If healthcare providers assist patients with making an accurate decision when making decisions related to their healthcare and comply with their medication, management of DM will be better. The manner in which this assists is that it provides more information on this topic and improves education techniques that can be more comprehendible.

Studies have shown that patients with diabetes mellitus who participate in nurse-led educational sessions improve their medication adherence and day-to-day diabetes management. The literature suggested that nurse-led educational sessions improved medication adherence and management skills among patients suffering with diabetes mellitus. The goal of an evidenced based practice change project was to increase knowledge and medication adherence in adult patients with diabetes through a nurse-led educational project. Twenty-six adult participants were enrolled in the evidenced based practice change project. Eleven of the participants completed the medication adherence post-test. All participants completed the diabetes mellitus knowledge portion of the evidenced based practice change project. Results reflected the intervention had a positive impact by increasing knowledge level and medication adherence in the participants. The results of the evidenced based practice change project exceeded the benchmark of a 28% increase in post-test scores set by the literature which suggests that implementation of a nurse-led educational intervention can improve medication adherence and patients' knowledge of diabetes mellitus management. The findings could be applied to other areas of the healthcare arena to improve medication adherence in patients with other chronic diseases. Book excerpt: "Background: Type 2 Diabetes (T2D) is a prevalent health care condition, affecting many people across the world. Despite the severity of this health condition, medication non-adherence among T2D patients has remained high, hindering them from achieving blood sugar control and desired health outcomes. Aim: To implement text messaging as an Evidence-based Intervention (EBI) to improve medication adherence and blood sugar control among T2D patients. Methods: Fifty T2D patients aged 18 years and above who had been identified as non-compliant, were engaged in the project for a period of 10 weeks. Participants' demographic information was collected using demographic survey forms. The initial step of the project involved the PM performing pretests for blood glucose. Later, blood glucose tests were performed at week 6 and week 10. Throughout the project, the participants were reminded through text messaging to take their medication, eat balanced meals, exercise, check their blood sugars, ask them how they are feeling, check their feet, remind them about clinical checks, ask them if they are feeling jittery and record blood sugar in the logs. Changes in blood sugar levels at weeks 6 and 10 were analyzed by PM. Findings: Thirty-four percent and 60% of the participants recorded improved blood sugar levels at weeks 6 and 10 respectively indicating that text message intervention at the end of the project was effective. The intervention exceeded the 50% benchmark at week 10. Implications: Text message intervention should be implemented as an evidence-based intervention to improve medication adherence among T2D patients. Nonetheless, there is a need to integrate the intervention with other interventions for more improvements in medication adherence.
