Book excerpt: This document is Volume I of a three-volume report which describes the development of a medical monitoring program for marine personnel who may be potentially exposed to hazardous chemicals during their work. The purpose of the program is to screen for adverse health effects of occupational exposures to noise and toxic chemical substances. A basic medical protocol for periodic screening examinations and a target organ protocol for examinations performed in response to acute overexposure to specific toxic substances are described. A recommended schedule for medical examinations is specified and guidelines for follow ups of abnormal laboratory results are provided. Auxiliary systems to support implementation and application for the medical monitoring system include: 1) A hazardous substance data system for nearly 700 marine hazardous substances, 2) A set of biochemical and medical data sheets for 179 of the more toxic marine hazardous substances, 3) Guidelines for biological monitoring of exposures to 26 marine hazardous substances for which methods are available, 4) Two methods for documentation of potential exposures to marine hazardous chemicals: one which is self-documented, and a second, which employs an indirect method based on cargo manifests and personnel records 5) Guidelines for an industrial hygiene program to complement medical monitoring, and 6) A data management system to provide a record of data and to provide a means of rapid access to data files for specific information and for data summaries.

The goal of this plan is to minimize the worker's exposure to hazardous materials during work activities. The environmental monitoring plan includes the use of (1) safe work practices that specify how to carry out activities that involve potential exposure to hazardous materials, (2) environmental monitoring devices for measuring chemical vapor or dust concentration in the air, and (3) personal protective equipment to prevent inhalation of and skin contact with hazardous materials. The environmental monitoring plan was subjected to a trial evaluation in a series of field tests. These field tests included two job activities in which workers were potentially exposed to hazardous chemicals for (1) U.S. Coast marine inspectors and pollution prevention boarding teams, (2) merchant marine tankerman working on chemical tankerships, and (3) merchant marine tankerman working on chemical barges. Book excerpt: A large number of chemicals are used on land at shore facilities, in the air in combat and reconnaissance aircraft, on seas around the world in surface vessels, and in submarine vessels by the navy and marine corps. Although the chemicals used are for the large part harmless, there is a significant amount of chemicals in use that can be health hazards during specific exposure circumstances. The Navy Environmental Health Center (NEHC) is primarily tasked with assessing these hazards. The NEHC completes its tasks by reviewing toxicological and related data and preparing health-hazard assessments (HHAs) for the different chemicals. Since the NEHC is continually asked to develop these HHAs, the National Research Council (NRC) was asked to assess independently the validity and effectiveness of NEHC's HHA process, in order to determine whether the process as implemented provides the Navy with the best, comprehensive, and defensible evaluations of health hazards and to identify any elements that might require improvement. The task was assigned to the Board on Environmental Studies and Toxicology's Committee on Toxicology's (COT's) Subcommittee on Toxicological hazard and Risk Assessment. Review of the U.S. Navy Environmental Health Center's Health-Hazard Assessment Process presents the subcommittee's report. The report is the work of expertise in general toxicology, inhalation toxicology, epidemiology, neurotoxicology, immunotoxicology, reproductive and developmental toxicology, pharmacology, medicine, risk assessment, and biostatistics. It is based on its review of documents provided by NEHC, presentations by NEHC personnel, and site visits to NEHC in Norfolk, Virginia and an aircraft carrier in San Diego, California.
