The “2016 OECD Review of Health Systems, Mexico” is analyzed in this note from an industrial perspective. Health reform has been debated in Mexico for decades. However, the map of suppliers and insurers, as well as of costs and rules of access is the same than in past decades. Law and regulation determine the hard vertical integration of public agencies, from financing to provision of services. In consequence, Mexican health insurance and service markets are segmented. Buyers cannot identify and access low cost sources of services and families are tied to a specific public insurer. A result is the extensive use of private services to overcome waiting times offered by public providers. So far, public policy has tried to face the problem through centralized mechanism, an approach supported by the OECD. Probably, a decentralized approach that allows the revelation of costs and value of needs will be needed to overcome the costs of fragmentation of the system.
