Book excerpt: Using a murine model, MIF expression was examined in target organs of lupus-prone MRL/lpr mice. MIF was highly expressed in kidneys of MRL/lpr mice. There was an age-dependent increase in renal MIF expression, and the time course paralleled the reported up-regulation of other key inflammatory cytokines and chemokines. Similarly, MIF expression was significantly higher in skin lesions of MRL/lpr mice, compared to non-diseased skin. Studies to address the functional contribution of MIF in the MRL/lpr lupus-prone model were performed using two approaches: MIF inhibition and genetic deletion of MIF. A Melbourne-based drug discovery and development company, Cortical, provided access to a small molecule MIF antagonist, known as COR100140. A pilot study using COR100140 in MRL/lpr mice was performed and a trend towards reduction of proteinuria was observed. In studies presented in Chapter 5, the functional contribution of MIF in murine lupus was definitively tested by the generation of MIF-deficient MRL/lpr mice. The disease phenotype in these MIF-/- MRL/lpr mice was compared to lettermate MIF+/+ MRL/lpr mice. MIF-deficient MRL/lpr lupus-prone mice had less severe renal and skin disease and, most importantly, significantly prolonged survival. This was associated with reduced renal expression of monocyte-selective chemokine MCP-1, leading to reduced renal macrophage infiltration, and near-complete protection from glomerular crescent formation and sclerosis. Intriguingly, MIF deficiency was not associated with significant alteration in immunological abnormalities in these mice including development of autoantibodies, indicating a downstream 'effector' role for MIF independent of the autoimmune lesion. The therapeutic potential of MIF inhibition was further investigated by studying the efficacy of long-term COR100140 treatment, which did not reduce disease severity in MRL/lpr mice. The relationship between MIF and glucocorticoids were also explored but the addition of COR100140 to low dose dexamethasone did not confer any further benefit in reducing disease severity, compared to dexamethasone alone. Finally the emerging role for MIF in regulating leukocyte trafficking was studied using intravital microscopy of dermal post-capillary venules in MRL/lpr mice. Lower rates of leukocyte adhesion were found in MIF-/- MRL/lpr mice suggesting MIF may have a role in promoting leukocyte-endothelial interaction in the microvasculature of lupus affected organs. The studies produced in this thesis demonstrate that MIF has an important effector role in the pathogenesis of SLE. While results indicate that MIF does not contribute to development of autoimmunity, it promotes tissue injury by its effect on leukocyte recruitment. Therapeutic antagonism of MIF shoud be explored further in future studies.

Affecting more than 2.4 million worldwide in the second and third decades of life, it is the primary cause of non-traumatic disability in the United States. Sensory and motor neuron loss follows the demyelination of axons in the brain and spinal cord after autoreactive T lymphocytes gain access to the central nervous system and mediate an inflammatory reaction. Thus, most current therapies in multiple sclerosis seek to suppress the immune system in order to slow progression. Several cytokines have been shown to be involved in the pathogenesis of autoimmune diseases, including multiple sclerosis. We have focused on macrophage migration inhibitory factor, a ubiquitously expressed proinflammatory cytokine that has been described in a number of syndromes. Utilizing an MIF knockout model of EAE in the C57Bl/6 strain of mice, we show that MIF is required for susceptibility to EAE. MIF knockout mice are protected with a decreased incidence of disease and lower clinical scores. Additionally, we show that an inhibitor of MIF is therapeutic during ongoing disease. Administration of an MIF inhibitor reduced clinical scores. We propose several mechanisms of MIF that mediate inflammation. First, MIF inhibits the expansion of CD4+CD25+Foxp3+ regulatory T lymphocytes. This population of cells is found in higher numbers after immunization in MIF knockout and inhibitor-treated mice. CD4+CD25+Foxp+ regulatory T lymphocytes are protective during EAE through contact inhibition with autoreactive lymphocytes and the production of IL-10. Second, we propose that MIF mediates leukocyte migration into the brain and spinal cord. We show that MIF knockout mice have profoundly reduced inflammation and the administration of an inhibitor of MIF prevents new migration into the brain. Furthermore, MIF knockout recipient mice were universally protected following adoptive transfer of autoreactive lymphocytes. Finally, MIF regulates the synthesis of testosterone. MIF knockout mice have a four fold higher level of testosterone than wild type mice. Testosterone is an immunosuppressive hormone, and gonadectomy of MIF knockout mice greatly increased the incidence of EAE. We propose that MIF mediates inflammation via different mechanisms depending on the timing of disease. Inhibition of testosterone in naive mice enhances susceptibility to disease. Later changes during inflammation, including inhibition of regulatory T cell differentiation and facilitation of migration into peripheral tissues allows the progression of disease. We propose that targeting MIF during multiple sclerosis could be therapeutic through novel regulation of multiple aspects of inflammation.

