteints et de ceux qui restent en suspens pour l'instant. Enfin, il décrit les difficultés diverses inhérentes à ce genre d'entreprise, sans équivalent actuel en France, sur le plan des solutions adoptées.
