Book excerpt: Malignant lymphomas remain a fascinating research topic for clinicians as well as basic scientists. Rapidly increasing technical sophistication, ex panded knowledge and broader implications of new findings underline the need for a forum to integrate the latest developments in the multiple areas involved in the challenging study of lymphoid malignancies. This volume includes contributions of renowned experts and is based on a selection of papers presented at the Second International Conference on Malignant Lymphomas that was held in Lugano, Switzerland, in June 1984. Updated information is provided on various experimental fields including cell biolo gy, immunology, genetics and cell biochemistry. Particular attention has been given to special clinical entities such as the lymphomas associated with the Acquired Immune Deficiency Syndrome. Recent advances in the treat of Hodgkin's disease, ncin-Hodgkin's lymphomas and childhood lym ment phomas are highlighted with emphasis on growing experience with new therapeutic approaches including monoclonal antibodies and bone marrow transplantation. The contents of the volume reflect and translate numerous avenues of exploration. While there might be some unavoidable overlap, most appear essentially complementary. The intent of the Program Committee of the Second International Conference on Malignant Lymphomas was to promote further interactions between all those who are devoting major efforts to elucidate the nature of these diseases or their optimal therapy. This volume will hopefully amplify this objective. The Editors wish to thank the contributors for delivering the manuscripts on schedule.

Book excerpt: This volume is intended to present an eclectic selection of informative reviews on aspects of the malignant lymphomas, including Hodgkin's disease. The choice of suitable review topics in this field is made hazardous by several facts. First, the growth of insight into basic mechanisms of lymphomagenesis is explosive, as is the expansion of our understanding of the need for precise morphologic classification of these diseases. Second, new therapeutic agents, cytotoxic and biologic, as well as new drug delivery strategies, are appearing with remarkable frequency. Finally, old "standard" approaches, such as the staging laparotomy for Hodgkin's disease, are being reconsidered and deserve attention in an up-to-date collection of papers. The 10 reviews presented here fall generally into three categories. First, the optimal diagnostic and therapeutic approaches to Hodgkin's disease are considered by Mark Leibenhaut, who reviews the status of the diagnostic staging laparotomy, and by Philip Bierman, who discusses his thoughts on the timing of high-dose treatment with bone marrow transplantation for this disease. There follow four reviews of current management of specific histologic subtypes of malignant lymphoma. First, Velasquez reviews prognostic factors in both intermediate and high grade lymphomas. Miller and Dahlberg review and update the Southwest Oncology Group experience in developing curative cytotoxic combination therapies for diffuse large-cell lymphomas. Lastly, optimal therapy for two kinds of high grade lymphoma are presented. Butler and Hainsworth describe the Vanderbilt experience with diffuse small noncleaved cell lymphoma and Picozzi reviews lymphoblastic lymphoma.

Book excerpt: Over the years, the management of lymphomatous disease has followed a general trend from the purely empirical and intuitive use of medications to radiation, to surgery, and to chemotherapy with alkylating agents and anti-metabolites. In the 20th century, the treatment of these diseases has been predominantly by radiation. As radiology emerged from ROENTGEN'S original work to the sophisticated science of the present, the radiological equipment, which had been crude and inaccurate, evolved into mechanical and electronic instruments of enormous complexity and capable of great accuracy. The progress and success of radiation therapy for malig nant disease paralleled the technical development of the devices employed. For this reason, the chronology of the parallel development is given in detail. The gross and histologic descriptions of the lymphomas, from 1832 to the present, are voluminous and controversial. Therefore the pathology is reviewed and these diseases classified before reviewing therapy of the lymphomas. I. Hodgkin's Disease 1. Pathology and Classification The current classification of the so-called lymphomata into HODGKIN'S disease, lymphosarcoma, and reticulum cell sarcoma is modern. Any historical or scientific review of ,the subject rightfully begins with a discussion of HODGKIN'S original paper entitled "On Some Morbid Appearances of the Absorbent Glands and Spleen" published in 1832. This classical report describes the clinical and gross pathologic findings in a group of patients whose symptomatology presented striking similarities; notably enlargement of peripheral lymph nodes in the neck, the axillae, and the groin and, frequently, enlargement of the mediastinal nodes.

