Etching times of 5, 20 and 80 seconds was used to evaluate the [micro]TBS on vital dentin with resin bonded composite [RBC] restoration (Esthet-X TM) which used a two step etch and rinse adhesive system(Prime Bond NT TM) for bonding. Methods: 26 premolars which were going to be extracted for orthodontic treatment were used for this experiment. The teeth were randomly divided into 3 groups based on the etch times and restored with RBC using two step adhesive and extracted in Guarulhos University (Brazil). After extraction, these teeth were sectioned (Microtome) in such a way to obtain 2 testing beams of 2mm diameter from each tooth were obtained which were trimmed (CNC specimen former, University of Iowa, IA, USA) to a dumb bell shape and tested for the microtensile bond strength in University of Iowa. Out of 26 teeth, 46 beams were obtained. For statistical analysis, One-way ANOVA with post-hoc Tukey-Kramer's test was used to determine whether there were significant differences in micro-tensile bond strength among three etching times under each condition. All tests had a 0.05 level of statistical significance. SAS for Windows (v9.1, SAS Institute Inc, Cary, NC, USA) was used for the data analysis. Fracture mode was analyzed using scanning electron microscopy. Results: Bond strengths in 20 seconds and 5 seconds groups were significantly greater than that observed in 80 seconds group, while there was no significant difference between 20 and 5 etching times. SEM showed majority of the failure was adhesive/ joint failure. Significance: The results in this In-vivo study are consistent with other in-vitro studies and thus bring an insight on the importance of duration of phosphoric acid application on dentin and extended etch times could compromise the bond strength of the restorations.

The repair of resin composite may hold clinical advantages. Currently, there is no available information regarding the repair potential of silorane resin composite with either silorane or methacrylate based resin composite. Objectives: The purpose of this study was to compare the repaired microtensile bond strength of aged silorane resin composite using different surface treatments and either silorane or methacrylate based resin composite. Methods: One hundred and eight silorane resin composite blocks (Filtek LS) were fabricated and aged by thermocycling between 8oC and 48oC (5000 cycles). A control (solid resin composite) and four surface treatment groups (no treatment, acid treatment, aluminum oxide sandblasting and diamond bur abrasion) were tested. Each treatment group was randomly divided in half and repaired with either silorane resin composite (LS adhesive) or methacrylate based resin composite (Filtek Z250/Single Bond Plus). Specimens were 12 blocks and 108 beams per group. After 24 hours in 37oC distilled water, microtensile bond strength testing was performed using a non-trimming technique. Fracture surfaces were examined using an optical microscopy (20X) to determine failure mode. Data was analyzed using Weibull-distribution survival analysis. Results: Aluminum oxide sandblasting followed by silorane or methacrylate based resin composite and acid treatment with methacrylate based resin composite provided insignificant differences from the control (p>0.05). All other groups were significantly lower than the control. Failure was primarily adhesive in all groups. Conclusion: Aluminum oxide sandblasting produced comparable microtensile bond strength compared to the cohesive strength of silorane resin composite. After aluminum oxide sandblasting, aged silorane resin composite can be repaired with either silorane resin composite with LS system adhesive or methacrylate based resin composite with methacrylate based dentin adhesive.

