Available in PDF, EPUB and Kindle. Book excerpt: Since its publication in 1998, The SAGES Manual: Fundamentals of Laparoscopic and GI Endoscopy has become a true classic. Now, in its new second edition, the manual continues to be a comprehensive, concise and timely contribution to the field of minimally invasive surgery. The text provides an authoritative synopsis of the major minimally invasive procedures in an easy-to-read outline format. The text has been fully revised with new chapters added to reflect advances in the field. The new edition now features laparoscopy, endoscopy and a new section on thoracoscopy. The laparosopic section reviews the important basics of laparoscopic surgery, including general principles, diagnostic laparoscopy and biopsy, laparoscopic cholecystectomy and common bile duct exploration, laparoscopic gastric surgery, procedures on the small inestine, appendix and colon, the pancreas, spleen and retroperintoneum, hernia repair, and pediatric laparoscopy. The endoscopic section reviews gastrointesinal endoscopy for the general surgeon. It begins with an overview of the general principles of flexible endoscopy, and continues to discuss such procedures as upper gastrointestinal endoscopy, choledochostomy, colonoscopy, and pediatric endoscopy. The thoracoscopy section covers the basic principles of thoracoscopic surgery, including patient preparation and positioning, exploratory thoracoscopy for staging malignancies, esophagomyotomy, and operations for esophageal diverticula. New to the second edition are chapters on laparoscopic bariatric surgery, using hand-assisted devices in laparoscopy, and a new set of chapters on thoracoscopy. 50 new illustrations complement the text. A must-have reference for the general surgery resident or practicing surgeon.

Book excerpt: The practice patterns of general surgery have changed significantly in the last decade as a result of the increasing use of flexible endoscopy, both diagnostic the introduction of laparoscopic endoscopy as a and therapeutic, and also therapeutic technique. These changes have occurred over a short period of time and continue rapidly with the increasing use of advanced laparoscopic proce dures. Residents who have completed their training and are entering the prac tice of general surgery must be very familiar with, and must be well educated in, these two areas. Flexible endoscopy and laparoscopy will be a significant part of their practice and with time will become more important and more widely used. Residency programs are struggling to develop educational programs that will adequately prepare residents for the future of endoscopy and laparoscopy. This is difficult in some programs because of the lack of clinical endoscopic material, the lack of understanding of the importance of endoscopic techniques in their medical communities, and a lack of interest and training of their staff. There is also a lack of good textbooks or educational tools aimed directly at residents to help these programs. There is a definitive need for a comprehen sive textbook directed at the education of residents.

Book excerpt: The second SAGES (Society of American Gastrointestinal Endoscopic S- geons) manual was intended to be a companion piece for the successful ?rst SAGES manual, edited by Carol Scott-Connor, that was published more than 4 years ago. Originally, the goal was to concentrate on tersely covered or often ignored aspects of the preoperative preparation of the patient and the operating room as well as the postoperative care of patients undergoing minimally in- sive operations. It was also our intention to include a section for each procedure where several different port placement schemes would be presented and brie?y discussed. Unique to this manual, the impact of the patient’s body habitus (short or long, narrow or wide) on port placement is also taken into account for many of the procedures. Also unique are chapters devoted to hypothermia, port wound closure, and the management of subcutaneous emphysema and abdominal wall hemorrhage caused by trocars. Naturally, the surgeon tends to focus on the technical aspects of the pro- dure, such as the operative tasks to be carried out, the order of operation, and the position of the surgeon and assistant. However, it is critical that the surgeon be aware that the CO pneumoperitoneum, far more so than laparotomy, results 2 in multiple physiologic alterations that, if not compensated for by the anest- siologist and surgeon, may endanger the patient or prevent the laparoscopic c- pletion of the procedure.
