Book excerpt: It is the position of the National Association of School Nurses that the management of pediculosis (infestation by head lice) should not disrupt the educational process. No disease is associated with head lice, and in-school transmission is considered to be rare. When transmission occurs, it is generally found among younger-age children with increased head-to-head contact (Frankowski & Bocchini, 2010). Children found with live head lice should remain in class, but be discouraged from close direct head contact with others. The school nurse should contact the parents to discuss treating the child at the conclusion of the school day (Frankowski & Bocchini, 2010). Students with nits only should not be excluded from school, although further monitoring for signs of re-infestation is appropriate. It may be appropriate to screen other children who have had close head-to-head contact with a student with an active infestation, such as household family members, but classroom-wide or school-wide screening is not merited (Andresen & McCarthy, 2009). In cases that involve head lice, as in all school health issues, it is vital that the school nurse prevent stigmatizing and maintain the student's privacy as well as the family's right to confidentiality (Gordon, 2007). The school nurse, as a student advocate and nursing expert, should be included in school district-community planning, implementation, and evaluation of vector control programs for the school setting. School nurses are also in a pivotal position to dispel myths and stigmas regarding pediculosis by providing education on the life cycle of the louse, methods of transmission, treatment options and care of the environment to the student's family, school and community at large. Despite the use of medicated shampoos and treatments, the prevalence of head lice continues to increase. Many parents have the misconception that medicate shampoos alone will eliminate head lice. As a result, many children experience recurrences due to ineffective treatment. Many research studies have been conducted regarding the effectiveness of medicated shampoos and treatments; however, no research is available that addresses the effect of parent education on appropriate head lice treatment and decreased recurrences of head lice. The purpose of this outcome study, which used a retrospective design, was to determine whether parent education on the appropriate treatment for head lice decreased recurrences in school-aged children. The expectation was that there would be a decrease in the number of head lice recurrences when parents were educated about appropriate treatment at the time of the first infestation. A small, rural school district served as the sample setting. All school-age children infested with pediculosis ho were sent home from school during the 1998-1999 (School Year One), 1999-2000 (School Year Two) and 2000-2002+1 (School Year Three) school years were identified from the school nurse's pediculosis case log and daily health office records. During school year one, parents were notified to pick up their child if head lice infestation was identified. Educational materials were not provided and appropriate head lice treatment was not discussed. During school year two and three, parents were called to pick up the child if pediculosis was identified. In addition, the school nurse met with the parent to discuss head lice treatment, identified the nits and or live lice in the child's hair for the parent, and answered any questions. Simple frequencies and percentages were used to compare the number of head lice infestations between school years and to evaluate the effectiveness of parent education in decreasing recurrence.

Head lice are associated with social stigma and are typically managed by home management and social and school exclusion. Pesticides and chemicals are the most widely recommended treatments for head lice. These products used inappropriately have the potential to cause harm to humans and the environment. Anecdotal evidence and research studies indicate that there are inappropriate and unnecessary exposures of young populations and the environment to chemical treatments used to treat lice infestations. There is a gap between the knowledge about head lice and the information that those treating head lice receive and their actions taken to eradicate head lice infestation.Purpose: The purpose of this grounded theory study is to explore the decision-making process that caregivers use regarding management of head lice for their children.Method: Qualitative constructive ground theoryFindings: Battling stigma: A grounded theory of the process of combating head lice emerged from the findings that parents described when they were confronted with decision making in the context of a stigmatized condition such as head lice. The process is cyclical and describes a variety of behaviors and actions that are guided by stigma and misinformation. The cycle repeats because parents learn experientially using misinformation to guide their decisions. The following constructs are included in the process: Perceived Equilibrium, Surprise Attack, Preparing for Battle, Combating, Coping with Aftermath, and Readjusting Sense of Self.Conclusion: The study highlights the actions and consequences that occur when parents react emotionally as opposed to using their everyday cognitive decision-making process for childhood illnesses. The consequences result in financial, health, and environmental harm. The process repeats and perpetuates ongoing stigma and misinformation concerning head lice. Nursing interventions and policy advocacy are necessary to provide a pathway for de-stigmatization of the condition and for more efficacious treatment with fewer health and environmental impacts. Applying the model to other stigmatized health conditions may give evidence for a much broader usage and utility of the model.
