Researches indicate that maternal prenatal emotional well-being influences the maternal-fetal attachment (MFA) which has important implication in postnatal bonding. The aim of this study was to examine the influence of maternal prenatal anxiety and depression on MFA in high risk pregnancies. Methods: A cross-sectional study was conducted between October 2017 and July 2018 on 95 high-risk women in their third trimester of pregnancy, hospitalized in a gynecology-obstetrics service in a public hospital in Tunis. The Hospital Anxiety and Depression Scale (HADS) was used to assess states of depression and anxiety. The Prenatal Attachment Inventory (PAI) was used for evaluation of MFA. Results: The recruited women were aged between 18 and 45 years old and the majority had a secondary level of education and 58% had previous children. Mean score on PAI was 57,21 (SD=9,04). The mean scores on the HADS was 17,4 with an average of 9,44 on the anxiety subscale and 7,96 on the depression subscale. According to the HADS scores, the sample was divided into 4 groups: definite cases of anxiety and depression (14,7%), anxiety without depression (24,2%), depression without anxiety (5,3%) and a group without anxiety nor depression (55,8%). PAI scores distribution was significantly different between these groups (p=0,049). Moreover, the PAI scores correlated negatively and significantly with the HADS total score (p=0,034) but when examining subscale scores, this correlation was found to be attributed to the depression dimension (p=0,046) and not to the anxiety one (p=0,061).Conclusion: This study highlights the importance of assessing for emotional wellbeing in at risk pregnancies especially depressive and anxiety symptoms given their influence on prenatal attachment.

Book excerpt: Objective : The aim of this study was to examine what effects the temperament and character of high risk pregnant women had on their depression, anxiety, and maternal fetal attachment.Methods : A hundred and five high risk pregnant women were recruited in the maternity ward of the Keimyung University medical center. All ubjects were administered a sociodemographic and obstetric questionnaire, Temperament and Character Inventory, Beck Depression Inventory II, State Trait Anxiety Inventory State, and Maternal Fetal Attachment Scale.Results : First, higher scores on the harm avoidance scale and lower scores on the self directedness scale were associated with higher depression scores. Second, higher scores on the harm avoidance and reward dependence scale were associated with higher anxiety scores. Lastly, higher scores on the cooperativeness scale were associated with higher maternal fetal attachment scores.Conclusion : Temperament and character in high risk pregnant women were associated with the degree of depression, anxiety, maternal fetal attachment. Based on these findings, it is suggested that examining temperament and character in advance and screening for vulnerability in pregnant women can help to prevent emotional problems for high risk pregnant women.
