Por eso también se dedica al cuidado y protección de las funciones de sistemas vitales, como el nervioso central, el nervioso autónomo, el cardiovascular, el respiratorio, el hepatorrenal y el hematopoyético, con el propósito de mantener la estabilidad hemodinámica y la homeostasis del organismo en los pacientes que presentan un muy mal estado general por desnutrición, anemia, depresión del sistema inmunitario, con estados de disfunción endocrina, hepática, renal y cardiovascular; todo esto debido a los tratamientos instituidos contra el cáncer. La incidencia de casos nuevos por cáncer aumenta día a día, por lo que se reconoce éste como un importante problema de salud pública debido a la alta frecuencia de casos, a su impacto económico y a su trascendencia social. A nivel mundial es la segunda causa de muerte después de las enfermedades cardiovasculares. En la atención del paciente con cáncer es muy importante evaluar la posibilidad de un tratamiento quirúrgico que implicará cirugía radical de gran duración y pérdidas sanguíneas importantes; la mayoría de los pacientes son de edad avanzada y padecen de importantes enfermedades concomitantes descompensadas, como diabetes mellitus, hipertensión arterial, cardiopatías, etc. Todo esto someterá al especialista a duras pruebas de habilidad y conocimiento para poder elegir la técnica anestésica ideal para cada caso en particular, y encontrará dificultades durante la inducción, la intubación, el mantenimiento, la emersión y el control posoperatorio inmediato.

The neurologic symptoms are often more disabling than the primary cancer. Symptoms including confusion, seizures, pain and paralysis may be a result of either metastases to the nervous system or one of several nonmetastatic complications of cancer. The physician who promptly recognizes neurologic symptoms occurring in a patient with cancer and makes an early diagnosis may prevent the symptoms from becoming permanently disabling or sometimes lethal. This monograph, an update of the first edition published in 1995, is divided into 3 sections. The first classifies the wide variety of disorders that can cause neurologic symptoms the patient with cancer, discusses the pathophysiology of nervous system metastases, the pathophysiology and treatment of brain edema and the approach to supportive care of common neurologic symptoms such as seizures, pain, and side effects of commonly used supportive care agents. The second section is devoted to nervous system metastases, addressing in turn, brain, spinal cord, meningeal and cranial and peripheral nerve metastases, describing clinical symptoms, approach to diagnosis and current treatment. The third section addresses several nonmetastatic complications of cancer and includes sections on vascular disease, infections, metabolic and nutritional disorders, side chemotherapy, radiation and other diagnostic and therapeutic procedures. The final chapter addresses paraneoplastic syndromes.The book is intended for practicing oncologists, neurologists, neurosurgeons and radiation oncologists as well as internists who treated patients with cancer. Our attempt was to write a book that would assist oncologists in understanding neurologic problems and neurologists in understanding oncologic problems. The book is also intended for physicians training to specialize in any of the above areas. It includes a practical approach to the diagnosis and management of patients with neurologic disease who are with known to have cancer or in whom cancer is suspected.

The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one." To this end, contact information is provided to assist potential users of the method.
