Book excerpt: Excerpt from Medicine and Public Health in the People's Republic of China In 1969 the Fogarty International Center began a series of studies of research and health activities in other countries in relation to their political and economic structure. The first study dealt with Soviet programs in research and public health as they have evolved in the political structure established following the Bolshevik Revolution of November 1917. In late 1970 the Fogarty International Center began a search for in formation about medical and public health activities in the People's Republic of China. A long silence has shrouded that nation, which has almost one-fourth of the world's population, an ancient and respected civilization, and a long and honorable medical tradition. During 1971 communication between the United States and Mainland China re sumed after a hiatus of more than twenty years. For those who believe that the universal desire for health and relief from disease and suffering may be the strongest key to peace and international cooperation, it is significant and heartening to observe that physicians were among the first groups granted permission to visit the Pe0p1e's Republic of China. Book excerpt: “Rich insights into how one country has dealt with perhaps the most central issue for any human society: the health and wellbeing of its citizens.” —The Lancet This volume examines important aspects of China’s century-long search to provide appropriate and effective health care for its people. Four subjects—disease and healing, encounters and accommodations, institutions and professions, and people’s health—organize discussions across case studies of schistosomiasis, tuberculosis, mental health, and tobacco and health. Among the book’s significant conclusions are the importance of barefoot doctors in disseminating western medicine; the improvements in medical health and services during the long Sino-Japanese war; and the important role of the Chinese consumer. This is a thought-provoking read for health practitioners, historians, and others interested in the history of medicine and health in China.

Book excerpt: The report recommends that China maintain the goal and direction of its healthcare reform, and continue the shift from its current hospital-centric model that rewards volume and sales, to one that is centered on primary care, focused on improving the quality of basic health services, and delivers high-quality, cost-effective health services. With 20 commissioned background studies, more than 30 case studies, visits to 21 provinces in China, the report proposes practical, concrete steps toward a value-based integrated service model of healthcare financing and delivery, including: 1) Creating a new model of people-centered quality integrated health care that strengthens primary care as the core of the health system. This new care model is organized around the health needs of individuals and families and is integrated with higher level care and social services. 2) Continuously improve health care quality, establish an effective coordination mechanism, and actively engage all stakeholders and professional bodies to oversee improvements in quality and performance. 3) Empowering patients with knowledge and understanding of health services, so that there is more trust in the system and patients are actively engaged in their healthcare decisions. 4) Reforming public hospitals, so that they focus on complicated cases and delegate routine care to primary-care providers. 5) Changing incentives for providers, so they are rewarded for good patient health outcomes instead of the number of medical procedures used or drugs sold. 6) Boosting the status of the health workforce, especially primary-care providers, so they are better paid and supported to ensure a competent health workforce aligned with the new delivery system. 7) Allowing qualified private health providers to deliver cost-effective services and compete on a level playing field with the public sector, with the right regulatory oversight, and 8) Prioritizing public investments according to the burden of disease, where people live, and the kind of care people need on a daily basis.

Available in PDF, EPUB and Kindle. Book excerpt: Mary Brazelton argues that the territories and peoples associated with China have played vital roles in the emergence of modern international health. In the early twentieth century, repeated epidemic outbreaks in China justified interventions by transnational organizations; these projects shaped strategies for international health. China has also served as a space of creativity and reinvention, in which administrators developed new models of health care during decades of war and revolution, even as traditional practitioners presented alternatives to Western biomedicine. The 1949 establishment of the People's Republic of China introduced a new era of socialist internationalism, as well as new initiatives to establish connections across the non-aligned world using medical diplomacy. After 1978, the post-socialist transition gave rise to new configurations of health governance. The rich and varied history of Chinese involvement in global health offers a means to make sense of present-day crises.

Available in PDF, EPUB and Kindle. Book excerpt: This book, based on extensive original research, traces the development of China’s public health system, showing how advances in public health have been an integral part of China’s rise. It outlines the phenomenal improvements in public health, for example the increase in life expectancy from 38 in 1949 to 73 in 2010; relates developments in public health to prevailing political ideologies; and discusses how the drivers of health improvements were, unlike in the West, modern medical professionals and intellectuals who understood that, whatever the prevailing ideology, China needs to be a strong country. The book explores how public health concepts, policies, programmes, institutions and practices changed and developed through social and political upheavals, war, and famine, and argues that this perspective of China’s development is refreshingly different from China’s development viewed purely in political terms.
