Book excerpt: In order to explore the causes of malnutrition, this paper estimates reduced-form nutritional status functions that include a linear combination of independent variables, which explain the household's per capita consumption expenditures. Instrumented consumption expenditures are considered a good proxy for permanent income, and the findings indicate that they are an important determinant of long-term (or chronic) malnutrition. Income, however, does not have a significant effect on current (or acute) malnutrition. Children from households that allocate a larger share of their land to producing export crops than food crops did not display more stunting or wasting. Mothers with more education will be less likely to have children who suffer from acute malnutrition, when controlling for income levels. The education of the father, however, does not confer the same positive benefits upon his children's nutritional welfare, except as mediated through higher earnings. Parental height, especially of women, has an important impact on long-term nutritional status. The characteristics of the village in which the household resides also plays an important role in determining levels of malnutrition.

Yet, many countries continue to face moderate to high degrees of undernourishment, especially where economies made least progress in transitioning towards high-productivity, modern agriculture and non-farm economic development and where policy stances have been weak in promoting agricultural development, reducing gender inequalities, and improving infrastructure and basic social services. The decline in undernourishment has come with a rise in the prevalence in overweight and obesity. The spread of this form of malnutrition has come with dietary shifts towards the consumption of more animal-sourced and processed foods that have accompanied urbanization and income growth. By 2015, the vast majority of countries faced moderate to high prevalence of adult overweight, and this form of malnutrition is also on the rise in countries with still significant rates of child undernourishment. No country in the world is showing declines in the rate of adult overweight.

In many developing countries, less than half of all primary school students have enrolled by age six, and many do not enroll until age eight or nine. This paper uses data from the Ghana Living Standards Survey to explore this phenomenon. The authors develop a number of explanations for delayed primary school enrollment in Ghana, but their main focus is on nutrition. They find that infant and child malnutrition has a major impact on the age at which children enroll in school. They argue that chronically malnourished children tend to be kept out of school by their parents because they perform poorly and the benefit to them of schooling is therefore low. Chronic malnutrition, which is extensive in Ghana, has been shown to stunt growth, retard mental development, and reduce motivation and energy levels. Because growth can eventually compensate for the initial retardation caused by malnutrition, the authors suggest that there may be an QUOTEoptimal ageQUOTE of primary school enrollment for malnourished children that is higher than that for other children. The authors also explore the effects of malnutrition on the number of years of schooling completed. By taking enrollment age into account when analyzing the statistics on dropouts, the authors are able to remedy a major shortcoming of previous studies. They thus develop a method of identifying the further negative effects of malnutrition even when delayed enrollment is common.

Book excerpt: Researchers and policymakers are paying increasing attention to the nexus of hunger, malnutrition, and public health, and to the related measurement of food and nutrition security. However, focusing on proxy indicators, such as food availability, and on selected head count figures, such as stunting rates, gives an incomplete picture. In contrast, global burden of disease (GBD) studies are outcome based, they follow an established methodology, and their results can be used to derive and monitor the burden of chronic and hidden hunger (undernutrition) at the global level. Judging by this measure, the international goal of halving global hunger between 1990 and 2015 has already been achievedwhich is in stark contrast to the picture that emerges if the first Millennium Development Goals indicator for measuring hunger is used. In view of current discussions of the post-2015 development agenda, this discrepancy highlights the need to choose carefully the indicators that are used for operationalizing any new set of goals. Better access to existing data, a more detailed coverage of nutrition-related health outcomes, and more frequent updates of GBD studies would facilitate further analyses and the monitoring of global food and nutrition security. While the disability-adjusted life years (DALYs) that are used as a health metric in GBD studies may be somewhat abstract, they can be converted tentatively into more easily understood monetary terms using per capita income figures. The resulting preferred estimate of the annual cost of global hunger in all its forms of 1.9 trillion international dollars may be better suited to illustrate the magnitude of remaining food and nutrition insecurity worldwide. Despite the progress that has been made so far in reducing global hunger, the problem is still huge and its eradication requires continued efforts.
