Book excerpt: Recent developments in TB treatment have prompted the need for revision of the TB treatment outcome definitions. An online expert consultation meeting organized by WHO’s Global TB Programme in November 2020 was to determine recent changes in TB treatment affecting treatment outcome definitions and discuss options for changing the definitions. The consultation led WHO to propose new treatment outcome definitions for drug-susceptible TB (DS-TB) and drug-resistant TB (DR-TB). The purpose of this publication is to summarise changes in the definitions of tuberculosis treatment outcomes, arising after the expert consultation. The publication is to inform a wide audience about the new definitions of TB treatment outcomes so that these definitions are readily adopted and used, globally.

Following advances in the TB diagnostics and treatment pipelines since the release of these TPPs as well as recent updates to WHO TB treatment and diagnostics guidelines, a revision process of this TPP was initiated. The objective of the revision was to steer the R&D pipeline discussions to address current diagnostic gaps, seeking alignment with and patient and population needs.

Book excerpt: Between 2011 and 2019, WHO has developed and issued evidence-based policy recommendations on the treatment and care of patients with DR-TB. These policy recommendations have been presented in several WHO documents and their associated annexes, including the WHO Consolidated Guidelines on Drug Resistant Tuberculosis Treatment, issued by WHO in March 2019. The policy recommendations in each of these guidelines have been developed by WHO-convened Guideline Development Groups, using the GRADE (Grading of Recommendations, Assessment, Development and Evaluation) approach to summarize the evidence, and formulate policy recommendations and accompanying remarks. The present WHO Consolidated Guidelines on Tuberculosis, Module 4: Treatment - Drug-Resistant Tuberculosis Treatment includes a comprehensive set of WHO recommendations for the treatment and care of DR-TB. The document includes two new recommendations, one on the composition of shorter regimens and one on the use of the BPaL regimen (i.e. bedaquiline, pretomanid and linezolid). In addition, the consolidated guidelines include existing recommendations on treatment regimens for isoniazid-resistant TB and MDR/RR-TB, including longer regimens, culture monitoring of patients on treatment, the timing of antiretroviral therapy (ART) in MDR/RR-TB patients infected with the human immunodeficiency virus (HIV), the use of surgery for patients receiving MDR-TB treatment, and optimal models of patient support and care. The guidelines are to be used primarily in national TB programmes, or their equivalents in Ministries of Health, and for other policy-makers and technical organizations working on TB and infectious diseases in public and private sectors and in the community.

Available in PDF, EPUB and Kindle. Book excerpt: Pili in Mycobacterium Tuberculosis: Structure, Function, and Therapeutic Advances discusses the types of pili produced by the tuberculosis-causing bacterium, Mycobacterium tuberculosis (MTBC. The book examines the structure of pili shown under electron and atomic force microscopy and by using whole genome sequencing, outlines the importance of pili in mycobacterium tuberculosis. It also explores the role of pili in bacteria-host interactions and in drug development and delivery. Pili in Mycobacterium Tuberculosis opens with an overview of the structure, synthesis, and function of pili in bacteria. This includes taxonomy and the role of pili in diseases. The book also examines the preparation of Mycobacterium tuberculosis for electronic and atomic microscopy and looks at the different life cycles and susceptibility patterns of MTBC. It does on to discuss the expression and function of pili using whole genome sequencing and PCR-based methods and delves into bacteria host interactions. Lastly, it explores the development of new drugs and its delivery by pili. • Covers the structure, synthesis, and function of pili produced by the tuberculosis-causing bacterium, MTB • Introduces seven types of pili in MTB • Explains the mechanism of bacteria–host attachment and importance of this phenomena in MTB pili and other bacteria • Emphasizes the use of advances techniques to study and investigate the TB-bacilli and its interaction with host cells

Available in PDF, EPUB and Kindle. Book excerpt: Every year, 10 million people fall ill with tuberculosis (TB) in the world, and of those, 1.5 million people die even though it is a preventable and curable disease. As a result, TB remains the world’s leading infectious cause of death. On the other hand, in industrialized countries, the TB incidence has fallen to a historic low, and clinicians’ experience with TB diminished significantly in recent years. Additional challenges for clinicians include atypical presentation of TB in the immunocompromised, especially among the elderly, and an increasing number of patients with drug-resistant TB. Delayed diagnosis of TB leads to the spread of TB including nosocomial transmission because TB is an air-borne infectious disease, and suboptimal treatment can result in development of drug resistance. Furthermore, treatment of latent TB infection (LTBI) can prevent future TB cases but it has been under-utilized despite recent innovations in diagnosis and treatment regimens of LTBI. This book focuses on advances in diagnostic tools and treatment for both TB disease and latent TB infection. Each chapter/topic is written by one of the top TB experts in the field and the authors are from Japan and the US. Pulmonary Tuberculosis and its Prevention offers up-to-date information that can be incorporated into a busy practice of clinicians while they can appreciate broad international perspectives and gain in-depth knowledge on TB.

The target audience for these consolidated guidelines consists primarily of national TB programmes (NTPs), primary health care (PHC) programmes, maternal and child health programmes, national AIDS programmes (or their equivalents in health ministries) and other health policy makers. Book excerpt: The aim of this operational handbook is to provide practical guidance on the implementation of the World Health Organization (WHO) policy recommendations on the prevention and management of TB in children and adolescents under programmatic circumstances and at different levels of the health system. The practical guidance aims to inform the development or revision of national policies and related implementation guidance (e.g. handbooks, standard operating procedures) on the management of TB in children and adolescents. This handbook can also help countries adequately plan for the uptake of interventions to better address the specific needs of children and adolescents with or at risk of TB. It can contribute to national efforts to build capacity among national and subnational programme managers and among health workers at all levels of the health care system. The target audience for this handbook includes NTPs and other child health programmes that provide care for children with or at risk of TB, including maternal, newborn, child and adolescent health programmes, HIV services, and PHC programmes. The handbook also targets paediatricians and other health care workers (HCWs) in the public and private sectors, school health services, civil society and community-based organizations, and health care educators.

Book excerpt: Recent years have witnessed key developments in the diagnosis and treatment of tuberculosis. Alongside this, and running in direct opposition to this progress, was the COVID-19 pandemic, which had an unprecedented detrimental effect on tuberculosis control and the achievement of targets set by the End TB Strategy. This timely and important Monograph provides a crucial update on recent changes, developments and setbacks in the field, and calls for a re-commitment to the achievement of the End TB Strategy and Sustainable Development Goals. Written by authors from across the world, the Monograph covers: diagnosis; advances in treatment; prevention; and tuberculosis control challenges in different populations and contexts.
