Available in PDF, EPUB and Kindle. Book excerpt: Stroke is a condition that predominantly affects older people, often leading to death, disability and dependency as well as occupancy of hospital and nursing-home beds. Older stroke patients are similar in many ways to their younger counterparts, but at the same time exhibit several key differences. Their outcome and care are complicated by delayed diagnosis, polypharmacy, difficult rehabilitation, ageism, false assumptions of poor outcome, multiple co-morbidity, social issues including implications for independent living, ethical dilemmas, and many others. The proportion of older people is increasing every day and with it the burden of disease and disability. The implications this has for health services are immense, especially for long-term conditions. Despite this there is limited literature available to clinicians on stroke with a particular focus on this age group. Traversing the whole stroke pathway, Stroke in the Older Person brings together key discussions on every aspect of the disease as it affects the older person, including its general aspects and those very specific to the older populations. All chapters are written by highly experienced clinicians that offer up-to-date evidence-based information as well as practical tips to promote excellent, empathetic care to older patients. Over 30 chapters, this resource addresses the epidemiology, aetio-pathogenesis, clinical presentation, diagnostic work-up (including imaging), primary and secondary prevention, and rehabilitation of older people. There is a special focus on intracerebral haemorrhage, carotid re-vascularisation, transient ischaemic attack, cognitive impairment, research, ethical and moral dilemmas including DNAR, advanced directives and end-of-life care.

This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Background: Stroke survivors often experience various problems which may impact their qualities of life if not sufficiently addressed. Post-stroke problems beyond functional disabilities are often neglected and may not be picked up by caregivers/clinicians for further management.Aim: We aimed to study post-stroke complications reported by a cohort of ischemic stroke patients in Singapore and compare them with complication burden perceived by their caregivers using the Post-Stroke Checklist (PSC). Methods: We surveyed 40 ischemic stroke patients (median age 68.5 years) and their caregivers at 3 months post-stroke, to assess for post-stroke complications. The 13-questions PSC was administered separately to patients and their caregivers via telephone interview by trained study coordinators. Results: The mean problem number from patientsu2019 survey was 3.7 (u00b12.9), while it was 3.0 (u00b12.9) from the caregiversu2019 survey. There was good agreement (92.5%) for the questions on problems with activities of daily living. However, disagreements were greater for mood (27.5%), absence of secondary prevention (25%), cognition (25%), personal relationships (22.5%) and other challenges interfering with recovery (30%). Differing opinions in other areas were: mobility (17.5%), spasticity (20%), pain (17.5%), incontinence (20%), communication (20%), life after stroke (15%) and fatigue (17.5%). Conclusion: There was good concordance for some aspects of the PSC between patients and caregivers, which were more observable. However, there are other aspects with poor agreement possibly because they were more subjective and not demonstrated outwardly. It is important to consider these findings when screening consequences of stroke and to appropriately consider the opinions from caregivers.

Available in PDF, EPUB and Kindle. Book excerpt: This open access book focuses on practical clinical problems that are frequently encountered in stroke rehabilitation. Consequences of diseases, e.g. impairments and activity limitations, are addressed in rehabilitation with the overall goal to reduce disability and promote participation. Based on the available best external evidence, clinical pathways are described for stroke rehabilitation bridging the gap between clinical evidence and clinical decision-making. The clinical pathways answer the questions which rehabilitation treatment options are beneficial to overcome specific impairment constellations and activity limitations and are well acceptable to stroke survivors, as well as when and in which settings to provide rehabilitation over the course of recovery post stroke. Each chapter starts with a description of the clinical problem encountered. This is followed by a systematic, but concise review of the evidence (RCTs, systematic reviews and meta-analyses) that is relevant for clinical decision-making, and comments on assessment, therapy (training, technology, medication), and the use of technical aids as appropriate. Based on these summaries, clinical algorithms / pathways are provided and the main clinical-decision situations are portrayed. The book is invaluable for all neurorehabilitation team members, clinicians, nurses, and therapists in neurology, physical medicine and rehabilitation, and related fields. It is a World Federation for NeuroRehabilitation (WFNR) educational initiative, bridging the gap between the rapidly expanding clinical research in stroke rehabilitation and clinical practice across societies and continents. It can be used for both clinical decision-making for individuals and as well as clinical background knowledge for stroke rehabilitation service development initiatives.
