In Mammography Wars, sociologist Asia Friedman examines cultural and medical disagreements over mammography. At issue is whether to screen women under age fifty, which is rooted in deeper questions about early detection and the assumed linear and progressive development of breast cancer. Based on interviews with doctors and scientists, interviews with women ages 40 to 50, and newspaper coverage of mammography, Friedman uses the sociology of attention to map the cognitive structure of the “mammography wars,” offering insights into the entrenched nature of debates over mammography that often get missed when applying a medical lens. Friedman’s analysis also suggests the sociology of attention’s unique potential for analyzing cultural conflicts beyond mammography, and even beyond medicine.

In The Big Squeeze, Dr. Handel Reynolds, a practicing radiologist, notes that this episode was only the most recent controversy in the turbulent history of mammography since its introduction in the early 1970s. In a book written for the millions of women who face the decision about whether to get a mammogram, health professionals interested in cancer screening, and public health policymakers, Reynolds shows how pivotal decisions made during mammography's initial launch made it all but inevitable that the test would be contentious. He describes how, at several key points in its history, the emphasis on mammography screening as a fundamental aspect of women's preventive health care coincided with social and political developments, from the women's movement in the early 1970s to breast cancer activism in the 1980s and '90s. It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.

Although cardiovascular-related deaths significantly outnumber breast cancer deaths, breast cancer is arguably the most feared diagnosis among American women. Great strides have been made to heighten public awareness of breast cancer, with particular emphasis on early detection with mammography. Breast radiologists regularly witness the extreme anxiety that just the thought of being diagnosed with breast cancer can cause a patient. This anxious anticipation is often heightened by a lack of knowledge about what to expect from the process of breast imaging evaluation, which is frustrating for both patient and practitioner and can negatively affect the experience. Physicians often encounter patients who have little or no understanding of the reasoning behind the examination or procedure about to be performed-sometimes even up to the day of their breast cancer surgery. Furthermore, most women who undergo breast evaluations will not be diagnosed with cancer. The incidence of breast cancer is only 125.3 per 100,000 women (or 3-6 in every 1,000 screenings), however, symptoms of benign breast abnormalities are quite common and impact many more lives. Accurately diagnosing these non-cancerous conditions can alleviate much anxiety, in addition to helping patients towards a correct treatment plan. It is carried out by both government and private organisations with the aims of: better prognosis/outcome for individuals; to protect society from contagious disease; to allow rational allocation of resources; to allow selection of healthy individuals; and for research purposes. About £500 million is spent on screening each year in Britain alone, and it is an issue that has relevance in health systems and for the general public and media. For many years, screening was practised without debate, but in the 1960s serious challenges were raised about standard screening procedures. Benefits of screening must be judged against negative side-effects, and concern was raised about potential and actual harm arising when people without a health problem received dangerous and unnecessary investigations and treatments as a result of 'routine' screening tests. Gardner traces women's cancer education campaigns back to the early twentieth century. Focusing on breast cancer, but using research on cervical, ovarian, and uterine cancers as well, Gardner's examination of films, publications, health fairs, and archival materials shows that women have promoted early cancer detection since the inception of the American Society for the Control of Cancer in 1913. While informing female audiences about cancer risks, these early activists also laid the groundwork for the political advocacy and patient empowerment movements of recent decades. By the 1930s there were 300,000 members of the Women's Field Army working together with women's clubs. They held explicit discussions about the risks, detection, and incidence of cancer and, by mid-century, were offering advice about routine breast self-exams and annual Pap smears. The feminist health movement of the 1970s, Gardner explains, heralded a departure for female involvement in women's health activism. As before, women encouraged early detection, but they simultaneously demanded increased attention to gender and medical research, patient experiences, and causal factors. Our understanding of today's vibrant feminist health movement is enriched by Gardner's work recognizing women's roles in grassroots educational programs throughout the twentieth century and their creation of supportive networks that endure today.