Both companies developed non-rinse substrate conditioners (i.e., Ketac Nano Primer-3M ESPE and GC Self-Conditioner-GC America) that should be used with these new materials instead of the conventional polyacrylic acid. It has been also advised by both manufacturers to use this novel substrate conditioner with the previously marketed RMGICs. Objective: to investigate whether the use of novel non-rinse conditioners (i.e., Ketac Nano Primer 3M ESPE and GC Self Conditioner GC America) as substrate pre-treatment and the new paste/paste resin-modified glass-ionomer cement, RMGIC (Ketac [trademark symbol] Nano 3M ESPE and Fuji Filling [trademark symbol] LC GC America) would affect the microtensile dentin bond strength (æTBS) of the material when compared to the traditional RMGIC with polyacrylic acid as a surface substrate pre-treatment. Materials and Methods: 96 extracted non-restored human molar were sectioned to expose occlusal dentin. Dentin surface was finished with SiC paper to standardize the smear layer. Bonding protocols of the different materials to dentin were performed following the use of two dentin conditioners. Eight groups (n=12) were tested: G1: Ketac Nano Primer + Ketac Nano, G2: Ketac Conditioner + Ketac Nano, G3: Ketac Nano Primer + Photac Fil, G4: Ketac Conditioner + Photac Fil, G5: GC Self Conditioner + Fuji Filling LC, G6: GC Cavity Conditioner + Fuji Filling LC, G7: GC Self Conditioner + Fuji II LC and G8: GC Cavity Conditioner + Fuji II LC. The specimens were stored in 37°C for 24h in 100% humidity before cutting non-trimmed beams for the æTBS with cross-sectional areas of approximately 0.8 × 0.8 mm2. Nine beams were used from each specimen. Test was done using universal testing machine at a cross-head speed of 1mm/min. Debonded specimens were examined under a stereomicroscope at 45× magnification to evaluate the failure mode. Eight randomly chosen representative debonded beams were imaged under a scanning electron microscope (SEM). Results: æTBS in MPa (mean " SE) were: G1: 9.5"1.0, G2: 11.0"1.0, G3:20.0"1.0, G4:16.8"0.9, G5: 15.1"1.0, G6: pre-test failure, G7: 20.0"1.0, G8:14.1"0.9. Weibull-distribution survival analysis was used to compare the differences in microtensile peak stress among the groups. Group5 has cohesive predominant faultier mod while the other groups have adhesive predominant failure. Conclusion: Within the limitations of this study, the use of the novel non-rinse conditioners did not improve the microtensile bond strength of new paste/paste RMGIC to dentin. In fact, the use of the novel non-rinse conditioners enhanced the bond strength of the traditional RMGIC to dentin.

When dual-cured or chemical-cured resin cements are used with these single-step adhesives, incompatibility issues exist. This has resulted in manufacturers making chemical changes in their products. Kerr Dental markets a new resin cement, Nexus Third generation (NX3), which utilizes a proprietary redox system different from the second generation of composite luting agent (NX2). The aim of this study was to evaluate microtensile bond strength and mode of failure of NX3 and NX2 with two different adhesive systems (total-etch and self-etch) after 1 week and after 3 months of storage. Methods: Sixty-four non-carious teeth were sectioned to expose the dentin using a low-speed saw. Dentin surfaces were ground with 320-grit SiC paper. The adhesives Optibond Solo Plus (SOL), and Optibond All In One (AIO) were applied, and resin cements (NX2, NX3) were used to lute 4-mm composite discs to the treated dentin surfaces. Microtensile bond strength was determined at 1 week (IM) and after 3 months (3MON) of storage using a universal testing machine (MTS). All specimens were examined under the stereomicroscope to determine the mode of failure. Random specimens from each failure group were examined using scanning electron microscopy. Statistical Analysis: Comparisons between the treatment combinations for differences in microtensile bond strength were performed using Weibull-distribution survival analysis. Comparisons between the treatment combinations for differences in the failure mode were performed using Fisher's Exact tests. The group NX3 SOL IM (30.5 MPa) had significantly higher bond strength than NX3 SOL 3MON (13.4 MPa); NX3 AIO IM (11.3MPa); NX3 AIO 3MON (8.2 MPa; NX2 AIO 3MON (5.8 MPa); NX2 SOL IM (6.3 MPa), and NX2 SOL 3MON (3.2 MPa). The group NX2 AIO IM (19.3 MPa) was not significantly different from NX3 SOL IM. The group NX2 SOL 3MON and group NX2 SOL IM had a significantly higher percentage of teeth with mixed failure than all of the other groups. None of the other groups had significantly different failure mode. The group NX3 SOL IM had 90-percent beam survival beyond 17 MPa, and NX2 AIO IM had 50 percent of beams surviving beyond 17 MPa, a better performance. For all the other groups, more than 50 percent of beams failed below 17 MPa. Results show high evidence of degradation for all groups considered in this investigation. The use of these types of cement adhesive combinations in clinical situations should be used with this understanding.