Book excerpt: Macrophage migration inhibitory factor (MIF) exhibits both cytokine and chemotactic properties and is expressed by many cell types, including hepatocytes and nonparenchymal cells. Excessive alcohol abuse can lead to alcoholic liver disease (ALD), which has multiple stages of pathogenesis including hepatic steatosis and hepatic fibrosis. During steatohepatitis, several precipitating events, including triglyceride accumulation in the liver, hepatocyte damage and leukocyte infiltration, lead to further pathogenesis of ALD. Hepatic fibrosis is dependent on interactions between hepatocytes, hepatic stellate cells (HSC) and leukocytes. Recruitment of monocytes to the liver is a key contributor to hepatic fibrosis. Due to MIF's chemotactic properties, we hypothesized that MIF would be a key contributor to ALD pathogenesis. Female C57BL/6 or MIF-/- mice were fed an ethanol-containing liquid diet or pair-fed control diet for 25 days. Expression of MIF messenger RNA was induced after 25 days of ethanol feeding. After chronic ethanol feeding, chemokine expression and monocyte recruitment was increased in wild-type mice, but not MIF-/- mice. In a model of12liver fibrosis, female C57BL/6 or MIF-/- mice were challenged with carbon tetrachloride (CCI4). Expression of mRNA for MIF, as well as plasma MIF content, were increased in female wild-type mice in response to an acute dose of CCI4. a Smooth muscle actin (aSMA) mRNA and protein, indicators of HSC activation, were increased in liver of wild-type after chronic CCI4 challenge. This response was dampened in MIF-/- mice. Despite lower activation of HSC, MIF deficient mice developed similar fibrosis after CCI4 exposure when compared to wild-type mice. Inflammatory scar-associated macrophages (SAMs) were recruited to the liver from the periphery in response to chronic CCI4 in wild-type and MIF-/- mice, but later converted to a restorative SAM phenotype during fibrosis resolution. Fewer inflammatory and restorative monocytes were recruited in MIF-/- mice compared to wild-type mice. Fewer restorative macrophages were associated with lower MMP13 enzymatic activity in MIF-deficient mice. Taken together, these data indicate that MIF is an important mediator of chemokine production and leukocyte infiltration in the liver during ethanol feeding. However, during resolution of hepatic fibrosis, MIF is a key mediator for recruiting SAMs, which are critical for extracellular matrix degradation.

A variety of cells that normally circulate in blood (leukocytes, platelets) or reside within the vessel wall (endothelial cells, pericytes) or in the perivascular space (mast cells, macrophages) are activated in response to inflammation. The activation products and chemical mediators released from these cells act through different well-characterized signaling pathways to induce the phenotypic changes in microvessel function that accompany inflammation. Drugs that target a specific microvascular response to inflammation, such as leukocyte-endothelial cell adhesion or angiogenesis, have shown promise in both the preclinical and clinical studies of inflammatory disease. The aim of this handbook is to provide an authoritative volume covering all aspects of MIF, from basic molecular biology to structure-function relationships, pathophysiology, genetics, and drug development.
