Mais peu d'études s'intéressent à l'heure actuelle aux conséquences de ce trouble sur les capacités de communication des patients ou leur qualité de vie. Or, en contexte clinique, l'optimisation des capacités de communication est un objectif thérapeutique majeur dans le suivi de ces patients. En pratique courante, l'évaluation des troubles de parole donne des scores prédisant mal l'impact de ces troubles sur la communication. L'analyse automatique de la parole, moins variable que l'évaluation perceptive habituellement utilisée, est un axe en plein développement. Dans cette thèse, nous avons cherché à mesurer l'altération de la communication au moyen d'analyses automatiques de la parole spontanée. Nous avons étudié trois aspects : la mesure de l'altération de la communication, l'analyse automatique de la parole spontanée, et la prédiction de l'altération de la communication par les paramètres automatiques. Pour ce faire, nous avons constitué un nouveau corpus de parole auprès de 25 sujets traités pour un cancer de la cavité buccale ou de l'oropharynx. Il comprend une tâche de parole spontanée enregistrée au cours d'un entretien semi-dirigé, mais aussi des autoquestionnaires autorisant la mesure de la communication et des facteurs associés à la parole et à la communication. Concernant le premier aspect, un score de référence mesurant de façon holistique la communication a été construit. Il permet de combler le manque d'outils disponibles en cancérologie ORL pour cette mesure. Le deuxième aspect concerne l'analyse automatique de la parole. Une revue systématique de littérature nous a conduits à nous intéresser aux outils applicables à l'analyse de la parole spontanée, qui est le contexte de production le plus proche de la communication quotidienne. Cent quarante-neuf paramètres automatiques issus des différents niveaux du modèle psycholinguistique de communication de Caron ont été extraits. Puis, un processus de sélection a abouti à retenir 75 paramètres pertinents et non redondants. Enfin, pour le troisième aspect, nous avons mené une modélisation prédictive de l'altération de la communication au moyen des paramètres automatiques retenus (corrélation de 0,83 entre score prédit et score réel). La corrélation atteint même 0,89 en incluant à la modélisation des facteurs associés (constitution des cercles sociaux, état anxio-dépressif, déficits associés, auto-perception du handicap lié au trouble de parole). L'utilisation de l'analyse automatique de la parole permet donc une prédiction fiable de l'altération de communication ressentie par les patients. Cette étude ouvre de nouvelles perspectives quant à l'utilisation et l'optimisation des systèmes de reconnaissance automatique de parole dans l'évaluation clinique d'une part, et la prise en compte des besoins fonctionnels et psychosociaux exprimés par les patients d'autre part.

Nous partons de l'hypothèse selon laquelle le traitement basé sur la radiothérapie plus ou moins associée à la chimiothérapie et à la chimiothérapie d'induction préserverait mieux la parole que celui basé sur la chirurgie associée à la radiothérapie et plus ou moins à la chimiothérapie. 38 sujets ont été sélectionnés. Ils ont bénéficié d'un traitement d'une tumeur de stade Tl à T4, par chirurgie (groupe C: 12 sujets) ou par rayons (groupe R: 26 sujets). L'évaluation est intervenue au "moins un an après la fin du traitement. Les données utilisées sont un test d'articulation, un grade perceptif de dysarthrie et de résonance nasale, une mesure du débit d'air nasal (EVA), un score d'intelligibilité et une auto évaluation de la qualité de la parole (SwaIQol). En ce qui concerne l'articulation, 65% des erreurs sont réalisées par les sujets du groupe C. Les phonèmes les plus touchés sont les consonnes occlusives qui mobilisent la langue. Au niveau de l'analyse perceptive, 100% des sujets du groupe C présentent une atteinte articulatoire alors que seul 67% des sujets du groupe R présentent une atteinte. 78% des sujets obtiennent un score d'intelligibilité supérieur ou égal à 9 mots sur 10. Concernant la mesure du débit d'air nasal, 8% des sujets du groupe C et 23% des sujets du groupe R présentent une insuffisance vélaire. Au niveau perceptif, seul 20% des sujets présentent une rhinolalie ouverte. La qualité de vie, concernant la parole, est moins altérée chez les sujets du groupe R. Ainsi, l'atteinte articulatoire semble modérée après un traitement du cancer de l'oropharynx, quoique le traitement par chirurgie semble impliquer davantage d'atteinte que le traitement par rayons.