However, these systems are very technique-sensitive and time-consuming compared to newer “self-etch” or “self-adhesive” systems and have been implicated in cases of postoperative sensitivity. The purpose of this study was to compare the effects of two surface treatment protocols (self-etch vs. selective-etch) on the clinical performance of a universal adhesive and resin composite in Class V non-carious cervical lesions (NCCLs). Thirty-three volunteer subjects (17 male; 16 female; age range = 20 to 75 years) having at least two NCCLs were selected from patients of record at Indiana University School of Dentistry. Each subject received one resin composite restoration (Tetric EvoCeram, Ivoclar Vivadent) utilizing a self-etch (SfE) universal adhesive (Adhese Universal, Ivoclar Vivadent) with no separate enamel etching and another restoration utilizing adhesive and selective enamel etching (SelE) with 37% phosphoric acid (H3PO4). Both the adhesive and composite were placed following the manufacturer’s instructions. The two techniques were compared for differences in sensitivity, retention, marginal discoloration, marginal adaptation, and clinical acceptability at baseline and 6 months using the Cochran-Mantel-Haenszel tests for stratified, ordered categorical outcomes. Seventy-four restorations (37 SfE, 37 SelE) in 30 volunteers were evaluated at 12 months. No significant differences were found between the SfE and SelE groups for any variable at the 12-month recall (p>0.21). Retention was 100% at 12 months for both groups. Marginal adaptation was significantly worse at 12 months than at baseline for SelE (p=0.0163), but there was no difference for SfE (p=0.08). Sensitivity improved significantly from baseline to 12 months for both SelE (p=0.0113) and SfE (p=0.0128). The results obtained from this study are comparable to results observed in similar studies. Like similar studies involving self-etch adhesives in non-carious cervical lesions, our study showed no restorations lost to caries and excellent retention. The deterioration of selective-etch dentin margins was a result that differed from similar studies. A likely explanation for this finding would be the difficulty of controlling precise placement of phosphoric acid gel, causing undesired etching of dentin; this could result in suboptimal bonding to dentin. This report on 12-month data for a two-year study indicates significantly reduced sensitivity for both the SelE and SfE groups, and deterioration of SelE marginal adaptation. No decreases in retention, marginal discoloration, or clinical acceptability were observed in either group.

The aim of such a presentation is to present the usage in very different aspects of application of the adhesives and present specific properties of adhesives. Adhesives' advantageous properties and relatively uncomplicated processing methods contribute to their increasing application and their growing popularity in the industry, medicine and other branches. Some adhesives represent properties superior to those of most adhesive materials, due to their excellent adhesion and chemical resistance. A wide variety of adhesives' considerable flexibility in modification of properties of adhesives allows adjusting the composition to particular applications.

Book excerpt: Rampant caries observed at all ages affects those teeth and tooth surfaces which usually are regarded as immune to ordinary decay. Restoration of these lesions with a strong permanent bond between dental tissues and restorative dental materials is a highly desirable property of restorative dentistry. Ultra morphological characterization shows that the interfacial morphological and bonding systems are strongly associated and thus understanding the interfacial phenomenon would be of direct relevance in the use of dental adhesives. Chemical, physiological and micro morphological differences such as decreased mineralization, small size and lower concentration of dentinal tubules, decreased permeability and more reactivity of primary dentin to acidic conditioner were thought to be responsible for lower bond strength and sealing ability in primary dentition. Despite simplification of bonding systems, technique sensitivity and substrate variability; concerns about enamel and dentin bonds have increased. In the light of these developments this study was undertaken to compare the dentin bonding agents.