Available in PDF, EPUB and Kindle. Book excerpt: Fernando Cabanillas In 1993, Fisher et al. published the results of a randomized trial comparing three third-generation regimens against the classic CHOP combination. For several years, the oncology community had been convinced that the third generation regimens were clearly superior to CHOP. It came as a shock to many that there was no difference in outcome between the four arms of this clinical trial. The logical conclusion is that CHOP is as good as any of the other regimens tested in that study. Unfortunately, this excellent study has been misinterpreted by many as proving that there has not been any progress in the field of lymphoma during the last 20 years. Furthermore, it has led to a fatalistic attitude in the reasoning of many clinicians who feel that 'nothing works better than CHOP' and therefore that it is not worth testing new drugs or developing novel regimens. However, the process by which we move forward in the oncology field is seldom by dramatic breakthroughs. Frequently, what appears at first glance to be a breakthrough turns out later to be just a modest step forward. Several steps forward eventually add up to a major advance, but this advance goes unnoticed because of the slow nature of the process. In this volume, we have chosen to discuss several of these steps, which we feel are clearly making a positive impact on the field of lymphomas and which soon should make a major difference in therapeutic results.

MUSSHOFF The group of diseases known as malignant lymphomas are, for a number of reasons, of equal interest to theoretical and clinical medicine. New findings in the fields of virology and immunology could conceivably shed some light on the so far unknown etiology of malignant lymphomas. A viral etiology could explain many of the dis parate findings with such a puzzling disease as lymphogranulomatosis, which shows both all the signs of malignancy, such as the characteristic Reed-Sternberg and Hodgkin cells and chromosome abnormalities, and those of an inflammatory disease, i.e. typical changes in the blood count and bone marrow and in the distribution of serum proteins. So far we have not been able to identify the etiology of this disease. However, clinical observations and pathomorphological investigations involving a joint effort by different branches of medicine have brought us closer to an understanding of the clinical picture. Descriptive anatomy has given us a clear picture of the regional distribution of the lymphatic system, which has long been neglected in the past. The introductory paper by Dr. S. KUBIK (Zurich), who has made valuable contributions to our knowl edge of the anatomy of the lymphatic system, deals with this subject.

Available in PDF, EPUB and Kindle. Book excerpt: This book provides a well-illustrated, comprehensive, and up-to-date account of Hodgkin lymphoma. Epidemiology, pathogenesis, and the role of the microenvironment are examined in detail. The initial clinical evaluation and use of staging systems are fully discussed, and prognostic factors are carefully considered. The various treatment options for first-line and relapsed Hodgkin lymphoma are presented, including the newly available treatments for recurrent disease and the management required in special clinical circumstances. Given the very high cure rates and the young age of many patients, there is an increasing need to develop better individually tailored treatment strategies that will help to avoid both early and late side-effects of treatment. Such strategies receive due attention, and clear guidance is provided on the handling of Hodgkin survivors. This book will be of great value to hematologists, oncologists, and all others with an interest in Hodgkin lymphoma.

Book excerpt: In the field of Hodgkin's lymphoma, many new data have been collected during the last decade both on the cell of origin of this disease and on more effective therapies to cure the majority of pa tients even in the advanced stages. Therefore, it seems to be justi fied to compile these new data in a special volume of Recent Re sults in Cancer Research. This volume summarizes the contribu tions presented at the First International Symposium on Hodgkin's Disease that took place in Cologne (FRG) on October 2-3, 1988. There is little doubt that the Hodgkin and Reed-Sternberg (H and RS) cells and their variants represent the malignant population in Hodgkin's lymphoma; however, there is still a fierce debate as to the possible cell of origin of Hand RS cells. Many of the problems confounding earlier research into this question were related to the difficulty or virtual impossibility, of obtaining purified populations of Hand RS cells. Most of the recent progress stems from the establishment of permanent cell lines of Hand RS cells in culture.

Book excerpt: The lymphatic system plays a key role in fighting out the bacteria infested in the human body. More importantly, it aids in the fight against disease. Inside the lymphatic system, lymph vessels widen to produce lymph nodes. These lymph nodes usually appear under the arm or neck of those who are sick or infected. If cells continue to swell or multiply, they can often be considered as cancerous. Lymphoma is the actual type of cancerous cell that spreads throughout the lymphatic system. One main group of cells that spread within lymphatic system is Hodgkin's Disease, which is recognised by its growth of Reed-Sternberg cells. This book presents cutting edge research from around the world.
