Although therefore the rules laid down will generally sufice, yet you will occasionally meet with cases in which the accustomed signs are absent, or in which unusual symptoms are present. For instance, no complaint has more strongly marked signs than peritonitis the excessive and general pain of the abdomen, the great tenderness on pressure, the rapid, wiry pulse - and yet you may meet with fatal peritonitis with scarcely any pain, or with out tenderness, or with a pulse not above the normal standard. In order to obtain the necessary Skill in diagnosis, it will be requisite that you should practise the taking Of cases. You should record the symptoms and phy sical signs present in each case, the order in which the symptoms have been developed, the treatment adopted, the progress of the disease, and, if it terminate fatally, you should add the morbid appearances discovered after death. You will readily understand that unless some plan is adopted, there is great probability that you will either encumber your description Of the dis ease with a number Of unnecessary details, or overlook important facts. I have therefore added the following suggestions for a plan, which you may perhaps find useful until experience enables you to propose a better. Commence with the name and address of your patient, his age, and occupation. - The age is important, because many diseases, such as cancer, are chiefly found at cer tain periods of life. The nature of the occupation Often gives a clue to the complaint; as, for instance, painters and other workers in lead are especially liable to colic and paralysis. In place of this we have been presented with the theories of closet practitioners, and the vagaries of dreamers, who were always more persistent in making known their views than the modest toiler by the bedside of the sick, in the dea'-room, or in the laboratory. There has, however, been a great change in this regard within the last quarter of a century; and men now value the facts of observation more than the conclusions arrived at by reasoning from imperfect data. Thus, the practice of medicine has attained such development that it may he regarded as a science. Webster defines science to be "a collection of the general principles or leading truths relating to any subject, arranged in a systematic manner." In order, therefore, that the practice of medicine should he termed scientific, it is necessary that there he certain well-defined truths, and that these he so grouped together as to establish fixed principles. Many deny that there is any science in the practice of medicine, and only admit it in chemistry and anatomy, out of the seven branches taught in our colleges. Others, and among these our best writers, while claiming that medicine is a science, almost entirely ignore it in practice, and in writing, results are given as almost entirely or quite empirical. That practical medicine is an art to he studied, and perfected by continued practice, must he admitted, but I claim that there is an underlying scientific basis that needs to be known to give the greatest success. The empiric may have success, hut it is only after lie has generalized his observations. and formed in his own mind, certain definite principles. The efforts of Newton were not directed to the discovery of original individual facts, but rather to the discovery of the connecting links which should serve to bind together the individual facts and minor generalizations attained by his predecessors, into one great Whole. In attempting to accomplish for Medicine, what Newton accomplished for what is called Physical Science, we may seem to attempt the unattainable; but believing that there is nothing in the nature of our subject which forbids a reasonable expectation of its accomplishment, - believing that the individual facts and minor generalizations are abundant and ready, and that the advanced Medical Mind is now prepared to receive and appreciate their connection, we boldly encounter the task; resting all upon the validity of our arguments, hoping for nothing which cannot stand the test of the severest logical criticism. That there is nothing in the present state of Medicine which forbids the conclusion that our object is attainable, may be seen in the fact, that Astronomy and all that are bow called exact sciences, were once in a state of Empiricism - once occupied the same ground, that medicine now occupies, - once encountered the same difficulty, - the Cause of physical phenomena being deemed occult, innate, mysterious and past finding out - having its analogy in the Vital Force of physiologists.
