Book excerpt: La fréquence des décès est plus élevée en court séjour gériatrique que dans le reste des services de court séjour. La surmortalité observée est souvent interprétée par les soignants et les familles des patients comme une fatalité liée à l'âge. L'objectif de notre étude est de contribuer à discerner le rôle de l'âge du rôle joué par d'autres facteurs dans la survenue du décès en gériatrie. Notre travail consiste en une lecture critique de la littérature suivie d'une étude rétrospective conduite chez 100 patients décédés et 100 patients survivants entre le 1er juillet 2007 et le 15 avril 2008 en court séjour gériatrique du CHU de Besançon. Les données issues de la littérature sont controversées quant au rôle attribué à l'âge dans la survenue du décès. Dans notre échantillon, l'âge moyen des patients décédés en cours d'hospitalisation est statistiquement supérieur à celui des patients vivants à leur sortie d'hospitalisation. La précarité nutritionnelle et fonctionnelle, la présence d'escarres ou de nombreuses comorbidités au premier rang desquelles les néoplasies et le tabagisme paraissent rétrospectivement être des marqueurs associés à une surmortalité hospitalière des patients hospitalisés en court séjour gériatrique. Les hospitalisations motivées par une altération de l'état général ou une dyspnée sont statistiquement plus souvent associées à une évolution fatale que les autres. L'âge pourrait être un marqueur de mauvais pronostic lors d'une hospitalisation en court séjour gériatrique. Cette variable ne peut toutefois être utilisée isolément, et doit être confrontée aux comorbidités, au motif d'hospitalisation, et au statut nutritionnel et fonctionnel de la personne âgée hospitalisée. A ce titre, l'évaluation gériatrique globale pourrait êtrer un outil standardisé de grande valeur pronostique.
