L'Assistant de Régulation Médicale (ARM) est à ce jour le premier intervenant dans la prise en charge d'un patient en extra hospitalier. Il est exposé de façon quotidienne à l'urgence, à l'anxiété et au stress des patients qu'il doit savoir gérer au mieux afin de prendre des décisions justes, décisives, et parfois vitales. Cette activité engendre de l'anxiété et du stress au travail et au quotidien. L'utilisation de l'hypnose médicale Ericksonienne dans la gestion de l'anxiété, n'a plus à faire ses preuves dans le milieu du soin. On remarque également que l'hypnose médicale a un effet à la fois chez le soignant et chez le patient. Aucune étude n'a été faite à ce jour entre l'utilisation de l'hypnose médicale et les ARM sur leurs anxiétés. Le but de notre étude est de montrer que l'utilisation de la communication hypnotique et l'autohypnose par les assistants de régulation médicale lors des conversations téléphoniques permettraient de diminuer leurs anxiétés. Matériel et Méthode : L'étude CHARM est monocentrique, prospective, de type avant-après, et interventionnelle. 14 assistants de régulation médicale ont été suivi au Centre Hospitalier Universitaire Nord de la Réunion. Le critère de jugement principal est l'évaluation de l'anxiété avant et après une formation à l'hypnose médicale. Le critère de jugement principal est la diminution de l'anxiété après une formation à l'hypnose médicale. Les ARM ont réalisé un questionnaire d'évaluation de l'anxiété STAI YA sur 3 jours consécutifs en début puis en fin de vacation, deux semaines avant et deux semaines après une formation à l'hypnose médicale de 3 jours. Le critère de jugement secondaire est l'évaluation du stress via une échelle numérique de stress (EN) de 0 à 10 réalisée dans les mêmes conditions que le STAI YA. Cette étude comprend également une analyse qualitative de l'exploration de l'expérience vécue des modifications perçues du niveau d'anxiété au travail après une formation à la communication hypnotique et l'autohypnose chez les ARM par entretiens en focus groupe réalisés deux semaines après la formation. Résultat : Sur une population de 14 ARM, Nous observons une différence significative entre les moyennes du score STAI Y-A avant et après la formation (p=0,002). De plus les ARM après leur formation avaient moins de risque d'avoir une anxiété moyenne à très élevée (STAI YA >45) par rapport à avant la formation (p = 0,020). On observe une différence significative entre les moyennes du score de stress avant et après la formation (p 0,001). Les ARM après leur formation en hypnose avaient moins de risque d'avoir un stress modéré à élevé (EN 6) par rapport à avant la formation (p =0,0078). L'évaluation qualitative retrouve une diminution de l'anxiété et du stress après la formation. Elle suggère que l'hypnose médicale devrait faire partie de la formation initiale de l'ARM. L'hypnose médicale permettrait une amélioration de la qualité de travail, de la relation ARM et appelants. Conclusion : D'après notre étude, la formation des assistants de régulation médicale à la communication hypnotique et l'autohypnose a un effet bénéfique sur l'anxiété et le stress. Elle suggère que ce nouvel outil devrait etre dans la formation de l'ARM permettant de faire face aux risques psychosociaux et mettant en avant la relation humaine avec l'appelant. Une étude contrôlée avec effectif plus grand peut être envisagé au vu des résultats de l'étude CHARM.

One, concerning Controversial Aspects of Resuscitation, was later edited by its chairman, PETER SAFAR, and published in 1963. At the request of the publisher, the discussion on hypnosis has been edited in a similar manner. The participants in the discussion on Hypnosis in Anes thesiology had agreed, prior to the meeting, on a list of questions to be debated, and Dr. STOKVIS' introductory statement on the nature of hypnosis was circulated among them in appropriate translations in order to give the debate a starting point. It had also been agreed that no formal papers should be read after this introduction, and that the participants should use at will the German, English or French language. Following each contribution it was the chairman's task to give a brief summary in the two other languages. Discussion was therefore somewhat slowed down and occasionally rendered difficult by misunderstandings or the omission of details. Drs. GUEGUEN, MOSCONI, and VOLGYESI, who were not able. to attend, had sent written contributions which the chairman presented briefly during the discussion. The present text has been worked out of the tape recording of the discussion and the chairman's correspondence with the panelists some time after the meeting. Whenever the original contributions were in French or German, the English translation has been the chairman's.
